A contribution of the volatile oil from ginger rhizomes (Zingiber officinale Roscoe) on inhabiting the 5-HT3 receptor complex had been shown. In the present study a possible interaction of some compounds of the volatile oil with the 5-HT3 receptor system expressed in N1E-115 cells and with the serotoninergic system of the rat ileum was investigated. The volatile oil was obtained by steam distillation and fractionated using a silica gel column resulting in five fractions. Compounds of the fractions were identified by GC-MS. The influence of the volatile oil, its fractions and pure components on serotonin-induced [14C]guanidinium influx into N1E-115 cells was measured indicating the inhibitory interaction with the 5-HT3 receptor channel system. Most potent inhibitors of cation influx were the volatile oil, fraction 4, beta-pinene, terpinolene, alpha-copaene and alpha-phellandrene. The volatile oil and fractions 1 and 4 were not able to significantly influence either serotonin (10 microM)-induced maximum contraction of the rat ileum or the second phase of the biphasic contraction 2.5 min after serotonin addition. However, beta-pinene, terpinolene and alpha-phellandrene reduced both contractions. In conclusion, the volatile oil and distinct compounds such as terpinolene, beta-pinene and alpha-phellandrene interact with 5-HT3 receptor channel system and possess an antispasmodic effect at the rat ileum.

Die erwerbsbezogene motorische Leistungsf?higkeit kann messtechnisch nicht direkt erfasst werden. Die Ergebnisse der ?blichen kurativ-medizinischen Diagnostik korrelieren nur unzureichend mit den tats?chlichen Auswirkungen k?rperlicher Beeintr?chtigungen (Handicap) auf die tats?chliche Leistungsf?higkeit. Assessmentsysteme dienen der Objektivierung des Leistungsverm?gens und Steuerung zielgerichteter Hilfe zur Teilhabe (Participation). Neben den Kontextfaktoren der ICF ist die Leistungsbereitschaft der Probanden bei der Testdurchf?hrung von motivationalen Faktoren beeinflusst. Ausgehend von kasuistischen Einzelbeobachtungen lie? sich ein erh?htes Auftreten negativer Befundverzerrungen bei FCE-Tests bei sozio?konomischen Problemlagen vermuten. Standardisierte Assessmentsysteme beinhalten Konsistenzpr?fungen um festzustellen, ob die beim Test gezeigte Leistung (Performance) unterhalb des tats?chlichen Leistungsverm?gens (Capacity) liegt. In Deutschland ist das EFL-System nach Isernhagen (IWS FCE) am weitesten verbreitet. In einer Analyse wurden 310 EFL-Testprotokolle station?rer orthop?discher Reha-Patienten ausgewertet. Dabei zeigte sich, dass in Abh?ngigkeit vom Erwerbsstatus bei Rentenantragstellern der Anteil schmerzbedingter Selbstlimitierungen auf 50% anstieg gegen?ber 9% bei vorhandenem Arbeitsplatz. Die Leistungsbereitschaft wurde nur bei 42% der Rentenbewerber als zuverl?ssig eingesch?tzt gegen?ber 86% bei den Erwerbst?tigen. Eindeutig inkonsistentes Verhalten wurde in der Rentengruppe bei 29% gegen?ber 8% der Erwerbst?tigen beobachtet. Arbeitslose zeigten nur wenig schlechtere Werte als die Erwerbst?tigen. Diese Unterschiede waren statistisch signifikant (alle p-Werte 0.01). Unabh?ngig von zugrunde liegender Erkrankung und tats?chlichen Funktionsbeeintr?chtigungen f?hrte offenkundig bereits der Entschluss zur Rentenantragstellung zur motivationalen Einschr?nkungen und verf?lschten Leistungsergebnissen. Auch unter Ber?cksichtigung der noch nicht abgeschlossenen kontroversen Diskussion ?ber die Wertigkeit des Verfahrens erlaubt aus unserer Sicht das FCE-Verfahren nach Isernhagen unter Anwendung der vorgegebenen Konsistenzpr?fung mit der ?rztlichen Befundanalyse die zuverl?ssige Erfassung von Selbstlimitierungen.
The dynamic hip screw (DHS) is a widely used method of osteosynthesis. Nevertheless, supplying instable pertrochanteric fractures is problematic because the gliding screw may cause a considerable dislocation of fragments in lateral direction. Is there any alternative method disposable, bearing stability of DHS- osteosynthesis can be improved by adding a lateral T-plate. We reached full bearing stability in four cases.
