BackgroundScabies is a common parasitic infection caused by the mite Sarcoptes Scabiei. About 300 million cases of scabies are reported annually. Scabies usually presents clinically with an erythematous excoriated papulovesicular rash, burrows, nodules and hyperkeratotic lesions in specific body areas.A rare presentation of scabies is the bullous pemphigoid-like bullous scabies. So far, to the best of our knowledge, only 32 cases of bullous scabies have been reported in medical literature, of which only 11 were under 60 years of age at the time of initial presentation. This is the first case of bullous scabies being reported from Pakistan.Case presentationHerein we discuss, with reference to the existing literature, the case of a 23-year-old Punjabi male who presented with a 3 day history of a tense, non-erythematous, non-tender bulla measuring approximately 0.5 cm x 0.8 cm on the right foot near the interdigital cleft. He was diagnosed to have bullous scabies.ConclusionThe diagnosis of scabies should be considered in all patients who present with tense bullous lesions accompanied by pruritus and a maculopapular rash. This is particularly relevant if these lesions do not resolve with steroid treatment. In such patients, in order to prevent a misdiagnosis of bullous pemphigoid, scrapings for Sarcoptes Scabiei mites and eggs should be taken.
Introduction: Plantar fasciitis is a painful ailment that causes frustration to both the patient and physician. Stretching exercises targeting the plantar fascia are an excellent therapy option for plantar fasciitis. Objective: To compare the outcome of a gastrocnemius-soleus stretching program versus tendo Achilles stretching exercises for the management of chronic plantar fasciitis. Methods: Patients aged 30-70 years of either gender with chronic plantar fasciitis were included and randomly divided into two groups. In group A, the gastrocnemius-soleus stretching program was applied, whereas in group B, the tendo Achilles stretching exercises were adopted. The pain score was determined at baseline and after eight weeks, and the change in pain score was calculated. All information was noted in the proforma and then entered and analyzed in the Statistical Package for the Social Sciences (SPSS) software version 21 (International Business Machines (IBM), New York, United States). An independent-samples ttest was conducted to compare the mean change in pain score in both groups. A P value of <0.05 was considered significant. Results: The mean age of the patients in the gastrocnemius-soleus stretching group was 48.70 ± 9.80 years, whereas that in the tendo Achilles stretching exercises group was 48.63 ± 8.43 years. Group A included 16 (53.3%) men and 14 (46.7%) women, whereas in group B, there were 15 (50%) men and 15 (50%) women. The mean change in pain score in group A was 2.57 ± 1.01, whereas that in group B was 1.77 ± 0.57. The difference in both groups was significant (P < 0.05).Conclusion: Gastrocnemius-soleus stretching exercises are more effective for reducing the symptoms of plantar fasciitis in the adult population.
Abstract:This study endeavored to find out the effect of financial variables on bank performance pre and post financial crisis of 2008 in Pakistan. Using regression analysis the study revealed that financial crises of 2008 posed a significant influence on the performance of conventional and Islamic banks in Pakistan and pronounced a negative relationship between financial crises and bank performance. The Capital adequacy, assets quality, management quality, liquidity quality, earning quality and bank size posed a negative influence on bank performance during crises while NIM / NNIM showed no effect.
