La pancreatitis aguda es una de las enfermedades gastrointestinales más comunes, la cual se presenta entre 13 a 45 personas por 100.000 habitantes. Esta entidad se refiere a las inflamaciones del páncreas por diferentes razones, la cual puede ser una inflamación intersticial edematosa del órgano y tejido peripancreático sin tejido necrótico como sucede en la mayoría de casos de aproximadamente el 80%. El otro 20% corresponde a los casos severos donde se encuentran complicaciones locales como tejido necrótico, abscesos e infecciones o donde culmina con una falla multiorganica debido a una Síndrome de respuesta inflamatoria sistémica (SRIS). Las 2 causas principales de Pancreatitis Aguda son los cálculos biliares y el consumo de alcohol, sin embrago existen otras causas menos comunes como la hipertrigliceridemia, post CPRE, ciertos fármacos y en algunas circunstancias idiopática. Para realizar el diagnostico se requieren 2 de los siguientes 3 criterios: Dolor abdominal característico, niveles de amilasa y/o lipasa séricas con elevación mayor a 3 veces los niveles normales y hallazgos típicos de pancreatitis aguda en las pruebas de imagen del abdomen. Para hacer su adecuada clasificación e identificar sus complicaciones se han creado diferentes escalas de severidad la cuales han mejorado con el transcurso de los años. El manejo médico de ésta patología se basa en la adecuada reanimación con líquidos, tratamiento del dolor de preferencia con opiáceos y descanso pancreático manteniendo inicialmente al paciente nada vía oral, mientras se busca la causa subyacente para su adecuado manejo.
La parálisis periódica hipocalemia es un trastorno autosómico dominante neuromuscular, que forma parte de un conjunto de enfermedades agrupadas conocidas como canalopatías. Estas diversas patologías se caracterizan por presentar mutaciones en los genes ubicados en los canales iónicos de sodio y calcio dependientes del voltaje, los cuales generan una inexitabilidad de la membrana muscular. Presentan diversas manifestaciones clínicas como episodios de parálisis y debilidad muscular de los cuatro miembros que puede durar horas, no obstante, suelen afectar principalmente a nivel de los miembros inferiores, relacionados con niveles bajos de potasio en sangre. Se describen dos genes causantes de las alteraciones en el voltaje denominados CACNA1S, SCN4A. El diagnóstico se basa en la historia clínica y sintomatología, así como la confirmación mediante pruebas genéticas, la adecuada educación acerca de la identificación de los factores de riesgo, consejos dietéticos, cambios estilo de vida junto con intervenciones farmacológicas son el tratamiento de elección ideal.
