BackgroundThe rate of spontaneous regression in CIN III lesions is controversial. Whereas some studies have reported high regression rates of up to 38 % after prolonged biopsy-conus intervals, others have shown rates between 0 and 4 % without considering time intervals. Identification of young patients with potentially regressing CIN III could offer the chance to avoid conisation, thus lowering the risk of preterm labour.MethodsTo further clarify the facts, we retrospectively compared 635 biopsies showing CIN III with the diagnosis of the conisation. Either regression (CIN I or less) or non-regression (CIN II and higher) was recorded. Diagnoses were made by light microscopy and p16 immunostaining.ResultsConisation was performed between 2 and 463 days after biopsy (median 8.9 weeks). Six hundred twenty one (98 %) were HPV-HR positive. In 345 cases, HPV subtyping was available, showing HPV16 infection in 57 %. Routine processing of the conisation tissue showed no corresponding CIN lesion (< CIN II) in 40 cases (6.3 %). Additional step sectioning of the tissue revealed small CIN II+ lesions in 80 %. Finally, eight cases (1.3 %) fulfilled the criteria of regression. No regression was seen in HPV16 positive cases. Twelve invasive carcinomas were detected by routine processing of the conisation tissue.ConclusionThese results are in contrast with some prior reports that might have overestimated spontaneous regression of CIN III. Study size and an accurate discrimination between CIN II and CIN III lesions by histopathology seem to be the most likely factors to explain the diverging results published. Complete step sectioning of the whole tissue is also mandatory in questionable cases. Although theories exist that the initial biopsy might stimulate the immune system, thus triggering regression within weeks, our data do not substantially support such a mechanism. Overall, the chance of a CIN III lesion to regress rapidly within weeks or months after diagnosis seems to be small. We found more previously undetected invasive cancer than we observed regression. Therefore, a change in the current policy to treat CIN III lesions is unwarranted.
ZusammenfassungDas Lipödem war bis vor einigen Jahren ein eher unbekanntes Erkrankungsbild. In den letzten 5 Jahren gewann die Lipödemerkrankung immer mehr Bekanntheit, insbesondere durch die mediale Aufmerksamkeit. Neben der konservativen Behandlung durch eine komplex-konservative Entstauungstherapie gibt es immer mehr Studien, die das Potenzial eines Liposuktionsverfahrens zur erfolgreichen Therapie des Lipödems nahelegen. Infolgedessen werden Betroffene dieser Erkrankung zunehmend bei Plastischen Chirurgen vorstellig mit dem Wunsch nach Korrektur und Schmerzlinderung. Da die Liposuktion in Deutschland allerdings seitens des gemeinsamen Bundesausschusses (G-BA) noch nicht als Behandlungsalternative zur Therapie von Lipödemerkrankten positiv bewertet worden ist, ist eine Leistungserbringung zu Lasten der GKV immer noch eine Einzelfallentscheidung, welche gesondert von den Patientinnen in Zusammenarbeit mit dem behandelnden Plastischen Chirurgen beantragt werden muss. Im vorliegenden Übersichtsartikel legen wir die aktuelle Rechtslage dar und geben Hilfestellung bei der Beantragung von Kostenübernahmen.
ZusammenfassungBei der Erstellung von Patientenaufnahmen und deren Verbreitung bzw. Veröffentlichungen sind vielfältige Rechte betroffen, die beachtet werden sollten. Es ist unerlässlich zu wissen, wann tatsächlich eine schriftliche Einwilligung des Patienten vorliegen sollte und wann nicht. Entscheidend ist auch, dass trotz einer vorliegenden schriftlichen Einwilligung des Patienten, Gesetze im deutschen Rechtsystem existieren, die darüber hinaus eine Veröffentlichung reglementieren. Dies ist ein schwierig zu durchschauendes juristisches Thema, welches viele ärztliche Kollegen verunsichert. In der folgenden Übersichtsarbeit haben wir versucht die von dieser Thematik betroffenen wichtigsten Gesetze verständlich in einem nachvollziehbaren Zusammenhang zu bringen.
ZusammenfassungDie Abrechnung der Liposuktion des Lipödems gestaltet sich mittlerweile mannigfaltig. Grundlage hierfür ist die Entscheidung des Gemeinsamen Bundesausschusses (G-BA), das Lipödem Stadium III bis zum Abschluss der Erprobungsstudie zu Lasten der Gesetzlichen Krankenversicherungen (GKV) abrechenbar zu machen, sofern die Qualitätssicherungsrichtlinien erfüllt sind. Demnach kann die operative Therapie bei Lipödem Stadium III sowohl stationär, wie auch ambulant abgerechnet werden, während die Übernahme der operativen Behandlung der anderen beiden Stadien weiterhin einer Einzelfallentscheidung der GKV vorbehalten bleibt und aktuell fast immer abgelehnt wird. Daher werden die Behandlungskosten von den Betroffenen häufig privatärztlich oder als Selbstzahler beglichen. Im Rahmen gerichtlicher Auseinandersetzungen stellt sich immer wieder die Frage nach der korrekten Abrechnung des Lipödems, welche Gutachter und Gerichte wiederholt vor eine schwierige Aufgabe stellt. Im folgenden Artikel stellen wir als AG Lipödem der Deutschen Gesellschaft für Plastische, Rekonstruktive und Ästhetische Chirurgie eine Übersicht über die verschiedenen Abrechnungsmodalitäten dar und stellen einen Vorschlag zur korrekten Abrechnung der Liposuktion des Lipödems im Rahmen der GOÄ vor.
