In our study the combined accuracy of the three methods was the same as combining touch imprint cytology and frozen sections or touch imprint cytology plus rapid immunocytochemistry. Rapid immunocytochemistry provides an additional parameter and preserves tissue for permanent sections.

This text is based on the recommendations accepted by the 4th Hungarian Consensus Conference on Breast Cancer, modified on the basis of the international consultation and conference within the frames of the Central-Eastern European Academy of Oncology. The recommendations cover non-operative, intraoperative and postoperative diagnostics, determination of prognostic and predictive markers and the content of cytology and histology reports. Furthermore, they address some specific issues such as the current status of multigene molecular markers, the role of pathologists in clinical trials and prerequisites for their involvement, and some remarks about the future.

Estrogen receptor (ER) testing has become an important part of breast cancer reporting as the ER status is a predictor of hormonal treatment efficacy. Progesteron receptors (PR) are often tested in parallel, and the best response to hormonal manipulations can be expected in tumors positive for both receptors. The existence of breast cancers with an ER negative and PR positive phenotype is controversial. A series of cases with this phenotype were reevaluated to clarify the existence and the frequency of this entity. A total of 205/6587 (3.1%; range of the rate per department: 0.3-7.1%.) cases reported to have the ER-negative and PR-positive status by immunohistochemistry were collected from 9 Hungarian departments. After careful reevaluation of the tumor slides and control tissues with a 1% cut-off for positivity and restaining of the questionable cases, all but 1 of the reevaluable 182 cases changed their original phenotype. Most cases converted to dual positives (n = 124) or dual negatives (n = 31) or unassessable / questionable. ER-negative and PR-positive breast cancers are very rare if existing. Such a phenotype should prompt reassessment.

Emlôbetegségek diagnosztikájában járatos patológusok megfogalmazták az emlôrák patológiai értékelésével kapcsolatos állásfoglalásukat. Javaslatot tesznek ezúton arra, hogy az emlôrák patológiai diagnosztikája ezen irányelvek szerint történjen. A konszenzus-dokumentumban foglaltakat nyilvánosan is megvitatták, és a dokumentum szövegét ezt követôen véglegesítették, azt a Patológiai Szakmai Kollégium is áttekintette és jóvá-hagyta. Az itt megfogalmazott ajánlások a szakma hazánkban mértékadó minôségi követelményeit tükrözik, amelyek alkalmazásával az emlôdaganatos betegek ellátásának szakmai követelményei teljesülhetnek.Alapelv, hogy az emlôbôl származó citológiai és szö-vettani mintákat az emlôpatológiában jártas patológu-sok vizsgálják és leletezzék.Az emlôbetegségek diagnosztikájában a nem operatív/preoperatív diagnosztika jelentôsége nôtt. A preoparatívan felállított kórisme elônyben részesítendô az intraoperatívan nyert diagnózissal szemben; lehetôleg minden esetben történjen diagnosztikus lépés a kóris-me mûtét elôtti megállapítására. NEM OPERATÍV (PREOPERATÍV) DIAGNOSZTIKAA radiológiailag vagy klinikailag észlelt, malignitás minimális gyanúját is keltô elváltozások patológiai (citoló-giai vagy szövettani) véleményezése szinte mindig indokolt a malignitás kizárása vagy megerôsítése céljából. Jóindulatúnak vélt elváltozások esetén a benignitás megerôsítése is cél lehet. A nem operatív diagnosztika történhet vezérelt vékonytû-aspirációval vagy automata pisztolyos hengerbiopsziával (core-biopsia), esetleg vákuum-asszisztált biopsziás eszközzel nyert mintából. Elôbbi esetben a C1-C5, utóbbi esetekben a B1-B5 kategorikus besorolás elvárás (1. táblázat), de lehetôség szerint ezen túlmenô információkat is (diagnózis, limitált prognosztikai információ, pl. rákok esetén szövettani tí-pus, nuclearis vagy becsült szövettani grade) meg kell adni a vizsgálati anyagokból. Tapintható képletek esetén is lehetôleg javasolt (elsô-sorban ultrahang-) vezérelve végezni a mintavételt, a tapintási/valós méret eltérése, a lehetséges nekrózis miatt, illetve azért, mert képalkotókkal nagyobb való-színûséggel megállapítható lehet, hogy a tapintási lelet hátterében nincs valódi körülírt elváltozás (pl. zsírlobu-lus), vagy hogy a tapintási lelet nem a képalkotó által jelzett elváltozással esik egybe.Meszesedéseket elsôdlegesen henger (core-) vagy vá-kuum-asszisztált core-biopsziával kell tisztázni. Amenynyiben valamilyen ok miatt meszesedéseknél mégis vékonytû-aspiráció történik, a negatív eredményû aspirációs citológiát nem lehet a malignitás kizárására elfogadni. A vékonytû-aspirációs vizsgálat -pozitivitás esetén -a malignitás preoperatív megerôsítésére lehet csupán alkalmas.Tekintettel arra, hogy az atípusos ductalis hámproli-ferációk és a DCIS térben változó spektrumot alkotva lehetnek jelen, a mikrokalcifikáció területébôl származó hengerbiopsziás minta sem lesz feltétlenül reprezentatív. The aim of this study was to investigate the familial clustering of nasopharyngeal carcinoma (NPC) in a non-endemic geographical region on the basis of two case reports and a review of the literature. Following an upper respiratory infection, NPC (WHO type III) was detected in a 57-year-old female (Case 1) who presented with nasal symptoms and a year later in her 36-year-old son (Case 2) who presented with enlarged lymph nodes. After a full diagnostic work-up, cT2a cN0 cM0 (stage IIA; Case 1) and cT2a cN2 cM0 (stage III; Case 2) disease were identified, and telecobalt irradiation was administered to both patients. The mother achieved complete remission and has been disease-free during a 14-year follow-up period. After initial complete remission, the son experienced regional (cervical) and base of the skull relapses within 2 years, which were treated unsuccessfully by means of radical neck dissection, a second course of radiotherapy and chemotherapy. Epstein-Barr virus (EBV) was detected in pathology sections from both patients. The authors review 20 additional well-documented cases of familial clustering of NPC in non-endemic geographical regions from the English language literature. This clinical entity typically has WHO type III histology; it may occur following an upper respiratory tract infection, and EBV-related serological titers were elevated in all 20 investigated cases. No consequent promoting factors were identified. The present two cases and the review of the literature strongly suggest that familial clustering of NPC in non-endemic geographical areas may be related to EBV infections. The difference in outcome of our two cases may be explained by the fact that the disease in Case 2 was diagnosed 1 year later than that in Case 1 and hence at a more advanced stage.

Upregulation of carbonic anhydrase IX (CAIX) was found to be associated with unfavorable prognosis and resistance to treatment in a broad spectrum of malignancies, recently also in classical Hodgkin’s lymphoma (cHL). As demonstrated, variable CAIX expression in a significant number of cHL cases was associated with poor treatment response. The current study focused on the quantification CAIX immunopositivity and its relative expression compared to the total CD30+ neoplastic pool using digital image analysis. One hundred and one lymph node samples featuring cHL histology were analyzed for both CD30 and CAIX by immunohistochemistry. Whole histological slides were scanned and immunopositivity was determined as the histoscore (H-score) using the DensitoQuant software module (3DHistech Kft., Budapest, Hungary). CAIX positivity was observed in the HRS-cells of 56/101 cases (55.44%) and frequently observed in the proximity of necrotic foci. CAIX H-scores were highly variable (range: 2.16–90.36, mean 18.7 ± 18.8). Individual CAIX values were independent of the much higher CD30 values (range 3.46–151.3, mean 52.37 ± 30.74). The CAIX/CD30 index proved to be the highest in the aggressive lymphocyte-depleted (LD) subtype (CAIX/CD30: 0.876). The CAIX expression and the CAIX/CD30 relative index can be precisely determined by image analysis, and values reflect the extent of a tumor mass undergoing hypoxic-stress-related adaptation in the most aggressive forms of cHL.

Statistical analysis confirmed that the two FISH assays are comparable in terms of detection of HER2 gene amplification. Proceeding from these findings, the genetic diagnoses obtained with the Poseidon kit can be considered to be as valuable as the results from the Food and Drug Administration approved PathVysion assay, and its utilisation in routine HER2 diagnostics is proposed.
