Vajinismus hastalarında depresyon ve anksiyete bozuklukları tanı oranlarına ilişkin bilgi eksikliği göze çarpmaktadır. Bu çalışmanın amacı vajinismus tanısı konulmuş kadınlarda depresyon ve anksiyete bozukluklarının yaygınlığını araştırmak ve bu bozuklukların cinsel işlevlerle olan ilişkisini incelemektir. Yöntem: Vajinismus tanısı konulmuş olan yüz kırk dört kadın çalışmaya dahil edildi. Katılımcılara DSM-IV Eksen I Bozuklukları için Yapılandırılmış Klinik Görüşmenin (SCID-I) anksiyete bozuklukları ve depresyon bölümleri ve Golombok-Rust Cinsel Doyum Ölçeği (GRCDÖ) uygulandı. Bulgular: Olguların %79,86'sında en az bir anksiyete bozukluğu ve/ veya depresyon ek tanısı saptandı. Hastalar arasında en yaygın olarak saptanan ek tanı özgül fobi (%63,9) idi. Bunu sırasıyla depresyon (%35,4), sosyal anksiyete bozukluğu (%13), panik bozukluğu (%10), obsesif kompulsif bozukluk (%5) ve yaygın anksiyete bozukluğu (%2) tanıları izledi. Ek tanısı olmayanlarla karşılaştırıldığında depresyon ek tanısı olan hastalarda GRCDÖ kaçınma puan ortalaması, panik bozukluğu ek tanısı olanlarda ise iletişim puan ortalaması daha yüksek bulundu. Sonuç: Vajinismus hastalarında majör depresyon ve özellikle özgül fobi olmak üzere anksiyete bozuklukları yaygınlığı genel topluma göre yüksek oranlarda saptanmıştır. Hem psikiyatrik tanıların yüksek oranda eşlik etmesi hem de cinsel işlevselliğin her alanda bozulmuş olması bu hastaların klinik değerlendirilmesinde bütüncül yaklaşımın önemini vurgulamaktadır.
Based on our study results, we recommend clomipramine in combination with psychotherapy as the treatment of choice in PGAD and to be used before any invasive procedure such as electroconvulsive therapy (ECT) or surgery.
