НИИ Арктической медицины ГБОУ ВПО Северный государственный медицинский университет МЗ РФ, г. Архангельск, РоссияРезюме. Ненецкий автономный округ (НАО) относится к территориям Крайнего Севера России. Округ является единственным регионом России, где до сих пор отсутствуют дороги. Экстремальные природ-ные условия, а также характер расселения жителей в НАО снижают доступность оказания квалифици-рованной специализированной медицинской помощи, как лицам коренных национальностей, так и дру-гих народностей, проживающих на территории округа, что требует использования специфических форм организации медицинского обслуживания населения. В статье представлены данные собственных про-веденных исследований на лямблиоз у различных категорий населения в НАО, полученные в результате применения практики обследования населения в труднодоступных регионах Арктической зоны Россий-ской Федерации на примере НАО при финансовой поддержке нефтяных компаний, работающих на тер-ритории округа. Материалом для исследования являлись сыворотка крови и эмульсия кала. В процессе исследования использовались следующие методы диагностики: для сыворотки крови -скрининговые иммунологические методы с использованием наборов российского производства, основанные на выяв-лении антител к антигенам лямблий в сыворотке крови, для эмульсии кала -микроскопический анализ нативного мазка, метод обогащения с использованием одноразовых концентраторов «Parasep», имму-нологические методы диагностики, основанные на выявлении специфического лямблиозного антигена в пробах фекалий с применением наборов для иммуноферментной диагностики отечественного произ-водства. Для обработки результатов использовался метод описательных статистик (средние значения, процент, ошибки среднего, построение линии тренда), выполненные в программе SPSS 20.00, Excel 2010, метод анализа данных официальной статистической отчетности лечебных учреждений по обследованию на лямблиоз, данные Управления Роспотребнадзора по НАО и данные собственных исследований в пе-риод с 2002 по 2013 год. Всего в процессе исследования выполнено 10 356 исследований на лямблиоз, за-регистрировано заболеваний по лямблиозу 3470 случаев, что составило 99,14% от общего количества за-регистрированных протозоозов и 37,4% от всех зарегистрированных на территории НАО паразитозов. Сравнение проводилось среди населения поселков округа, в которых проживают как коренные жители, ведущие кочевой образ жизни, так и представители других этнических групп, ведущих оседлый образ, с городским населением города Нарьян-Мара. В статье приведен анализ показателей заболеваемости по лямблиозу по данным окружной официальной статистической отчетности по сравнению с показате-лями заболеваемости в Российской Федерации за тот же период времени. The article is devoted to the study of functional status dynamics (working capacity, stress) of oil and gas workers of various professional groups during the shift period in Arctic conditions. The survey involved 70 specialists from oil and gas producing companies working on a shift basis in the Nenets Autonomous District of five professional groups: operators for oil and gas production, operators of the boiler and treatment facilities, drivers, engineers and maintenance technicians. The research methods are studying documentation, monitoring the work process, questioning, psycho-physiological and psychological testing, statistical methods of data analysis. The study of the functional status was carried out with the help of biochemical (analysis of saliva on cortisol), psychophysiological (complex visual-motor response) and psychological (questionnaire: “Feeling. Activity. Mood.” by V.A. Doskin and co-authors; the test of color preferences of M. Lusher in adaptation L.N. Sobchik) methods. As a result of the study, By the end of the shift period the statistically significant increase was revealed in the stressful status of the operators of the boiler and treatment facilities, maintenance workers and drivers. According to the dynamics of the functional status, one can observe a decrease in the working capacity of all specialists by the end of the shift period. The specialists, working in the open air (drivers and engineers), had more negative dynamics of functional status in comparison with other professional groups. These workers can be referred to the risk group.
In the article there is presented an analysis of the own parasitological studies, as well as indices of the prevalence of parasitic diseases according to the official statistical reports of medical institutions, Service for Supervision of Consumer Rights Protection and Human Welfare in the Nenets Autonomous District (NAD) over the periodfrom 2002 to 2013. The survey on parasitoses was performed in the indigenous population - the Nenets reindeer herders and their families, as well as in the alien population residing in the territory of the NAD settlements: Varnek, Krasnoe, Karatayka, Nes, Haruta, Norey-Ver, Iskateley, village Oma, and city of Naryan-Mar. During this period, by means of the method of the native smear there were surveyed 5891 cases, method of Kato - 217,417 persons, by perianal scraping - 3054 persons, by ELISA for the presence of specific antibodies to the antigens of various parasites - 11556 cases. The statistical analysis was performed with the use of Statistical Software Package - Excel 2010. There were revealed both downward trends in the prevalence of the county's population for giardiasis, enterobiasis, diphyllobothriasis and the gain in indices of prevalence of ascariasis in the population of the District, also there were detected and found new types ofparasitoses for county - opisthorchiasis, toxocariasis in all age groups of the population of NAD.
Importance of parasitic diseases for public health appears from their diverse and profound effects on human bodies. Being a common human disease and frequent pathology, parasitoses burden course of many somatic and infectious diseases and cause reduction of labor activity and long-term disability. In the article, there have been presented the most common parasitoses in the Nenets Autonomous Area among Nenets patients and persons of other ethnic origin living in the Area, the City of Naryan-Mar and settlement Iskateli.
