The prevalence of infertility has risen 2.9 times in recent years in Ukraine. The use of assisted reproductive technologies (ART) is needed. The State program for infertility treatment by budget funds was approved in Ukraine since 2004. However, funding for this program is enough only for 500–600 cycles per year, but ART centers conducted more than 27,000 cycles in 2018. This means that many women have to pay out-of-pockets. Our research has shown that between 2000 and 2018, the number of ART clinics and the number of cycles, pregnancies, and children born increased significantly in Ukraine. The rate of ART cycles per 1 million population in Ukraine has increased from 226.9 to 655.3, but it is lower than the WHO recommended rate 800–1000 cycles per 1 million population. Changes in the structure of ART cycles for 2000–2018, the increase of ICSI, and egg donation cycles are shown. It requires an adequate supply by hormonal drugs for ovarian stimulation in ART centers. Since 2004 Ministry of Health of Ukraine annually approved the list of medicines for State program of ART. Only 8 drugs were purchased by the state funds, which amounted for USD 227 438 in 2017, 2018. A retrospective analysis of drug prescriptions in short and long stimulation protocols was performed. The frequency of drug prescriptions according to the ATC-classification based on medical records was determined, all prescriptions were accordance with the requirements of ART treatment standards in Ukraine. The costs of the three hormonal stimulation schemes, which were the most prescribing in ART center, were calculated. It was found that costs for hormonal schemes increased by 22–54% during 2015–2020, it significantly reduces the availability and affordability of ART for the population in Ukraine.
Мета роботи. Вивчення впливу 7-ми кількісних факторів на властивості мас для таблетування і основні показники таблеток при розробці нового комбінованого таблетованого лікарського засобу з гіпоглікемічною активністю на основі сухих екстрактів листя чорниці і трави козлятника в комбінації з таурином. Матеріали і методи. Активними фармацевтичними інгредієнтами були сухі екстракти листя чорниці і трави козлятника, таурин, допоміжні речовини (ДР). Дослідження впливу 7-ми кількісних факторів ДР на такі показники, як процес пресування, однорідність маси таблеток, стійкість таблеток до роздавлювання, стираність і розпадання таблеток екстрактів листя чорниці і трави козлятника та таурину методом прямого пресування проводили відповідно до вимог Державної фармакопеї України (ДФУ), 2 видання. Досліджували також процес заповнення матриці таблетної машини порошковою масою для таблетування та середню масу зпресованих таблеток як показники, що найбільше характеризують придатність порошкової маси до прямого пресування. Результати й обговорення. За допомогою методу випадкового баласу вивчено вплив кількостей 7-ми ДР на основні показники порошкових мас і таблеток екстрактів листя чорниці і трави козлятника та таурину. Для кожного із показників будували діаграми розсіювання. Аналіз діаграм розсіювання показав, що із збільшенням кількості лактози покращується однорідність маси таблеток та зменшується час розпадання. Із збільшенням кількості маніту погіршується однорідність маси таблеток та збільшується час розпадання. Збільшення кількості натрій карбоксиметилкрохмалю в складі таблеток веде до суттєвого погіршення однорідності маси і в межах вивчених інтервалів зменшує час розпадання. При збільшенні кількості неусіліну US 2 в складі таблеток екстрактів листя чорниці і трави козлятника та таурину покращується однорідність маси, підвищується стійкість до роздавлювання, зменшується стираність, проте дещо збільшується час розпадання. Збільшенння кількості кальцій карбонату в складі таблеток веде до погіршення однорідності маси, підвищення стійкості таблеток до роздавлювання, зменшення їх стираності, однак призводить до суттєвого збільшення часу розпадання. Збільшення кількості натрій кроскармелози в складі таблеток покращує однорідність дозування та суттєво зменшує час розпадання таблеток. Висновки. За допомогою методу випадкового балансу вивчено вплив 7-ми кількісних факторів на фармако-технологічні показники таблеток екстрактів листя чорниці і трави козлятника та таурину. За сукупністю основних показників кращими ДР для створення таблеток на основі екстрактів листя чорниці і трави козлятника в комбінації з таурином відібрано таблетозу, неусілін US 2, натрій кроскармелозу, МКЦ 200 та магній стеарат.

Серйозною проблемою є розвиток нудоти та блювоти вагітних (НБВ), особли-во тяжкої форми, що суттєво впливає на якість життя жінки, її соціальні функції, а також на здоров'я майбутньої дитини [3,6,8, 15, 16, 25]. Фармакоекономічні дослідження показали, що витрати на лікування ускладнень при вагітності, зокрема НБВ, у США становлять щорічно понад 1,7 млрд доларів (2012), вра-ховуючи прямі витрати охорони здоров'я і непрямі витрати, пов'язані з втраченою роботою або потребою у догляді та допо-мозі інших осіб (членів родини) вагітним жінкам у домашніх умовах [30].Встановлено, що протягом 2012 року у США було надано більш ніж 240 тис. кон-сультацій при НБВ та її тяжкій формі і ви-трачено на оплату близько 250 млн до-ларів, тобто витрати є досить великими. Зростання витрат на лікування НБВ прямо пропорційно збільшенню тяжкості перебі-гу НБВ і, відповідно, витрат на препарати 2 ряду призначень, зокрема метоклопра-мід та ондасетрон [31]. Зниження витрат на лікування НБВ можна досягнути шля-хом зменшення симптомів і рецидивів НБВ, повторних консультацій лікаря та госпіталізацій. Н. М. is an equally efficacious alternative to urinary derived FSH, Menopur®. One differences between these products is their delivery devices; Gonal f® is provided as a multidose pen, Menopur® in vials or multidose preparation, and Bemfola® as a fixed dose pen. The aim of this study was to determine the impact of delivery device on drug wastage and associated cost. Methods: A retrospective analysis of Gonal-f® and Menopur® prescription and usage data from five UK infertility clinics was conducted to identify the level of wastage occurring from IVF cycles. Data collected included: number IVF cycles conducted in previous 12 months; daily dose; length of treatment; dose adjustment following ultrasound scan and FSH formulation(s) prescribed. The drug wastage for each patient was calculated and results compared to the potential Bemfola® wastage for these patients. The wastage cost was then determined using NHS list prices. Results: A total of 4724 IVF cycles were incorporated into the analysis. Of the 4078 Gonal-f® cycles, overall drug wastage was 650,775IU (5.7% of total dose prescribed) equating to 160IU per patient. For the 646 Menopur® cycles the wastage was 190,163IU (11.6% of total dose prescribed) equating to 294IU per patient. Had Bemfola® been used in these patients, the wastage would have been reduced to 104IU and 61IU per patient respectively. The use of Bemfola®, across all cycles, results in a drug wastage reduction of 376,800IUs and an associated cost saving of £100,011. Extrapolating this data to the annual number of UK IVF cycles results in a potential cost saving of £1,157,579. ConClusions: Using Bemfola® as an alternative to Gonal-f® or Menopur® can potentially reduce drug wastage and associated costs to both patients and the NHS.
