We report a case of a giant cell tumor (GCT) of rare localization, Developed at the expense of the tendon sheath of the long fibular tendon In a 45-year-old woman Who consults for a painful swelling below the cuboid bone. Treated with surgical excision, the diagnosis was confirmed by histology.
……………………………………………………………………………………………………....Il s'agit d'une patiente de 45 ans, sans antécédents, qui avait consulté pour une douleur inflammatoire de la plante du pied en regard de la base du 1 er métatarsien, évoluant depuis 2 ans, dont l'examen clinique avait objectivé une tuméfaction de petite taille, siégeant au dessous de l'os cuboῒde, ferme, adhérente au plan profond, sans signes inflammatoires en regard. La radiographie standard ne montrait pas d'atteinte osseuse. Une imagerie par résonnance magnétique (IRM) avait montré la présence d'une formation ovalaire hyposignal en T1, hypersignal en T2, fortement rehaussée après injection de produit de contraste (PC) au niveau de la gaine synoviale du tendon long fibulaire. Une exérèse chirurgicale réalisée par voie d'abord plantaire centré sur la tumefaction, a montré une masse brunâtre en rapport avec le tendon long fibulaire. L'étude histologique était en faveur d'une TCG des gaines tendineuses. Les suites opératoires étaient simples. Après un recul de 2 ans, aucune récidive n'a été notée.La TCG des gaines tendineuses est fréquente au niveau de la main ; rare au niveau du pied ; où elle peut siéger dans les synoviales articulaires, les gaines tendineuses et les bourses séreuses. L'affection peut revêtir deux formes anatomo-cliniques : localisée ou diffuse. Le diagnostic est clinico-radiologique et confirmé par l'histologie où la tumeur est caractérisée sur le plan microscopique par une prolifération de cellules géantes multinuclées. Le traitement est chirurgical, basé sur l'exérèse complète de la tumeur, pour prévenir les récidives. La radiothérapie adjuvante ne s'avère utile qu'en cas de chirurgie incomplète.
Nous rapportons l'observation d'un enfant de 12 ans, sans antécédents, ayant présenté une douleur chronique progressive depuis six mois sur l'arche interne du pied gauche. S'intensifiant à la marche, et associée à une difficulté de chaussage. A l'examen et au regard de l'os naviculaire, on note la présence d'un oedème et d'une inflammation, ainsi qu'une douleur à la palpation de la face médiale du pied. La radiologie standard a montré la présence d'un os naviculaire accessoire gauche:-Le scanner des deux pieds a objectivé un os naviculaire accessoire à gauche de type II, faisant une synchondrose avec l'os naviculaire, ainsi qu'une insertion des fibres du tendon tibial postérieur sur l'os accessoire, et l'absence de l'os accessoire à droite. Une scintigraphie osseuse a montrée une hyperfixation intense au niveau du dôme et du col de l'astragale gauche. Le traitement médical usuel a été prescrit, associé au port d'une orthèse plantaire et de chaussures adaptées. Une infiltration de corticoïdes a également été réalisée. Devant l'échec du traitement conservateur, un traitement chirurgical consistant en une arthrodèse de l'os naviculaire avec son accessoire à l'aide de vissage a été réalisé. L'appui immédiat et la rééducation précoce ont été indiqués:-L'os naviculaire accessoire est un os surnuméraire rare du tarse. Il est attaché par une synchondrose avec le corps principal de l'os naviculaire. Plus fréquent chez la femme jeune, souvent bilatérale et asymptomatique. Le traitement est chirurgical, indiqué en cas d'échec du traitement médical, consiste soit en un forage percutané de la synchondrose, une arthrodèse de l'os naviculaire avec son accessoire, ou une simple excision de l'os naviculaire accessoire.
