O presente estudo tem como objetivo revisar na literatura a eficácia da laserterapia de baixa intensidade (LBI) como tratamento adjuvante no pós-operatório de exodontias de terceiros molares. Para isso, foi realizada revisão integrativa da literatura, estruturada em seis etapas distintas, previamente estabelecidas, mediante acesso virtual às bases de dados MEDLINE via PubMed e BVS. Após buscas bibliográficas, identificou-se um total de 40 publicações, das quais, após aplicação dos critérios de inclusão e exclusão, dez artigos foram selecionados. Com base na literatura revisada, pode-se perceber que a LBI possui importantes ações terapêuticas eficazes no tratamento adjuvante para redução de morbidades pós exodontia de terceiros molares, dentre as quais destacam-se o potencial analgésico e a redução do edema local e trismo. Contudo, alguns estudos divergem quanto aos seus efeitos e este fato decorre, principalmente, quanto a falta de padronização ao protocolo clínico empregado na pesquisa. Nesse sentido, novos estudos devem ser realizados com o foco na padronização do protocolo de intervenção como parâmetro de irradiação, desenho de estudo, tempo e locais de aplicação da LBI, dessa forma permitindo assim conclusões mais objetivas para guiar a prática clínica.
This study aimed to understand how hyperbaric oxygen therapy can act in the treatment of osteoradionecrosis of the jaws in patients with head and neck cancer. A total of 266 publications were found. After applying the inclusion and exclusion criteria, 19 articles were selected. Osteoradionecrosis is a condition with great potential for bone destruction, and therefore, many methods for treatment have been vehemently studied. Although the mechanism of action of hyperbaric oxygen therapy is not fully understood, it is known that the physiochemistry that drives the action is a result of two factors: 100% inspired oxygen and exposure to high atmospheric pressure. In addition, oxygen when present in larger quantities in the body can act to heal necrotic regions, oxygenating regions where hypoxia and hypoperfusion have occurred. However, this therapy is not totally innocuous and has significant issues for the patient, among them, the limitation of its access, and with this, the cost for oxygen therapy is very high. The negative impact that osteoradionecrosis has on oncology patients can be seen, and how hyperbaric therapy can act in the treatment of necrosis caused by oncotherapy in patients with head and neck cancer, due to its numerous effects on oxidative metabolism. In contrast, it is necessary to correctly evaluate the indication of the treatment, because the therapy has relevant adverse effects that deserve attention.
This study aimed to understand how fibrin rich in platelets and leukocytes acts as an alternative for tissue regeneration in surgeries for maxillary sinus elevation. This integrative review was developed in six steps, where the guiding question was formulated according to the PICo strategy. Thus, a total of 142 publications were searched and a total of 23 articles were selected for this review after applying the inclusion and exclusion criteria. Platelet- and leukocyte-rich fibrin (L-PRF) has been increasingly investigated as a potential bioactive substance for more effective bone regeneration, not only because it is considered an easily obtained autogenous material, but also due to its high concentration of fibrin, platelets and leukocytes, assisting in the process of angiogenesis, thanks to its growth factors, which contributes to bone formation when associated with grafts, as it multiplies fibroblasts and osteoblasts. Furthermore, for the surgical method, the use of L-PRF decreases the dispersion of the graft particles used, helping to condense more mineral and consequently resulting in a smaller volume introduced into the maxillary sinus, thus minimizing the time to obtain vertical bone height. However, since the preparation is properly autologous, the amount of L-PRF acquired is insufficient, causing a disadvantage in its use. In this sense, we realize the importance of L-PRF in maxillary sinus elevation surgery, as it is a simple, cheap and the most accessible technique for the production of autologous fibrin membrane or platelet concentrate.
