Objetivo. Estimar os custos atribuíveis a hipertensão arterial, diabetes e obesidade no Sistema Único de Saúde (SUS) no Brasil em 2018. Métodos. Realizou-se uma estimativa dos custos atribuíveis a doenças crônicas não transmissíveis a partir dos riscos relativos e das prevalências populacionais de hipertensão, diabetes e obesidade, considerando custos de hospitalizações, procedimentos ambulatoriais e medicamentos distribuídos pelo SUS para tratamento dessas doenças. As informações de custo foram obtidas nos sistemas de informação em saúde disponíveis no SUS. A análise explorou os custos das doenças segundo sexo e idade na população adulta. Resultados. Os custos totais de hipertensão, diabetes e obesidade no SUS alcançaram 3,45 bilhões de reais (R$) (IC95%: 3,15 a 3,75) em 2018, ou seja, mais de 890 milhões de dólares (US$). Desses custos, 59% foram referentes ao tratamento da hipertensão, 30% ao do diabetes e 11% ao da obesidade. No total, 72% dos custos foram com indivíduos de 30 a 69 anos de idade e 56%, com mulheres. Considerando separadamente a obesidade como fator de risco para hipertensão e diabetes, os custos atribuíveis a essa doença chegaram a R$ 1,42 bilhão (IC95%: 0,98 a 1,87), ou seja, 41% dos custos totais. Conclusões. As estimativas dos custos atribuíveis às principais doenças crônicas associadas à alimentação inadequada evidenciam a grande carga econômica dessas doenças para o SUS. Os dados mostram a necessidade de priorizar políticas integradas e intersetoriais para a prevenção e o controle da hipertensão, do diabetes e da obesidade e podem apoiar a defesa de intervenções como medidas fiscais e regulatórias para alcançar os objetivos da Década de Ação das Nações Unidas sobre Nutrição.
BackgroundObesity is a global public health problem and a risk factor for several diseases that financially impact healthcare systems.ObjectiveTo estimate the direct costs attributable to obesity (body mass index {BMI} ≥ 30 kg/m2) and morbid obesity (BMI ≥ 40 kg/m2) in adults aged ≥ 20 incurred by the Brazilian public health system in 2011.SettingsPublic hospitals and outpatient care.MethodsA cost-of-illness method was adopted using a top-down approach based on prevalence. The proportion of the cost of each obesity-associated comorbidity was calculated and obesity prevalence was used to calculate attributable risk. Direct healthcare cost data (inpatient care, bariatric surgery, outpatient care, medications and diagnostic procedures) were extracted from the Ministry of Health information systems, available on the web.ResultsDirect costs attributable to obesity totaled US$ 269.6 million (1.86% of all expenditures on medium- and high-complexity health care). The cost of morbid obesity accounted for 23.8% (US$ 64.2 million) of all obesity-related costs despite being 18 times less prevalent than obesity. Bariatric surgery costs in Brazil totaled US$ 17.4 million in 2011. The cost of morbid obesity in women was five times higher than it was in men.ConclusionThe cost of morbid obesity was found to be proportionally higher than the cost of obesity. If the current epidemic were not reversed, the prevalence of obesity in Brazil will increase gradually in the coming years, as well as its costs, having serious implications for the financial sustainability of the Brazilian public health system.
Este artigo teve como objetivo propor recomendações para a realização de estudos de custo da doença no sistema de saúde brasileiro. Realizou-se uma revisão da literatura sobre aspectos relacionados ao método de custo da doença e aplicou-se ao caso brasileiro. Os resultados estão divididos em dois blocos. O primeiro refere--se aos conceitos inerentes aos estudos de custo da doença, tais como tipos de custos, perspectiva da análise, apresentação dos valores monetários e fontes de informação. O segundo diz respeito a recomendações para aplicar o método de custo da doença ao contexto brasileiro. Foram propostas sete etapas para a elaboração de um estudo de custo da doença: 1) selecionar a doença e elaborar a pergunta a ser respondida; 2) definir a perspectiva, a abordagem e os tipos de custo a serem analisados; 3) obter os riscos de incidência das comorbidades que poderiam ser parcialmente evitadas caso a doença investigada não existisse; 4) calcular o risco atribuível à doença investigada nas suas comorbidades; 5) levantar os custos de cada patologia associada; 6) calcular o custo da doença final; e 7) analisar os resultados. É viável adotar o método de custos da doença no Brasil, pois as informações não só estão disponíveis, como também são de acesso público. Apesar de algumas limitações, os estudos sobre o custo da doença são muito úteis para o planejamento em saúde, pois permitem estimar o custo global de uma dada patologia no País.
RESUMOA obesidade é um grave problema de saúde pública e seu aumento tem sido associado ao crescimento do consumo de ultraprocessados em diversas regiões, incluindo a América Latina. Os guias alimentares são diretrizes oficiais para a promoção da alimentação saudável e podem servir como ferramentas para a prevenção da obesidade. O Brasil foi o primeiro país a adotar o nível de processamento para classificar os alimentos e fazer recomendações a partir dessa classificação em seus guias alimentares. Assim, o objetivo deste artigo é compartilhar a experiência brasileira na incorporação de tais recomendações como diretrizes oficiais e na elaboração de guias alimentares. Para o desenvolvimento dos guias alimentares brasileiros, o Ministério da Saúde buscou valorizar a construção coletiva, com participação e discussão do conteúdo por atores estratégicos. No Guia Alimentar para Crianças Brasileiras Menores de 2 Anos houve aprimoramento do processo, com formação de um comitê gestor e um comitê de monitoramento político, chamada pública de pesquisadores e profissionais de saúde e oficinas de escuta com atores-chave para a definição do escopo do guia. O processo de validação contou com oficinas adicionais, com presença de atores estratégicos, além de oficinas em todos os estados para debate e mobilização para participação na consulta pública on-line. Como resultado do aprendizado, destacamos a importância da participação de atores estratégicos e a necessidade de dar ampla transparência ao processo de elaboração e validação de guias alimentares.
RESUMOO objetivo do artigo é descrever e comentar as ações implementadas em nível federal, no período de 2014 a 2018, no escopo da Estratégia Intersetorial de Prevenção e Controle da Obesidade, publicada pelo governo federal no Brasil. A Estratégia tem por objetivo prevenir e controlar a obesidade e foi organizada em seis grandes eixos de ação: 1) disponibilidade e acesso a alimentos adequados e saudáveis; 2) educação, comunicação e informação; 3) promoção de modos de vida saudáveis em ambientes específicos/territórios; 4) vigilância alimentar e nutricional, 5) atenção integral à saúde do indivíduo com sobrepeso/obesidade na rede de saúde e 6) regulação e controle da qualidade e inocuidade de alimentos. Para levantamento das informações, questionários semiestruturados foram enviados a representantes dos Ministérios da Educação, Saúde, Planejamento, Cidadania e Desenvolvimento Agrário. As ações implementadas são apresentadas segundo os eixos da Estratégia. Resultados positivos foram alcançados a partir do modelo de coordenação adotado, com gestão compartilhada, ainda que desafios necessitem ser superados. A agenda de ações propostas na Estratégia está alinhada ao contexto brasileiro e o modelo intersetorial de gestão permanece vigente até o período de elaboração do artigo.
