The choice of artemisinin-based combination that is being adopted for malaria treatment in sub-Saharan Africa may depend on several factors, including cost, efficacy, side effects, and simplicity of administration. We tested the hypothesis that artesunate-sulfamethoxypyrazine-pyrimethamine is as efficacious as the four-dose regimen of artemether-lumefantrine for treatment of Plasmodium falciparum malaria. The study was carried out during two transmission seasons (2003 and 2004) in Sotuba, Mali. Participants at least 6 months of age with uncomplicated P. falciparum malaria were randomly assigned to receive artesunate-sulfamethoxypyrazine-pyrimethamine or artemether-lumefantrine. Treatment efficacy was assessed using the World Health Organization 28-day protocol. A total of 606 (303 in each arm) patients were enrolled. The cure rate was higher for artesunate-sulfamethoxypyrazine-pyrimethamine than for artemether-lumefantrine (98.7% versus 89.6%; P < 0.0001). After correction for cases of re-infection, the cure rates were 100% and 99.0%, respectively (P = 0.08). No serious adverse events occurred. Artesunate-sulfamethoxypyrazine-pyrimethamine is well-tolerated and effective against P. falciparum malaria. It showed an additional benefit of preventing new infections.

RESUMEObjectifs : Cette étude examine l'état de conservation de la relique forestière de l'Université d'AboboAdjamé et son importance dans la préservation des espèces. L'impact de la pression humaine sur la relique est évalué par une estimation des pressions aussi par l'utilisation des fourmis comme bioindicateurs de la perturbation induite par ces pressions. Méthodologie et résultats: Neuf types de pressions anthropiques ont été retenus pour comparer les îlots forestiers de la relique. Les fourmis ont été échantillonnées en utilisant trois types de méthodes adaptées aux trois strates de la forêt (sol -litière -végétation). Les résultats montrent que l'îlot 2 subit la plus grande pression, suivi de l'ilot 3 (plus grand) tandis que l'îlot 1 (plus petit) ne subit que de faibles pressions. L'îlot 2 a le plus faible nombre d'espèces de fourmis ; cependant l'îlot 3 est le plus riche. Un grand nombre d'espèces existe dans la relique mais elles sont caractérisées par de faibles abondances. Conclusions et application des résultats : Ce travail a permis non seulement d'obtenir une classification des îlots forestiers de la relique de l'Université d'Abobo-Adjamé selon leur degré de perturbation mais aussi de déterminer les pressions majeurs qui menacent ces îlots. Les résultats indiquent également que la relique est devenue un refuge pour les fourmis et que la plupart des espèces y ayant trouvé refuge, s'installent dans la litière. Les résultats de cette étude semblent précieux en ce sens qu'ils mettent en exergue l'importance de la relique pour la conservation de la biodiversité au sein de cette université dédiée à la protection de l'environnement. Ils serviront d'outil de décision en permettant à l'autorité Universitaire de mieux définir les actions prioritaires en vue de la restauration et de la conservation de la relique forestière.Impact of fragmentation and human pressure on the remnant forest of Abobo-Adjamé University (Côte d'Ivoire) ABSTRACT Objectives: This study examines the conservation state of the forest relict of Abobo-Adjamé University and its importance in species preservation. The impact of human pressure is evaluated through an estimation of these pressures. In addition to that, ants are used as bioindicators to assess perturbation within the relict. Methodology and results: Nine types of anthropic pressures were used to compare the forest fragments within the relict. Three different methods were used to collect ants from the three main strata of the forest (soil -liter -vegetation). The results showed that forest island 2 is subject to a relatively greater pressure and it has the lowest species richness. We explore how the therapeutic landscape of Bangui, capital of the Central African Republic, shaped and reflected caregivers' home care of childhood diarrhea. Using interviews, group discussions, and participant observation, we found that caregivers described several categories of diarrhea, but in practice, adopted an experimental approach to home diarrheal care. Many managed incertitude by initially dosing children with street medicines and herbal infusions; they delayed seeking professional medical care to avoid expenses, observed their children's symptoms, consulted social networks, and used therapeutic foods. The logics underpinning these practices emerged from caregivers' conviction that diarrhea necessitated medical therapy and restricted choices within Bangui's therapeutic landscape, a consequence of lengthy political economic crisis. This crisis impoverished Bangui populations, eroded formal health care, and sharpened religious differences, discouraging care seeking from traditional healers. Analyses of therapeutic landscapes illuminate why caregivers embraced specific practices and logics and should guide the elaboration of more effective public health interventions.
Le conflit armé qui a éclaté en Côte d'Ivoire le 19 septembre 2002 a conduit à la partition du pays entre le Nord et le Sud. La rébellion des Forces Nouvelles qui contrôle la moitié nord du pays a justifié son action par la dénonciation de l'exclusion et des injustices sociales dont les « nordistes » seraient les victimes. Cette rébellion a été rejointe par de nombreux jeunes originaires du Nord. Cette étude basée sur les études de cas et biographies de 22 jeunes combattants de la rébellion et des entretiens avec leurs leaders montre les raisons justifiant leur engagement. Elle questionne leurs perceptions du conflit et analyse les rhétoriques de la justification. Il ressort que dans l'ensemble des cas, l'engagement a pour but la reconnaissance de leur citoyenneté qui passe par l'acquisition de cartes d'identités. Le sentiment victimaire né de l'exercice de la violence légale par les forces de l'ordre, la vengeance, le besoin de sécurité ou encore l'attrait pour le métier des armes nourrissent les motivations qui conduisent à l'enrôlement des jeunes. L'étude montre aussi l'existence d'un environnement s'appuyant sur la parenté, un discours culturel et la mobilisation de la mémoire ancestrale collective qui ont favorisé l'adhésion des jeunes nordistes au mouvement rebelle. Au-delà des expériences individuelles, l'étude découvre, non une armée ethnique mais plus une organisation à base identitaire engagée dans la renégociation violente de son appartenance nationale.
