Résumé -De nombreuses colonnes d'extraction en phase solide dédiées à l'extraction des toxiques et leurs protocoles spécifiques sont proposées à l'analyste. Nous avons optimisé l'un de ces protocoles pour l'extraction à partir de sang total -en une seule étape -de principes actifs de médicaments acides, neutres et basiques. Chaque niveau du protocole a été conçu de manière à obtenir les meilleurs rendements d'extraction. L'analyse des extraits (identification et quantification) a été réalisée à l'aide d'un système de High Performance Liquid Chromatography system with Diode Array Detector (HPLC-DAD). La colonne SPE Evolute ABN r (Acid Base Neutral) fournie par Argonaut est celle qui extrait le mieux les différents composés analysés parmi toutes les cartouches testées. La colonne Oasis HLB r (Hydrophilic Lipophilic Balance) proposée par Waters permet également l'extraction de la totalité des molécules mais avec des rendements inférieurs. Quant aux autres colonnes étudiées (distribuées par Varian et Phenomenex), elles ont pré-senté des résultats insuffisants dans le cadre de ce projet. Les limites de détection et de quantification obtenues avec ce protocole optimisé ont été comparées aux concentrations thérapeutiques et toxiques habituellement retrouvées. La mise au point de cette nouvelle procédure a permis d'optimiser une méthode de dépistage large (criblage) de médicaments fréquemment rencontrés dans les matrices biologiques en toxicologie clinique et médico-légale. Many solid phase extraction columns which are dedicated to the extraction of various drugs associated with their specific protocols are proposed to the analyst. Résumé -Un enfant de 6 ans est hospitalisé en réanimation pédiatrique en raison d'une encéphalite d'aggravation rapide l'ayant mené dans le coma, sans diagnostic précis. Le quatrième jour, une recherche sanguine de toxiques ré-vèle la présence d'halopéridol et de rispéridone non prescrits a priori à l'enfant, à des concentrations thérapeutiques. Cinq mois plus tard, cet enfant est transféré pour rééducation à l'hôpital de Garches. Devant le contexte psychiatrique familial, les médecins évoquent la possibilité d'un syndrome de Münchausen par procuration. Une analyse de cheveux est demandée afin de confirmer l'éventuelle administration des deux neuroleptiques. Les cheveux étant relativement courts, un prélèvement de poils de jambe est également réalisé. L'halopéridol et la rispéridone n'ont pas été retrouvés en chromatographie liquide couplée à la spectrométrie de masse (CL-SM/SM) ni dans les cheveux ni dans les poils, ne permettant pas d'éclairer le diagnostic. En revanche, il a été retrouvé les molécules correspondant au traitement reçu durant l'hospitalisation. Ont ainsi été mis en évidence dans les deux segments de cheveux étudiés et les poils du métoclopramide ( Mots clés : Neuroleptiques, métoclopramide, dompéridone, cheveux, poils, syndrome de Münchausen, CL/SM/SM Abstract -We report here the case of a 6-year-old boy, hospitalized in a paediatric resuscitation service because of severe encephalitis which left him in a coma, without any specific diagnosis. Four days after his entry, a blood analysis of toxics was carried out and revealed haloperidol and risperidon at therapeutic concentrations. Months later, the boy was transferred for re-education to Garches Hospital. Knowing about the psychiatric familial context, doctors considered Münchausen syndrome by proxy. So, a hair analysis was requested in order to confirm the possible administration of these two neuroleptics. Analysis was carried out on hair samples and leg hair with liquid chromatography coupled with mass spectrometry (LC-MS/MS). Haloperidol and risperidon were not found, probably because sampling was too late. However, with a quantification method developed and validated on this occasion, we measured in two segments of the hair and in leg hair metoclopramid (301 and 210 pg/mg in hair, 799 pg/mg in leg hair) and domperidon (703 pg/mg, 1176 pg/mg and 108 pg/mg, respectively). Diazepam and its metabolites, nordiazepam and oxazepam, were also measured in hair (373 and 391 pg/mg, 271 and 249 pg/mg and 68 and 111 pg/mg, respectively), all these compounds reflecting the hospital treatment. We report here the first description of domperidon and metoclopramid concentrations in a Caucasian blond child's hair and leg hair in chronic therapeutic use.
