Social network indices such as being single or widowed are associated with CAC, independent of age and coronary risk factors. Because coronary calcification has been identified as a potential marker of early atherosclerosis, these findings may partially explain the predictive value of limited social networks for future adverse cardiovascular health outcomes.
The present study identifies and compares the effects of two types of motor practice on stuttering adaptation. The study was designed to determine if whispered reading practice affects stuttering adaptation, and if practice in reading aloud is superior to whispered reading in promoting adaptation. In a control condition, eight stutterers read one of two matched passages aloud five times in succession. In an experimental condition, the remaining passage was read aloud on the first and fifth trials, and was read in a whisper on Trials 2, 3, and 4. The results indicated that (1) the stutterers' adaptation in the control condition was similar to the typical course of adaptation, (2) the three intervening whispered readings in the experimental condition neither inhibited nor facilitated adaptation, and (3) practice in reading aloud was superior to whispered practice in promoting stuttering adaptation. It was concluded that whispered reading practice does not facilitate stuttering adaptation when the measurement of that process requires reading aloud. Traditionally, adaptation has been measured only during reading done aloud. Consequently, practice in reading aloud has the greatest positive influence on the adaptation process.
Parent to Parent of Vermont, WinooskiAs service providers, speech-language pathologists are in the midst of a transition from the "expert" model of intervention to forming partnerships with families and serving as resources. Although the shift from client-centered to family-centered service delivery is underway, little information has been made available on steps being taken at the pre-service training level to accomplish this change and the success of such efforts. This paper describes an innovative approach to preparing speech-language pathology students to be family centered in their professional interactions and service delivery. Over a 4-year period, 41 students embarking on their first semester of clinical training were paired with families of children with special needs for a family visit of 2 to 4 hours in length. The components of the training included previsit classroom exercises focused on personal values clarification and language sensitivity, visit orientation for the students and families, a family visit, journal writing by students about the visit experience, and post-visit class discussion of the visit experiences and learning outcomes. Pre-and post-measurement of students' attitudes regarding a family's role in intervention reflected a statistically significant change in students' responses to questionnaire items. The students' responses indicated an increase in family-centered attitudes and demonstrated the effectiveness of this approach in shaping those attitudes.
Background The enhanced knowledge of cancer biology has led to considerable advancement in systemic therapy for advanced breast cancer. Recently, studies showed that cyclin-dependent kinase (CDK) 4/6 inhibitor, when added to endocrine therapy, had improved the outcomes of patients with advanced ER-positive HER2-negative breast cancer. However, the disease often progresses following a period of treatment response. In a subset of patients, disease progression may occur at limited sites, i.e., oligoprogressive disease (OPD). In the past few years, stereotactic radiotherapy (SRT) has emerged as a safe and effective treatment for advanced cancer when delivered to limited metastatic sites. Hence, it is worth investigating the role of SRT in the setting of oligoprogressive breast cancer. Method AVATAR is a multicentre phase II registry trial of SRT with endocrine therapy and CDK 4/6 inhibitor for the management of advanced ER-positive HER2-negative breast cancer. The study aims to enrol 32 patients with OPD limited to 5 lesions. The primary endpoint of the study is time to change systemic therapy measured from the commencement of SRT to change in systemic therapy. Secondary objectives include overall survival, progression free survival and treatment related toxicity. The exploratory objective is to describe the time to change in systemic therapy by the site (bone only vs. non-bone lesions) and number (1 vs. > 1) of OPD. Discussion This study aims to explore the effect of SRT in maximising the benefit of systemic therapy in patients with oligoprogressive ER-positive HER2-negative breast cancer. This approach might help reduce the burden of disease and improve the life quality in these patients. Trial registration ACTRN, ACTRN12620001212943.
