The adverse effects of primaquine (0.50 mg/kg/day) were investigated in eleven patients with vivax malaria (three patients with glucose-6-phosphate dehydrogenase deficiency). Clinical and laboratorial alterations indicated acute hemolysis in only the enzymopenic patients and treatment was interrupted. Our results suggest that screening for G6PD deficiency should be carried out in patients with vivax malaria infection in order to avoid complications due to primaquine.
O status epidemiológico da amebíase está sendo reavaliado desde que a Entamoeba histolytica (patogênica) foi considerada espécie distinta de Entamoeba dispar (não patogênica). Em nosso estudo, realizamos pesquisa de antígenos de E. histolytica em amostras fecais de pacientes residentes na cidade de Belém, Pará, Brasil, utilizando ensaio imunoenzimático (E. histolytica Test, TechLab Inc., Blacksburg, Estados Unidos) disponível comercialmente. Foram analisadas 845 amostras, com positividade em 248 (29,35%). A infecção por E. histolytica foi maior no grupo etário acima de 14 anos (30,36%) que no grupo de 0-14 anos (28,28%), porém sem significância estatística (p < 0,05). Do total de amostras analisadas, 334 foram também investigadas por métodos parasitológicos (direto, Hoffman e Faust et al.). Houve discordância nos resultados dos métodos ELISA e coproscópico em 83 amostras (24,85%), com maior número de positivos no teste imunoenzimático. Assim, nossos resultados sugerem que a amebíase intestinal é um importante problema de saúde pública na Região Metropolitana de Belém.
Norovirus (NoV) is a major cause of nonbacterial acute gastroenteritis (AGE) outbreaks worldwide, with infections reported in semiclosed environments, particularly in hospitals and nursing homes. Astrovirus (HAstV) is prevalent worldwide, especially in developing countries. We aimed to determine the prevalence, spatial distribution, and genetic diversity of NoV and HAstV in children under 5 years of age in Rio Branco city, Acre State, Amazon Region, Brazil. Stool samples from children with (n = 240) and without (n = 248) AGE were collected from January to December 2012 from seven neighborhoods. The overall NoV prevalence was 12.3% (60 of 488); representing 15.8% (38 of 240) of the symptomatic samples and 8.9% (22 of 248) of the controls. HAstVs infection was observed in 4.7% (23 of 488) of the samples tested, 6.2% (15 of 240) of AGE cases, and 2.4% (6 of 248) of the controls (plus two without information about feces consistency). Infections were found in all age groups with higher frequency in children less than two years of age, for both viruses. NoV was detected in all neighborhoods, with a higher concentration in the fourth (30%; 18 of 60). NoV nucleotide sequencing performed in 86.7% (52 of 60) of the positive samples showed the circulation of the strains GII.4 (57.7%; 30 of 52), GIIPe/GII.4 (19.2%; 10 of 52), GII.7, GII.Pg/GII.1, and GII.Pc (3.8%; 2 of 52 for each), GII.6 and GII.Pg (1.9%; 1 of 52 for each), and GI.3 (7.7%; 4 of 52). Three GII.4 variants were detected: Den Haag_2006b (n = 1), New Orleans_2009 (n = 1), and Sydney_2012 (n = 14). HAstV types HAstV‐1a (81.8%; 9 of 11) and HAstV‐2c (18.2%; 2 of 11) were observed in the 47.8% (11 of 23) of characterized samples. This is the first data obtained in Acre State regarding the prevalence of these viruses and provides epidemiological and molecular information for a better understanding of their role among children with and without AGE.
O artigo expõe a comparação de métodos de diagnóstico de Entamoeba histolytica em amostra da população de Belém do Pará. Foram analisadas amostras fecais de crianças e adultos (Grupo I), amostras fecais e soros de adultos (Grupo II) e material fecal de crianças (Grupo III). Nos grupos I e III foram empregados os métodos direto com lugol (MD), Faust e cols. (MFF) e ELISA (detecção de coproantígeno anti-GIAP de E. histolytica); no grupo II, MD, hematoxilina férrica (HF), MFF, ELISA e reação de imunofluorescência indireta (RIFI) para detecção de anticorpos IgG. A positividade encontrada foi de 10,50%, empregando (MD + MFF) e de 28,99% pelo ELISA. Não houve correlação entre positividade e grupo etário. No Grupo II (n = 87), a positividade encontrada foi de 4,59% pelos métodos coproscópicos (MD + MFF), 8,04% por HF, 4,59% pela RIFI e 21,83% pelo ELISA. O teste de ELISA foi o mais sensível para todos os grupos. Conclui-se que a RIFI ainda não é ferramenta útil para diagnóstico desta protozoose. O teste de ELISA, de fácil execução, foi feito em 1/3 do tempo usado para HF e RIFI, melhorando a qualidade do diagnóstico. Recomenda-se o ELISA como método de diagnóstico nos caso suspeitos de infecções com E. histolytica.
RESUMOO Município de Juruti, Estado do Pará, Brasil, vem passando por intensas mudanças ambientais, demográficas e socioeconômicas devido à exploração de bauxita, que desencadeou um intenso processo migratório em função das oportunidades de emprego e renda. Devido ao impacto desta migração à saúde pública local, a Secretaria de Saúde do Município e o Instituto Evandro Chagas desenvolveram o Projeto Saúde Juruti, que possibilitou a investigação de agravos importantes para o Município, como as doenças diarreicas. O objetivo deste estudo foi identificar os fatores de riscos e a etiologia infecciosa associados às doenças diarreicas em Juruti durante o período de 2007 a 2010, por meio de técnicas estatísticas denominadas análise exploratória de dados e análise bivariada. Os resultados demonstraram que a faixa etária menor de 1 ano de idade e a queima de lixo foram identificadas como possíveis fatores de risco para a ocorrência da diarreia. Outras variáveis como as práticas alimentares, o cuidado com manuseio de alimentos e os hábitos de higiene pessoal e domiciliar necessitam ser investigadas. Entre as causas bacterianas e parasitárias, as bactérias foram a principal causa de diarreia, com destaque para a associação de Shigella e de Escherichia coli patogênicas aos casos diarreicos. Diante desses resultados, são sugeridas ações de monitoramento como o registro dos casos diarreicos e de prevenção e controle da diarreia, manter a distribuição e utilização de hipoclorito de sódio, juntamente com as ações de educação em saúde e com a melhoria nas condições de saneamento básico.
RESUMOOBJETIVO: Investigar a ocorrência de enteropatógenos bacterianos na etiologia da doença diarreica aguda (DDA), associando variáveis clínico-epidemiológicas, em menores de 5 anos de idade, atendidos em unidades de saúde do município de Rio Branco, estado do Acre, Brasil, em 2012. MATERIAIS E MÉTODOS: O estudo envolveu a análise de dados obtidos das fichas clínico-epidemiológicas de 502 crianças, sendo 272 casos de DDA e 230 controles. A frequência de cada enteropatógeno pesquisado e a associação desses agentes com as condições socioeconômicas das crianças foram verificadas por meio dos testes qui-quadrado e teste exato de Fisher, adotando-se um nível de significância de 0,05 (5%) e utilizando-se o programa BioEstat v5.0. RESULTADOS: As bactérias mais frequentes em ambos os grupos foram Escherichia coli diarreiogênicas (80,2%), Campylobacter jejuni e Campylobacter coli (9,0%), Shigella spp. (5,6%) e Salmonella spp. (4,0%). Houve associação significativa entre a ocorrência de E. coli e a presença de DDA (p < 0,001). A distribuição temporal das E. coli diarreiogênicas foi maior nos meses de novembro e dezembro (período chuvoso). A associação entre a renda familiar e o grupo clínico foi estatisticamente significativa (p < 0,00051). Quanto às condições de habitação, foi observado maior risco quando a frequência da coleta de lixo era irregular. Constatou-se que o risco da criança desenvolver DDA era maior quando a mãe tinha entre 14 e 19 anos de idade. CONCLUSÃO: Verificou-se que os casos de DDA em menores de 5 anos de idade, em Rio Branco, estavam relacionados à circulação de enteropatógenos bacterianos, em especial a E. coli diarreiogênica. This study evaluated the microbiological quality of farm oysters, cultivated in an estuary in northeastern Pará, Brazil, before and after purification process. Water and oyster samples were collected in June, August, October, and December 2019. Oysters underwent purification for up to 36 h in a small-scale adapted debugger. Microbiological analyses were performed in oysters and water depuration collected at different times of purification (0 h pre-treatment, 6 h, 12 h, 2 4h, and 36 h), using the multiple tube technique, to determine the most probable number (MPN) of Escherichia coli, thermotolerant coliforms - TTC, and total coliforms - TC. Bacterial isolation was used to determine the presence of Salmonella spp. Before purification, oysters had E. coli counts above the limit allowed by Brazilian legislation (≤ 230 MPN/100g). High TTC, and TC counts were observed in all the months of the study, and Salmonella spp. was present in August. All water samples were contaminated by TTC beyond the level allowed (> 43 MPN/100mL) in two months. The purification process of 36 h was effective in reducing the load of E. coli, TTC, and TC and eliminating Salmonella spp. in oysters.
