Diabetic foot ulcer (DFU) is a form of chronic wound which becomes a serious complication in diabetes mellitus (DM). Recently, the role of vitamin D on T cell-mediated immunity, pancreatic insulin secretion, and its mechanism on cell growth and healing processes have been reported. This study aims to compare the vitamin D level of DM patients with DFU and without DFU to assess the duration and severity of DFU and its correlation with vitamin D levels. The sociodemographic characteristics and DFU duration were documented.The severity was examined in accordance with PEDIS classification. 25-hydroxyvitamin D (25[OH]D) was analysed using in-vitro chemiluminescent immunoassay (CLIA). Statistical analysis was performed and the P-value <.05 was considered as statistically significant. The vitamin D levels in DM patients with and without DFU were 8.90 ng/mL (6.52-10.90) and 16.25 ng/mL (13-19.59), respectively, with P < .001. There was no correlation between the duration of DFU and DFU severity by PEDIS score with vitamin D levels.Vitamin D levels in DM patients with DFU are lower than those in patients without DFU. However, there was insufficient evidence to conclude that there is no correlation between the DFU duration and DFU severity by PEDIS score with vitamin D levels.
Scabies is one of the most prevalent infectious skin diseases in Indonesia, especially in boarding schools. Diagnosis of scabies can be made using the four cardinal signs through anamnesis and simple physical examination. The high incidence of scabies requires collaboration between various parties to diagnose and treat scabies. “DeSkab” (scabies detection form) is a form containing questions for non-medical personnel to detect scabies before being confirmed by trained medical personnel. Face and content validity of “DeSkab” is first established by boarding school’s supervisors, experts in community medicine, and dermatovenereologists. To establish reliability, study is conducted in Al-Hidayah Islamic Boarding School, Indonesia. Six non-medical personnel were chosen to examine 81 students of the boarding school using “DeSkab”. Each student is examined by three non-medical personnel, then confirmed through an examination by dermatovenereologists. Result of McNemar test shows that scabies detection by five nonmedical personnel have no statistically significant difference compared to the dermatovenereologist (McNemar test value of P>0.05). Based on this result, “DeSkab” proves to be an option for non-medical personnel to use as an instrument for detecting scabies in Indonesia.
ABSTRAKSifilis merupakan infeksi menular seksual (IMS) yang diketahui berhubungan dengan peningkatan risiko infeksi human immunodeficiency virus(HIV) dan begitupun sebaliknya. Hingga saat ini, deteksi sifilis dengan atau tanpa koinfeksi HIV menggunakan tes serologi masih menjadi pilihan utama, terutama di negara-negara berkembang. Tes serologi yang rutin digunakan meliputi tes nontreponema dan treponema. Dalam menegakkan diagnosis, klinisi seringkali merujuk pada tes serologi tersebut. Hasil tersebut perlu menjadi perhatian karena manifestasi klinis dan tes serologi sifilis pada HIV sering tidak biasa. Hal ini diduga akibat respons imun pada infeksi HIV yang dapat mengubah perjalanan alamiah sifilis. Beberapa hasil tes serologi sifilis pada koinfeksi HIV dapat menunjukkan hasil negatif palsu, terjadinya serokonversi, dan adanya fenomena serofast. Selain itu, perlu juga dilakukan tes penapisan serologi sifilis secara rutin pada koinfeksi HIV dan untuk mengevaluasi efektivitas terapi. Berbagai pola serologi sifilis pada koinfeksi HIV yang tidak biasa tersebut perlu diantisipasi dengan baik sehingga tata laksana dapat berjalan efektif, dan diharapkan mengurangi risiko penularan terutama pada kelompok dengan risiko tinggi.
