Objetivo: Revisar o diagnóstico e o tratamento da esteatose hepática aguda da gestação (EHAG). Relato de caso: Paciente do sexo feminino, 30 anos, primigesta na 36ª semana de gestação, pré-natal de risco habitual, foi admitida com relato de icterícia, dor abdominal difusa e náuseas. Negava febre, colúria, acolia fecal ou prurido. Sem alterações no apetite. Movimentação fetal presente. Sem perdas vaginais. Ao exame da admissão, apresentava-se estável hemodinamicamente, normotensa, ictérica 3+/4+ e afebril. Batimentos cadíacos fetais de 145 bpm, dinâmica uterina ausente e tônus uterino normal. Os exames iniciais mostraram alteração na função hepática (FA 560 / GGT 206), hiperbilirrubinemia com predomínio de bilirrubina direta (BD) (bilirrubina total 10 / BD 6,21), RNI 2,13, tempo de tromboplastina parcial ativado (TTPa 52,4 / controle 27) e plaquetas de 135.000. Foi hospitalizada e indicou-se a interrupção da gestação, questionando-se EHAG. O parto não teve intercorrências e o recém-nascido foi encaminhado para o alojamento conjunto. Resultados: A paciente evoluiu com piora dos exames laboratoriais e hipoglicemia e foi encaminhada ao centro de terapia intensiva (CTI), onde permaneceu por quatro dias. Recebeu alta do CTI e manteve boa estabilidade hemodinâmica. A alta hospitalar ocorreu no 8º dia pós-parto e a paciente manteve o acompanhamento ambulatorial com equipe da clínica médica e da obstetrícia. Conclusão: A EHAG é um diagnóstico de exclusão, e sua identificação e tratamento precoces são essenciais para melhorar a sobrevida da paciente e do feto.
