Objective This study compared the effect of diode laser (DL) 980 nm and double antibiotic paste (DAP) on response of mature teeth with necrotic pulp and apical periodontitis to regenerative endodontic therapy in a dog model. Methods Pulp necrosis and periapical pathosis were induced in 40 mature double rooted premolars in four 2-year-old mongrel dogs. These teeth were randomly divided according to disinfection protocol into four equal groups (10 teeth each/20 roots), group I: DAP; group II: DL980 nm; group III: positive control (without treatment) and group IV: negative control (untouched teeth). These groups were further subdivided into 2 subgroups (5 teeth each/10 roots) according to evaluation period, subgroup (A): one month and subgroup (B): three months. Revascularization techniques were performed using bleeding induction and application of platelet rich fibrin (PRF). The coronal cavities were sealed with mineral trioxide aggregate (MTA) and glass ionomer cement. The inflammatory response, vital tissue in-growth, new hard tissue formation and bone resorption were assessed. Statistical analysis was done utilizing ANOVA, Tukey’s post hoc and paired t tests. Results In both subgroups, there were no significant differences between DAP and DL980 in terms of inflammatory cell count, vital tissue in-growth, new hard tissue formation and bone resorption (P ≤ 0.05). Conclusion Diode laser 980 nm can alternate DAP as a disinfection method of the root canal during RET for mature necrotic teeth, therefore it may accelerate regenerative endodontic therapy for both the patient and dentist and allows for RET in a single appointment.
Purpose: The present study was designed to detect the effect of coronal preflaring on the amount of debris extrusion during canal preparation by reciprocating systems, WaveOne Golda nd Reciproc. Material and Methods: Sixty lower first molars having mesiobuccal canal curvature range between 25°-35°were used. They were divided into 2 experimental groups (Ι and ΙΙ) according to the method of preparation of the root canal (30 samples each). Group I: Samples were prepared with coronal preflaring. Group II: Samples were prepared without coronal preflaring. The experimental groups were classified into 2 subgroups (A, B) according to the reciprocating single-file system used (15 samples each). Subgroup A:Samples were instrumented using Reciproc file. Subgroup B: Samples were instrumented using WaveOne Gold file. The samples were mounted in a model to measure the amount of debris extrusion. Results: In Reciproc subgroup, no statistical difference was found in samples prepared with and without coronal preflaring regarding the median of debris extrusion(P>0.05).In WaveOne Gold subgroup, samples prepared with coronal preflaring showed lower value of apically extruded debris than samples prepared without coronal preflaring. However, no statistical difference was found between the tested samples in the median of debris extrusion(P > 0.05).In group I and II: (with and without coronal preflaring respectively), no significant difference was found between Reciproc and WaveOne Gold in the median of debris extruded apically (P> 0.05). Conclusions: During preparation of the canal WaveOne Gold and Reciproc reciprocating single-file systems presented similar behavior regarding the amount of debris extrusion.During canal preparation coronal preflaring has no effect on the amount of debris extrusion using reciprocating systems, Wave One Gold and Reciproc.
Purpose: This study was designed to compare marginal adaptation of MTA and Endosequence Root Repair Material (ERRM) as retrograde filling materials. Materials and methods: 40 maxillary anterior teeth were prepared and obturated with gutta-percha and AH plus using lateral compaction technique. Apical 3mm of the roots were resected and retrograde cavities were prepared with diamond coated ultrasonic retrotip. Teeth were divided randomly into two main groups (I,II) according to blood contamination of retrograde cavities and every group then divided into two sub-groups (A,B) according to retrograde materials (MTA,ERRM). Teeth sectioned longitudinally and examined for marginal gap using SEM. Results: revealed that the high mean value in cavities without blood contamination and filled with MTA in group I and the high mean value in cavities smeed with blood contamination and filled with MTA in group II. There was no statistically significant difference between 4 groups. Conclusion: MTA and ERRM exhibited nearly comparable marginal adaptation in dry or blood environment.
