Background/Aim. Treatment of periodontitis undergoes several treatment phases. Non-surgical periodontal treatment (NSPT) represents the basic treatment stage, and it is applied to all the patients undergoing periodontal treatment. Adjunctive antimicrobial photodynamic therapy (aPDT) is one of several contemporary and relatively new possibilities with a role to inactivate microorganisms responsible for the occurrence and progression of the disease. The aim of this study was to comparatively analyze the clinical and microbiological effects of the NSPT alone, as well as combined with aPDT. Methods. A split-mouth method design was used in a prospective randomized controlled trial. The following clinical parameters were registered and monitored: plaque index (PI), bleeding on probing, probing depth (PD), and clinical attachment level (CAL). The presence of microorganisms Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, and Treponema denticola was detected by the polymerase chain reaction (PCR) method. Samples were tested before the therapy, as well as three months after the therapy. Therapeutic modalities of NSPT and NSPT with adjunct aPDT were examined. Results. All of the analyzed clinical parameters proved statistically significant improvement after the application of both treatment modalities (p < 0.001). Microbiological analyses showed that the total number of microorganisms was statistically significantly lower after the application of both methods (p < 0.001). Following the treatment, there was a statistically significantly reduced number of microorganisms Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, and Treponema denticola (p < 0.001). NSPT combined with aPDT led to a statistically significant improvement of both clinical parameters and microbiological status compared to NSPT applied on its own. Conclusion. The study showed improvement of all clinical indicators after the application of both treatment modalities. The total number of microorganisms was reduced as well as the number of specific microorganisms. Combining aPDT with NSPT led to a statistically significantly higher reduction in the number of microorganisms compared to NSPT alone.
Increase in the number of mast cells in the infected gingiva can be correlated with an increased influx of inflammatory cells from blood circulation into the gingival stroma, as well as with the collagen lysis, since these cells produce substances with collagenolytic potential. Based on the distribution of mast cells, it could be concluded that in the evolution of periodontal disease there are significant dynamic alterations in migration and localization of these cells.
Lichen planus je hronična, inflamatorna, autoimuna dermatoza koja se osim po koži često manifestuje i u usnoj duplji u različitim kliničkim oblicima. Prevalenca oboljenja u opštoj populaciji iznosi 1% do 2%. Etiopatogeneza je još uvek nedovoljno razjašnjena. Za adekvatnu dijagnozu oralnog lihen planusa (OLP) pored osnovnih metoda, anamneze i kliničkog pregleda, od suštinskog značaja je i histopatologija. Veoma raznolika i bogata histološka slika je zajednička karakteristika svih oblika oralnog lihen planusa. U radu je prikazan slučaj oralnog lihen planusa kod pacijenta starosti 49 godina koji se javio na Kliniku za stomatologiju, Medicinskog fakulteta Priština, zbog simptoma žarenja i pečenja kao i promena na obraznoj sluzokoži. Uzet je biopsijski uzorak tkiva i histopatološkim nalazom potvrđena klinička dijagnoza lihen planusa. Zbog mogućnosti da lezije maligno transformišu, dugoročno praćenje pacijenata sa ovim oboljenjem je od izuzetnog značaja.Ključne reči: Oralni lihen planus, histopatološki nalaz UVODLichen planus je hronično mukokutano, inflamatorno oboljenje, nepoznate etiologije koje osim kože često napada i sluzokožu usne duplje. Prema podacima iz literature ova se bolest u 30 do 70% obolelih može pojaviti na oralnom epitelu, a da promene na koži nisu izražene. Lihen planus je rasprostranjen na svim kontinentima i među svim rasama. Prevalenca oboljenja sa oralnom lokalizacijom u opštoj populaciji kreće se od 0.1% do 2.2%, sa većom učestalošću kod žena starosti između 40 i 50 godina [1,2]. Bolest je hroničnog toka sa povremenim egzacerbacijama. Prva saznanja o ovoj bolesti potiču iz 1869.godine, kada je Erasmus Wilson opisao promene kod tri bolesnika i definisao ih kao lichen planus mucosae [3], dok je Dubreuilh 1906 godine. dao prvi opis histološke slike OLP [4].Etiologija lihen planusa je još uvek nedovoljno razjašnjena. Novija istraživanja ukazuju na multifaktorijlnu etiologiju. Smatra se da različiti spoljni agensi, kao recimo virusi, i brojni unutrašnji faktori, stres, anksioznost, nasleđe, autoimunitet i drugi, mogu samostalno ili udruženi da dovedu do pojave lihena [5]. Za razliku od kožnih lezija (Lichen ruber), gde je lihenska papula crvene boje i jasno ograničena, oralni lihen karakterišu bele papule, diskretne ili u obliku različitih formacija. Oralni lihen planus (OLP) se može razviti na bilo kom delu oralne sluzokože. Najčešće se pojavljuje simetrično obostrano na sluzokoži obraza i to u predelu okluzalne linije i u retro molarnom regionu, zatim na jeziku, gingivi, dok su ostale lokalizacije znatno ređe. OLP se može pojaviti u više kliničkih oblika: retikularni, atrofični ili eritematozni, i erozivni sa ulceracijama i buloznim lezijama. Background/Aim. Persons identification and their age assessment is necessary in vast number of cases and there are different methods used for such purposes. Numerous studies indicate that the third molar development could play a crucial role in identifying an individual`s age. The aim of this study was to determine the possibility for estimating the chronological age based on the third molars development stages in children and young adults in Serbian population. Methods. A total of 570 Serbian patients aged 6-27 years were chosen for this study. Out of the total number of subjects, there were 248 males with an average age of 12.21±3.91 years, and 332 females with an average age of 12.88±4.06 years. Stages of dental formation were determined on Orthopantomograms by comparing with standard Demirjian radiographic appearances. Results. Third molars mineralization occurs more rapidly in males than in females. Large percentage of persons with third molar (stage H development according to Demirjian) is older than 18 years of age. Conclusion. Тhird molar mineralization stages determination on orthopantomograms is useful additional method for determination of chronological age in living individual. That might be important for forensic studies, focusing on the determination of the legally important ages.The variability among different ethnic groups has to be taken into consideration when applying this method. It is necessary to carry out extensive surveys on a larger sample in order to determine the norms for assessing the dental and chronological age within Serbian population. ApstraktUvod/Cilj. Identifikacija osoba i procena njihove starosti su neоphodni u velikom broju slučajeva i postoje različite metode koje se koriste u takve svrhe. Brojne studije ukazuju na to da razvoj trećeg molara može igrati ključnu ulogu u određivanju starosti osobe. Cilj ove studije je bio da se ispita mogućnost procene hronološke starosti na osnovu stadijuma razvoja trećeg molara kod dece i mladih u srpskoj populaciji. Metode. Za ovu studiju odabrano je ukupno 570 pacijenata iz Srbije uzrasta 6-27 godina. Od ukupnog broja ispitanika, 248 osoba je muškog pola prosečne starosti 12.21±3.91 godina, dok je 332 osobe ženskog pola prosečne starosti 12.88±4.06 godina. Stadijumi dentalne zrelosti odrеđeni su na ortopantomogramima poređenjem sa standardima po Demirjianu. Rezultaiti. Mineralizacija trećeg molara odvija se brže kod muškaraca nego kod žena. Veliki procenat osoba sa formiranim trećim molarima (stadijum razvoja X prema Demirjianu) je stariji od 18 godina. Zaključak. Određivanje stadijuma mineralizacije trećeg molara na ortopantomogramima prеdstavlja korisnu metodu za određivanje hronološke starosti živih osoba. Ovo može biti značajno za forenzička istraživanja, posebno zbog pravnog aspekta utvrđivanja da li je osoba punoletna. Parodontopatija predstavlja inflamantornu reakciju na gram-negativne anaerobne bakterijske infektivne agense koja pogađa potporni aparat zuba, uključujući gingivu, parodontalni ligament, cement i alveolarnu kost. Cilj istraživanja bio je da se utvrdi kvantitativno-kvalitativni sastav mokroorganizama subgingivalnog plaka kod pacijenata obolelih od hronične i agresivne parodontopatije PCR metodom (eng. Polimerase Chain Reaction), a zatim proceni povezanost različitih stepena inflamacije i destrukcije tkiva parodoncijuma sa prisustvom i koncetracijom ovih mikroorganizama. Ispitivanjem je obuhvaćeno 70 pacijenata starosti od 16 do 65 godina. Utvrđivanje mikroorganizama u subgingivalnom plaku, određen je PCR metodom. U cilju postavljanja dijagnoze i određivanja stepena destrukcije parodontalnog tkiva, bili su korišćeni standardni epidemiološki kriterijumi: plak indeks (Silness-Löe), gingivalni indeks (Lӧe-Silness), SBI indeks (Mühleman-Son) i PDDZ. Prisustvo parodontopatogena u subgingivalnom plaku pokazalo je statističku vezu sa kliničkim parametrima težine parodontopatije i inflamacije gingive. Nalaz Porphyromonas gingivalis i Actinobacillus actinomycetemcomitans je bio statistički značajno češći kod pacijenata sa srednje teškom i teškom formom parodontopatije u odnosu na prosečnu dubinu parodontalnih džepova. Detekcija P.g. i A.a. je bila statistički značajno češća kod osoba sa umerenom i intezivnom inflamacijom gingive. Kvantitativno-kvalitativni sastav mikroorganizama subgingivalnog plaka ima direktan uticaj na inflamatorno-destruktivne procese u parodontalnim tkivima.Ključne reči: parodontopatija, subgingivalni plak, PCR. UVODParodontopatija, oboljenje potpornog aparata zuba, jedno je od najraširenijih oboljenja današnjice koje zahvata čitavu ljudsku populaciju bez obzira na sistemski zdravstveni status pacijenata, rasu, socijalnu, geografsku, versku, polnu i svaku drugu pripadnost. Klinička prezentacija bolesti varira od perzistentnog gingivitisa do pojedinih formi destruktivnog parodontitisa. Patološke promene koje se odvijaju u toku parodontopatije su destruktivne prirode i vode ka postepenom ili progresivnom obolevanju parodoncijuma i njegovoj anatomskoj i funkcionalnoj dezintegraciji, što rezultira postepenim rasklaćenjem, a kasnije i gubitkom jednog ili više zuba [1]. Epidemiološki podaci iz čitavog sveta ukazuju na izuzetnu rasprostranjenost gingivitisa i parodontopatije. Smatra se da su ovo, pored karijesa, najrasprostranjenija oboljenja ljudskog roda. Prema "Beogradskoj studiji", učestalost oboljenja parodoncijuma u domaćoj populaciji među osobama starijim od 18 godina je 84,6% i kreće se do 86,6% kod osoba starijih od 65 godina. Od ovog broja, blagi stepen destrukcije parodoncijuma ima 14,1% osoba, srednji 20,3%, dok su najteža oštećenja kod 10,4% ispitanika. Prema tome, ukupno 44,8% pregledanih osoba ima oštećenje dubljih tkiva parodoncijuma i formirane parodontalne džepove [2].
