Sonographic findings in the spleen of patients with FAS are characterized predominantly by a small spleen with absence of parenchymal vascularization on color Doppler sonography in most cases. Future prospective studies will be necessary to confirm these findings and to determine whether FAS can be diagnosed reliably with sonography.
Schon Galen spricht von der Milz als einem "organum plenum mysterii" in Unkenntnis der Funktion dieses Organs. In einer kürzlich erschienenen Monographie über die Bildgebung der Milz wird sie als "the silent and forgotten organ of the abdomen" bezeichnet (3). Aufgrund des seltenen Auftretens von fokalen Milzläsionen mit einer Inzidenz von 0,2% gilt die Milz im diagnostischen Bereich im Vergleich zu anderen parenchymatösen Organen als ein "langweiliges" Organ (5).Mit Hilfe von Schnittbildverfahren ist eine Beurteilung der Milzfunktion nicht möglich. Häufigkeit, Ätiologie, Diagnostik, klinische Symptomatologie und prognostische Bedeutung des Befundes einer funktionellen Hypo-/Asplenie sind weitgehend unbekannt. Mit der Beobachtung von fatalen infektiösen Krankheitsverläufen bei splenektomierten Patienten, der sog. "overwhelming post splenectomy infection (OPSI)" rückte das Krankheitsbild der funktionellen Hypo-/Asplenie in das Interesse von Patienten und Ärzten. Diagnostik der funktionellen Hypo-/AsplenieDie Aufgaben der Milz beinhalten grundsätzlich die Fähigkeit der Phagozytose, der Hämatopoese, der Entkernung der erythrozytären Vorläuferzellen (Normoblastenentfernung), des Reservoirs von Blutzellen sowie immunologische Funktionen. Mit der szintigraphischen Darstellung von Leber und Milz durch mit Technetium 99m beladene Kolloidpartikel oder hitzedenaturierter Erythrozyten sowie dem Nachweis von intraerythrozytären Einschlusskörperchen im peripheren Blutausstrich (Howell-Jolly-Bodies) kann die Funktion der splenischen Phagozytose von Partikeln beurteilt werden (Abb.1) (13).Durch den verbreiteten Einsatz der Sonographie rückt der Befund der "kleinen Milz" zunehmend ins Interesse. So werden auffallend kleine Milzen u.a. im höheren Lebensalter, im Rahmen von Infektionskrankheiten, bei Sichelzellanämie entzündlichen Darmerkrankungen und nach allogener Knochenmarktransplantation beobachtet. Es gibt Hinweise dafür, dass insbesondere kleine Milzen mit fehlendem Nachweis von Flusssignalen in der Farbdopplersonographie mit einer funktionellen Hypo-/Asplenie assoziiert sind (15). kurzgefasst: Die Diagnose der funktionellen Hypo-/Asplenie beruht auf dem Nachweis von Howell-Jolly-Körperchen im peripheren Blut und der fehlenden Milzdarstellung in der Leber-Milz-Szintigraphie.
