Factores de riesgo de la atrición dental severa: un estudio de casos y controles* Severe dental attrition and associated factors: a case-control study * RESUMEN Objetivo: identificar los factores de riesgo de la atrición dental severa en pacientes que acudieron a instituciones de atención odontológica, públicas y privadas, de la ciudad de Cuenca, Ecuador. Materiales y métodos: fueron analizados 237 pacientes adultos, 79 casos con atrición dental grados 2, 3 y 4, según el índice de Smith y Knight, y 158 controles con atrición grados 0 y 1. Se realizó un examen clínico y fotográfico para determinar la relación entre la atrición dental y factores como edad, sexo, procedencia, número de dientes residuales, pH salival y bruxismo. Resultados: los pacientes mayores de 25 años (OR= 2,47; IC= 1,35; X2= 10,21; p= 0,001), con menos de 20 dientes en boca (OR= 4,95; IC= 1,47 -16,62; X2= 7,97; p= 0,004 ) y con bruxismo (OR= 2,64; IC= 1,45 -4,81; X2= 10,49; p= 0,001) tienen más probabilidades de presentar atrición dental severa. Luego de una regresión logística binaria se confirmó dicha relación: pacientes mayores de 25 años (OR= 2,03; IC= 1,08 -3,818) con menos de 20 dientes en boca (OR= 3,90; IC= 1,07 -14,19) y con bruxismo (OR= 3,22; IC= 1,70 -6,10); sin embargo, se observó una capacidad predictora muy baja de la variables ajustadas según el R-cuadrado de Cox y Snell (0,123) y el R-cuadrado de Nagelkerke (0,172). Conclusiones: edad avanzada, menor cantidad de dientes en boca y bruxismo aumentaron la posibilidad de presentar atrición dental severa en los pacientes analizados. Mientras que el sexo masculino, la procedencia rural y el pH salival crítico (menor a 6) no se comportaron como factores de riesgo.Palabras clave: Atrición dental; factores de riesgo; estudios de casos y controles; bruxismo; edentulismo.ABSTRACT Objective: to identify the risk factors of severe dental attrition in patients who attended public and private dental care institutions in the city of Cuenca-Ecuador. Results: after a bivariate analysis, it was evident that patients older than 25 years (OR= 2,47; IC= 1,41 -4,35; X2= 10,21; p= 0,001), with less than 20 teeth in the mouth ((OR= 4,95; IC= 1,62; X2= 7,97; p= 0,004 ) and with bruxism (OR= 2,64; IC= 1,45 -4,81; X2= 10,49; p= 0,001) are more likely to have severe tooth attrition. After a binary logistic regression this relationship was confirmed: patients older than 25 years (OR= 2,03; IC= 1,08 -3,818) with less than 20 teeth in the mouth (OR= 3,90; IC= 1,07 -14,19) and with bruxism (OR= 3,22; IC= 1,70 -6,10), however a very low predictive capacity of the adjusted variables was observed according to R-square of Cox...
Introducción: el dolor es uno de los síntomas principales de los trastornos musculoesqueléticos y tiene alta frecuencia en los profesionales de la odontología. El objetivo del estudio fue determinar la prevalencia del dolor musculoesquelético y los factores sociodemográficos y laborales asociados en odontólogos que laboran en la ciudad de Cuenca, Ecuador. Materiales y métodos: se realizó un estudio transversal en 240 odontólogos inscritos en la Dirección Provincial de Salud del Azuay de la ciudad de Cuenca hasta el año 2015. Se usó el cuestionario de Kuorinka para determinar síntomas musculoesqueléticos. Se creó un formulario con variables sociodemográficas y las características del dolor para establecer asociaciones estadísticas. La información fue procesada en el programa SPSS versión 22. Resultados: el 58,7% eran mujeres. El promedio de edad fue de 37 años con una DE de 11,6. La prevalencia de dolor musculoesquelético fue del 73,3%. El 90,3% atribuyó el dolor a causas laborales. Los factores asociados al dolor fueron: sexo femenino, sedentarismo, trabajar en el sector público y carga horaria mayor a 30 horas semanales. El dolor musculoesquelético en los odontólogos está asociado estadísticamente a: sexo femenino (OR= 2,60; IC 95%= 1,35 – 4,99; p= 0,004), sedentarismo (OR= 2,42; IC 95%= 1,20 – 4,89; p= 0,013), ejercer en el área pública (OR= 2,97; IC 95%= 1,26 – 6,99; p= 0,012) y carga horaria mayor a 30 horas semanales (OR= 2,21; IC 95%= 1,15 – 4,21; p= 0,017). Conclusiones: el ejercicio de la práctica clínica odontológica constituye un factor de riesgo para la generación de trastornos musculoesqueléticos.Palabras clave: Dolor musculoesquelético; osteomuscular; salud ocupacional; odontólogos; atención odontológica
Oral Squamous Papilloma is the most common benign tumor that can be found in the oral cavity, the main etiological factor is Human Papilloma Virus (HPV), the route of transmission of this virus is through direct contact, despite being a lesion with benign characteristics, it has been observed in recent years that HPV infection is directly associated with the presence of Squamous Cell Carcinoma in the oral cavity, becoming a factor of importance that must be addressed by the different health services worldwide. We present the case of a 63-year-old male patient who attended the Dental Clinic of the Faculty of Dentistry of the Pontificia Universidad Javeriana Bogotá-Colombia. Through the intraoral examination, a papillary lesion on the soft palate was found. An excisional biopsy was performed and the diagnosis is confirmed by histopathological examination.
RESUMEN INTRODUCCIÓN:El Ataque Cerebrovascular (ACV) Isquémico constituye una de las principales causas de discapacidad permanente a nivel mundial, pese a su magnitud no se cuenta con una descripción epidemiológica en nuestra población y mucho menos etiológica, por lo que el objetivo del estudio consistió en identificar la prevalencia del ACV Isquémico así como sus características clínicas .MÉTODOS: en base a la clasificación de TOAST "Trial of Org 10172 in Acute Stroke Treatment" se definió la etiología del evento . El presente es un estudio descriptivo de corte transversal, que incluyó a todos los pacientes cuyo motivo de ingreso a la unidad de neurología del Hospital José Carrasco Arteaga entre febrero y julio de 2013 fue un ACV Isquémico, se registraron los principales datos demográficos, Se incluyó sintomatología y estudios complementarios realizados hasta su alta o fallecimiento . La descripción epidemiológica se realiza en base a porcentajes y tasas . RESULTADOS:Se registraron 42 pacientes, que representó una prevalencia del 40% como causa de ingreso a la unidad, según TOAST el 33 .3% de los casos fue Indeterminado/causa desconocida por estudios normales, el 23 .8% cardioembólicos, el 16 .7% Infarto Lacunar e Indeterminados por estudios no realizados cada uno, 7 .1% infarto aterotrombótico y 2 .4% causa inhabitual . El 31% de los pacientes fue mayor a 80 años, predominando el sexo femenino con 54 .8% . La condición clínica de mayor asociación fue la Dislipidemia en el 83 .3%, seguido de la Hipertensión arterial en el 78 .6% . El 69 .1% de los pacientes debutó con afección de pares craneales, la mortalidad fue del 19% . CONCLUSIÓN: la prevalencia de ACV es igual a la reportada en otros países .
The University is the place where the students learn and develop skills that will allow them to pursue an occupation, and at the same time improve their attitudes, health skills, that allow them to have healthy living habits and lifestyle. Dental health is an essential part of health and represents an important element for the quality of life. In this sense it is intended to evaluate the attitudes and behaviors of dental health using the approved index Hiroshima University Dental Behavior Inventory (HU-DBI) among the different areas of knowledge of the University of Cuenca. This study was a survey with Sociodemographic variables and 20 dichotomous questions of the HU-DBI, which was applied after signing the informed consent forms in all the training areas of the University of Cuenca. The response rate was 56% N=7790 students, with an average age of 21,15 years, DS±2,42 variance of 5,89, the male-female ratio is 1:1,07. The majority of students are of the urban origin with 82,8% and the vast majority are single (7125 students=91,4%). The average of the HU-DBI index is 6,06 with a DS±1,81 and a variance of 3,27. There is a better index of the students of health science compared with the students of other areas. Comparing them with the dental students and the others of the health sciences, dental students who had no clinical contact with patients didn't have any difference from those of the other health sciences, but the students who have had the clinical contact with patients have a statistically significant difference with the other students of health sciences.
