Dünya Sağlık Örgütü, pestisitler için tarım zararlıları olan her türlü böcek, kemirgen, mantar ve yabani otların öldürülmesi için kullanılan kimyasallar tanımını yapar. Bu çalışmanın amacı, Türkiye'de yapılmış ve ulusal literatürde yayımlanmış, gıda numunelerinde pestisit kalıntısı araştıran 2010 yılı ve sonrası makalelerin sistematik derlemesini yapmak ve gıda numunelerinde pestisit kalıntısı bulunma durumunu ortaya koyabilecek kanıtlar elde edebilmektir. İzmir Katip Çelebi Üniversitesi Elektronik Kütüphane kaynakları ve Google Akademik'te yapılan tarama sonunda, dahil etme kriterlerine uygun toplam 35 kayda ulaşılmıştır. Bunların 16'sında (%45,7) pestisit kalıntısı bulunduğu ve Maksimum Kalıntı Düzeyleri (MRL) değerinin aşıldığı; 10'unda (%28,6) pestisit kalıntısı bulunduğu ancak MRL değerlerinin aşılmadığı tespit edilmiştir. Çalışılan gıda numuneler çeşitlilik gösterse de üzüm, çilek, turunçgiller ve sık tüketilen bazı sebzelerde pestisit kalıntısı fazlaca çalışılmıştır ve pestisit kalıntısına rastlanmıştır. Pestisit kalıntısına rastlanmayan gıdalar içinde süt ve bal dikkati çekmektedir. Konu üzerine mevzuatın gözden geçirilmesi ve caydırıcı cezaların getirilmesinin yanı sıra konu hakkında toplum ve iş sağlığına yönelik eğitim çalışmalarının arttırılması önerilir.
Bu çalışmanın amacı; Türkiye'de geleneksel ve tamamlayıcı tıp (GETAT) uygulamalarını sağlık politikalarının gelişimi açısından ele almak ve bu uygulamaların etik boyutunu değerlendirmektir. Türkiye'de 2000'li yılların başında uygulanmaya başlanan "Sağlıkta Dönüşüm Programı" ile gelen ve reform adıyla sunulan uygulamalar GETAT uygulamalarının da hukuksal dayanağını kuvvetlendirmiştir. 663 sayılı Kanun Hükmünde Kararname'ye de dayanarak hazırlanan Geleneksel ve Tamamlayıcı Tıp Uygulamaları Yönetmeliği (tarih: 27 Ekim 2014 ve sayı: 29158) Resmî Gazete'de yayımlanarak yürürlüğe girmiştir. Bu çalışmada, GETAT için etik boyut da değerlendirilmiştir. Türkiye'de GETAT tıbbi müdahalelerin hukuka uygunluğunun tespiti için mevzuat yetersizdir. Geleneksel ve Tamamlayıcı Tıp alanında yapılacak bilimsel çalışmaların İlaç ve Biyolojik Ürünlerin Klinik Araştırmaları Hakkında Yönetmelik kapsamında yapılması öngörülmektedir. Bu araştırmalar etik kurullar tarafından değerlendirilmeli, Helsinki Deklarasyonu'na uygun olarak planlanmalı ve yürütülmelidir. Bizim görüşümüze göre GETAT alanında kanıt düzeyini en yukarıya taşımak için sistematik derleme ve meta-analiz çalışmalarının yapılarak yayımlanması gereklidir. Etik konulardan bazıları: Hastalık için tanı, konvansiyonel tıbba göre konulmalıdır. Standart tedavi varken GETAT uygulamasını tercih etmek tıbbi standarttan sapma anlamına gelmektedir, ancak bu sapma tek başına doktorun tedaviyi uygulama hatası anlamına gelmemektedir. Standart tedavi varken GETAT tercih eden hekim için özen yükümlülüğü ve aydınlatma yükümlüğü ile tedavinin gerekçelendirilmesi hususları önem arz eder. GETAT alanında Türkiye'de bilimsel kanıtların artırılması ve etik alanında eksikliklerin giderilmesi için çalışmaların sürdürülmesi gerekmektedir.Anah tar Ke li me ler: Tıp; geleneksel; etik kurallar; Türkiye ABS TRACT The aim of this study is to consider traditional and complementary medicine (TCM) practices in Türkiye in terms of the development of health policies and to evaluate the ethical dimension of these practices. The applications that came with the "Health Transformation Program", which started to be implemented in the early 2000s in Türkiye and presented as reform, also strengthened the legal basis of TCM applications. The Regulation on Traditional and Complementary Medicine Practices (date: October 27, 2014 and number: 29158), which was also prepared based on the Decree-Law No. 663, was published in the Resmî Gazete and entered into force. In this study, the ethical dimension for TCM was also evaluated. Legislation is insufficient to determine the legality of TCM medical interventions in Türkiye. It is foreseen that scientific studies to be carried out in the field of Traditional and Complementary Medicine will be carried out within the scope of the Regulation on Clinical Trials of Pharmaceuticals and Biological Products.
