ResumenSe presenta el caso de una mujer adulta mayor, previamente sana, que ingresa con antecedente de cuadro confusional, debilidad generalizada, cambio de coloración de piel y mucosas, síntomas gastrointestinales y fiebre. En estudios de imagen no se evidencian lesiones encefálicas. Ante cuadro clínico y hallazgos laboratoriales se realiza frotis de sangre periférica, confirmándose diagnóstico de púrpura trombocitopénica trombótica. Se indica plasmaféresis, sin embargo, la paciente no responde a la terapia y fallece.Palabras claves: púrpura trombocitopénica trombótica, ictericia. AbstractThis is the case of old adult woman, previously healthy, admitted with history of confusion symptoms, general weakness, change of skin and mucous color, gastrointestinal symptoms and fever. Medical imaging studies do not show evidences of encephalic lesions. Considering the clinical symptoms and laboratory findings, a smear of peripheral blood is made confirming a diagnosis of thrombotic thrombocytopenic purpura. Although plasmapheresis is indicated, the patient does not respond to the therapy and die. ResumenSe presenta el caso de una paciente de 36 años, portadora de lupus eritematoso sistémico (LES) e hipertensión arterial, que acude a la consulta por cuadro de dolor abdominal agudo relacionado a la ingesta de alimentos. Se concluye el diagnóstico de pancreatitis aguda y se descartan causas comunes. En determinación de marcadores de autoinmunidad se constata hipocomplementemia, ANA 1:160 y anti DNA positivo, catalogándose cuadro como reactivación de LES. Recibe tratamiento esteroideo a altas dosis, con buena respuesta. A diagnosis of acute pancreatitis is made and commons causes are discarded. In the determination of autoimmunity markers, hypocomplementemia, ANA 1:160 and positive anti DNA are found, qualifying this case as a reactivation of SLE. The patient receives treatment with high doses of steroids with good response. ResumenPresentamos el caso de un varón adulto con lupus eritematoso sistémico (LES) que ingresó al Servicio de Urgencias con historia de hemiparesia faciobraquial derecha y disartria. El ictus isquémico se confirma con los métodos auxiliares de diagnóstico. El perfil colagénico fue positivo para IF-ANA, antiADNehipocomplementemia. Los anticuerpos antifosfolípidosfueron negativos. El tratamiento se realiza con corticosteroides y anticoagulación. Este caso pone de relieve la importancia de la detección precoz y el tratamiento oportuno de estas manifestaciones en pacientes con enfermedades del tejido conectivo como el LES.Palabras claves: accidente cerebrovascular isquémico, lupus eritematoso sistémico. AbstractWe present the case of a male adult with systemic lupus erythematosus (SLE) admitted to the Urgency Service with a history of right faciobrachial hemiparesis and dysarthria. The ischemic stroke was confirmed by the auxiliary diagnostic methods. Collagen profile was positive for IF-ANA, anti-DNA and hypocomplementemia. Antiphospholipid antibodies were negative. Treatment was made with corticosteroids and anticoagulation. This case emphasizes the importance of early detection and opportune treatment of these manifestations in patients with connective tissues diseases like SLE. Se constata hemiparesia faciobraquial derecha. En estudios laboratoriales se encuentra anemia, plaquetopenia y microalbuminuria. Se interna bajo la sospecha de ACV. No se evidencian lesiones tempranas de ACV en tomografía computada de cráneo pero en la resonancia nuclear magnéticase encuentra área isquémica en lóbulo parietal izquierdo (gráficos 1-4). Se indica tratamiento antiisquémico con buena evolución. La ecocardiografía transtorácica, ecografía Doppler carotidea y ecocardiografía transesofágica son normales. En el perfil colagénico se halla hipocomplementemia, ANA 1:320, antiDNA positivo. El perfil trombofílico es negativo. Se inicia tratamiento con prednisona 0,5mg/kp/día y anticoagulación con heparina de bajo peso molecular. Se concluyecomo diagnóstico: ACV isquémico en el contexto de un paciente con LES.
