Introduction: Antimicrobial resistance (AMR) is increasing and represents one of the world’s major challenges. AMR increase morbimortality, length of hospital stay and costs. Antimicrobial Stewardship Programs (ASP) are one of the key strategies to promote the rational use of antimicrobials since AMR is mostly driven by antimicrobial consumption.Objective: To describe the ASP implementation in a teaching hospital from the perspective of Donabedian quality assessment and the Brazilian regulatory requirements.Method: This was a descriptive study with secondary data collection, including document review of the ASP. The study setting was a general public 392-bed hospital. The ASP activities were performed by the hospital infection control committee (HICC), hospital pharmacy (HP) and diagnostic support laboratory (DSL). The description of the three services mainly involved in the ASP was based on a quality assessment model involving the dimensions of “structure”, “process” and “result” proposed by Donabedian. The distribution among dimensions was guided by the checklist of essential elements of the ASP that compose the Brazilian regulatory requirements. The checklist was applied in July, 2022, and the ASP results described from 2016 to 2021.Results: ASP actions have been gradually implemented since 2008 with the implementation of HICC and improved over the years. Regarding structure, the investments in technology were mapped, quantifying 26 computers and three software programs employed to computerize the ASP processes performed in specific physical areas by HICC, HP and DSL. Institutional guidelines used by HICC, HP and DSL guided clinical practices to operationalize ASP. The evaluation metrics improved for 10 indicators and worsened for four indicators. From the 60 items composing the checklist, the hospital met the requirements in 73.3% of the items (n = 44).Conclusion: This study described the implementation of ASP in a teaching hospital, applying the Donabedian perspective. Although the hospital still does not have a classic ASP model, there were investments to improve structure, processes and results, aiming to comply with international guidelines. A high proportion of key elements of ASP in the hospital were followed according to the Brazilian regulatory requirements. Aspects related to antimicrobial consumption and the emergence of microbial resistance deserve further investigations.
Ao entender que práticas educacionais podem contribuir para melhores resultados nos processos de cuidado em saúde, o presente artigo possui o objetivo de descrever a experiência da implementação de oficinas como estratégia educacional em uma clínica de anticoagulação de um hospital geral. A execução das oficinas consistiu na dinâmica em grupo e utilização de recursos visuais. Essa prática foi subdividida nas temáticas “conhecendo o problema de saúde”, “uso correto da varfarina”, “alimentação e anticoagulação” e “conhecendo o corpo”. A descrição da experiência ocorreu por meio da análise observacional, gravação dos encontros e análise das falas por meio da técnica de análise de conteúdo. Participaram das oficinas 19 pacientes em uso de varfarina e sete acompanhantes. Identificou-se duas categorias, sendo a “Sistematização do fazer”, onde identificou-se que o processo de cuidado ocorre de forma médico centrada, sem a valorização do paciente como sujeito em ato, e “Redescobrindo-se no tratamento”, onde os participantes foram capazes de redimensionarem o processo de cuidado e construírem um novo olhar sobre o sua participação nesse processo.As oficinas propiciaram para os participantes, uma troca de experiências e dúvidas, podendo contribuir para uma maior compreensão do problema de saúde e tratamento., além de propiciar aos profissionais melhor compreensão sobre a experiência dos pacientes em relação ao tratamento.
The aim of this study is to research about the ideas that the Biology students that attended the Unified Secondary Course had about how often they carried out activities related to leaming within the field of socio-affective relationship and their point of view on the relevance of that learning in their assessment.
O artigo objetiva uma análise crítica dos desafios para o enfrentamento da pandemia do Coronavírus Sars-Cov2 a partir da vivência em hospital de caráter regional da parte norte da Região Metropolitana de Belo Horizonte. Além da caracterização da organização hospitalar e das articulações existentes com as estações cuidadoras do SUS-BH, é abordado o conjunto de ações adotadas para prevenção, controle e assistência aos pacientes e trabalhadores no contexto da Covid-19 e as lições aprendidas. Aprendizado esse relacionado à importância da agilidade institucional para o desenvolvimento das adequações estruturais e organizacionais para manejo da nova doença e à comunicação como ferramenta de transparência para envolvimento das equipes. Apesar da articulação com o SUS-BH das ações planejadas, identifica-se também a necessidade da ampliação de estratégias junto à Atenção Básica para a continuidade do cuidado aos pacientes egressos hospitalares com diagnóstico de Covid-19.
