Cerebral palsy remains a significant problem today, despite scientific and technological progress. The high degree of disability and unsatisfactory quality of life of patients in this category, necessitates the search for and implementation of the latest treatments, one of which is cell transplantation. The use of regenerative cell technologies in the treatment of patients with cerebral palsy is extremely promising. Numerous experimental studies have significantly expanded the understanding of the mechanisms of the effect of the use of stem cells in cerebral palsy. Clinical applications of stem cells of different origins are safe, which is one of the prerequisites for continuing research in this area. This review is devoted to the use of regenerative cell technologies in the treatment of cerebral palsy, the current state and prospects of the approach to the treatment of cerebral palsy with stem cells.
Demyelinating diseases, especially multiple sclerosis (MS) is not only medical but also socio-economic issue. Unsatisfactory quality of life and high degree of disability in these patients require the implementation of the advanced treatments, such as stem cell transplantation. Numerous experimental and clinical studies are being conducted, revealing new mechanisms of the action of stem cells in demyelinating diseases. Further research is needed to understand many more aspects of clinical use. This review is devoted to the use of regenerative cell technologies in the treatment of demyelinating diseases, the current state and prospects of the approach to the treatment of such pathology with stem cells.
Повторні нейрохірургічні втручання при травмі периферичних нервів верхньої кінцівки Вступ. Повторні нейрохірургічні втручання-один з найменш вивчених розділів хірургії периферичних нервів. Матеріали і методи. Проаналізовані результати лікування 135 хворих, яким проведене повторне хірургічне втручання на нервах верхніх кінцівок. Використані клініко-неврологічний, нейровізуалізуючі та нейрофізіологічні (НФ) методи дослідження, в тому числі стимуляційна електронейроміографія (ЕНМГ) та внутрішньом'язова голкова електроміографія (ЕМГ). Результати. У 135 хворих виявлене ушкодження 156 нервів. Повторні оперативні втручання, виконані в пізні строки після першої операції, мають деякі особливості. За ступенем відновлення рухової функції хороші й задовільні результати досягнуті у 114 (84,4%) повторно оперованих хворих. Хороший і задовільний ступінь відновлення чутливості відзначений у 116(75,9%) повторно оперованих на нервових стовбурах верхньої кінцівки. Висновки. 1. Клініко-нейрофізіологічний моніторинг процесу регенерації периферичних нервів дозволяє визначити подальшу тактику лікування. 2. Проведення повторного хірургічного лікування при травмі периферичних нервів покращує нейромоторну функцію верхньої кінцівки. Расстройства аутистического спектра (РАС) являются довольно распространенной патологией, которая диагностируется у 1 % детей и оказывает значительное влияние на жизнедеятельность индивида и его близких. РАС на данном этапе является неизлечимым заболеванием, но ранняя диагностика и своевременно начатые лечебно-реабилитационные мероприятия могут улучшить понимание особых потребностей пациента и помочь с адаптацией в обществе. Большое количество факторов ассоциированы с риском развития РАС: низкий вес ребенка при рождении, пожилой возраст родителей на момент зачатия, нарушения метаболизма и аутоиммунные заболевания у матери (диабет, ожирение), внутриутробные инфекции, прием вальпроевой кислоты или ингибиторов обратного захвата серотонина, интоксикация пестицидами. Часто у детей с РАС встречаются нарушения иммунитета, желудочно-кишечные расстройства и дисбактериоз кишечника. Исследования иммунного статуса детей с РАС подтвердили наличие патологии как минимум у половины из них, что отображается в патологическом цитокиновом профиле. Кроме того, установлена прямо пропорциональная связь между ростом уровня провоспалительных цитокинов и углублением нарушений социального поведения. Получены также данные, свидетельствующие об активации астроглии и микроглии у пациентов с РАС. Ввиду особенностей этиопатогенеза РАС одним из перспективных методов лечения может быть клеточная терапия. Клеточные регенеративные технологии являются чрезвычайно перспективными в лечении хронических инвалидизирующих заболеваний нервной системы, методы лечения которых в настоящее время отсутствуют. Данные многочисленных in vitro, доклинических и клинических исследований демонстрируют положительный эффект применения мезенхимальных стволовых клеток при ряде заболеваний, имеющих нейроаутоиммунную и нейродегенеративную составляющую. Данный обзор освещает современное состояние проблемы лечения пациентов с расстройствами аутистического спектра с помощью регенеративных клеточных технологий.
Multiple sclerosis is a demyelinating disease of the nervous system that affects young people of working age and quickly leads to disability. Treatment of this pathology with umbilical cord mesenchymal stem cells is promising, given their immunomodulatory and neurotrophic properties. The study involved 27 patients diagnosed with multiple sclerosis, 12 of whom underwent combined treatment (intravenous and intrathecal administration) of umbilical cord multipotent mesenchymal stromal/stem cells. The effectiveness of treatment was determined by the degree of neurological deficit and spasticity. Combined treatment with umbilical cord mesenchymal stem cells significantly improves the condition of patients with multiple sclerosis and promotes the regression of neurological deficits and spasticity. This treatment is safe, but for a deeper study, it is necessary to continue research in this area.
