The atypical hemolytic uremic syndrome (aHUS) is the most complex disease in the diagnosis, treatment and prognosis among the thrombotic microangiopathy. The article describes the clinical case of a 32-year-old woman with an aHUS in 32 week of gestation. It is given the diagnosis and treatment of this case till clinical convalescence.Observation within 1 year after the episode of the aHUS is showing up a chronic kidney failure, which will provide renal replacement therapy in future.
Currently, the concern of endometrial dysfunction is still relevant and is of increasing interest. This is because of new factors have been identified that affect the reproductive function in women. Chronic endometritis and endometriosis mainly affect women of childbearing age, which threatens normal cyclic transformation of the endometrium and tissue receptivity. This consequently affects the main function of the endometrium, which is to ensure successful implantation of the fertilized egg and the development of the chorion and embryo. A detailed understanding of the functioning of the endometrium with a combination of chronic endometritis and endometriosis will improve their diagnosis and treatment.

The analysis of the results of a retrospective study revealed the regional features of the emerging role of opportunistic flora in the development of inflammatory diseases of the pelvic organs in women, as well as the spectrum of resistance of the detected pathogens to antimicrobial drugs.Key words: inflammatory diseases of the pelvic organs, microbiota, antibiotic resistance.Одной из наиболее часто встречаемых и социаль-но значимых проблем в акушерстве и гинекологии яв-ляются воспалительные заболевания органов малого таза (ВЗОМТ). Частота этой патологии стабильна и со-ставляет до 50 % среди амбулаторных и стационарных больных [2, 6].Пусковым механизмом в развитии воспалений женских половых органов служит воздействие микроб-ного фактора. Следует отметить, что в настоящее вре-мя наблюдается тенденция полимикробного синергиз-ма патогенных и условно-патогенных микроорганизмов в этиологии ВЗОМТ, что при нарушении барьерных ме-ханизмов защиты и локального противоинфекционного иммунитета может способствовать хронизации заболе-ваний и активации аутоиммунных процессов [1, 2, 6]. Известно, что видовой состав возбудителей ВЗОМТ может существенно различаться в разных регионах мира, где они могут проявлять и различный уровень резистентности к рекомендуемым для лечения этой па-тологии антибактериальным препаратам. Это подтвер-ждается и неодинаковой клинической эффективностью одних и тех же режимов антибактериальной терапии ВЗОМТ [3][4][5].Наряду с отсутствием снижения частоты ВЗОМТ и ростом случаев самолечения отмечается развитие ле-карственной устойчивости к большинству антибиотиков, а также рост бета-лактамазных штаммов микроорганиз-мов, образование L-форм, биопленок, на фоне эндо-и экзогенной супрессии, что безусловно, значительно ос-лабляет эффективность стандартных лечебных схем и диктует необходимость поиска альтернативных терапев-тических подходов терапии ВЗОМТ.

Atypical hemolytic-uremic syndrome being referred to autoimmune diseases appears to be one of the most severe forms of thrombosis and microangiopathy. Difficulties in diagnosis cause a delay in pathogenetic therapy, which causes a high maternal mortality rate. Pathogenetic therapy with eculizumab, suppressing the terminal activity of complement, may contribute to the recovery of blood coagulation properties, but the long-term prognosis for chronic renal failure remains unfavorable. The article deals with a clinical case confirming this conclusion.

The goal of our study was to examine local and serum cytokine level involved in regulating inflammation in patients with chronic endometritis combined with endometrial hyperplastic processes. On admission, all patients underwent hysteroscopy with separate diagnostic curettage followed by histological examination of samples isolated from the uterine and cervical canal mucosa. Such manipulations were indicated due to abnormal uterine bleeding as well as suspected endometrial pathology based on ultrasound examination. According to the histological examination data of the endometrial samples, all patients were divided into two groups: Group I contained 45 women with CE combined with PEG without atypia; Group II - 38 patients with morphologically verified CEE combined with AEG without atypia. Level of IL-1β, IL-2, IL-6, γ-INF, TNF-α in biological fluids (aspirate from the uterus; serum) was measured by using enzyme-linked immunosorbent assay. We found that in both groups (91.1% and 89.6%, respectively) the vast majority of patients was hospitalized due to abnormal uterine bleeding. Oligomenorrhea alternated with intermenstrual bleeding (66.7% and 71.2%, respectively) and dominated in pattern of menstrual cycle disorders in the examined patients, whereas 11 (24.4%) and 7 (18.4%) patients from group I and II, respectively, were noted to suffer from severe menstrual bleeding.Overall, analyzing the data on cytokine level both in the uterine aspirate and serum evidences about ongoing inflammatory process found at examination time point. Upon that, such process was not only local, but also exhibited signs of a systemic inflammatory response. The data on cytokine level in the uterine aspirate from patients with CE coupled to PGE or CGE without atypia point at local inflammatory process characterized by significantly increased concentration of IL-1β, IL-2, IL-6, TNF-α and IFN-γ. At the same time, higher level of IL-1β and IFN-γ in patients from group II might indicates that degree of morphological changes in the endometrium could affect the level of local cytokine production. Thus, the data obtained evidence that immune changes in chronic endometritis combined with non-atypical endometrial hyperplastic processes mostly occur locally. In this regard, measuring cytokine concentration in the uterine aspirate is a diagnostic predictor and serves as a sign for monitoring therapeutic effectiveness of therapy in this cohort of patients.
