INTRODUÇÃOO transplante renal realizado a partir de 1954, nos Estados Unidos da América, é o melhor tratamento para a maioria dos pacientes com insuficiência renal crônica, superando os processos dialíticos e apresentando menor custo com melhor resultado.O receptor de um transplante renal é particularmente suscetível a complicações cirúrgicas, se comparado a uma cirurgia convencional 1,2 . Os princípios cirúrgicos dos transplantes renais surgiram no início do século, através dos trabalhos de Alex Carrel em animais de laboratório. Tecnicamente, tratava-se de desenvolver métodos viáveis para a anastosmose dos vasos sanguíneos, pré-requisitos essenciais para um implante bem sucedido. Assegurar o suprimento vascular no transplante renal é um princípio básico para a sobrevida do enxerto, bem como diagnosticar as complicações cirúrgicas, atuando prontamente na sua correção.Dentre as complicações cirúrgicas do transplante renal, destacam-se as complicações vasculares que podem ser venosas e arteriais. As complicações arteriais, principalmente a estenose de arté-ria renal, ocorrem mais freqüentemente 3 . O local do enxerto para a anastomose arterial do receptor tem papel importante no desenvolvimento da estenose de artéria renal. A anastomose arterial término-terminal com a artéria hipogás-trica (ilíaca interna) foi a técnica anastomótica arterial mais usada durante o período inicial dos transplantes renais 4 . Nesse sentido é interessante que se examine qual tipo de técnica anastomótica a ser realizada.Para alguns autores, a estenose de artéria renal no transplante poderia ser mais comum em situações nas quais fosse realizada a anastomose arterial término-lateral 5 e que seria, igualmente, prevalente em receptores de rim de doador cadá-ver e doador vivo, não relacionando sua ocorrência aos fatores imunológicos 6 . Já outros autores não correlacionaram nenhuma diferença na incidência de complicações vasculares entre anastomoses arteriais término-terminais ou término-laterais 6,13,24,25 . Ultrasonography is the main non-invasive technique for screening of ureterocele, but presents some difficulties for its diagnosis. Other supplementary diagnostic methods have the disadvantage of being invasive or using ionizing radiation. Magnetic resonance (MR) has a high sensitivity for diagnosing urinary tract malformations in adults and children. We report one case of ureterocele in a 1-year old child with the purpose of presenting its diagnosis through MR.
204THE JOURNAL OF UROLOGY® METHODS: We modified the standard 8.5 french infant optical urethrotome (Richard Wolf Medical Instruments Corporation, Vernon Hills, Illinois) to accommodate shaft insertion into the working element of a 9 french infant resectoscope. This resulted in blade shaft insertion at the twelve-o-clock position rather than the six-o-clock position giving an inferiorly directed blade. The first case scenario demonstrates incision of PUV s in a one week old infant. The crescent blade urethrotome is used to engage the valve tissue at the 5 and 7 o'clock positions and incise without the need to rotate orientation of the cystoscope. A wide open urethral channel is achieved. A 12 o'clock incision is not required here. The second scenario utilizes the crescent blade urethrotome to incise an obstructing ureterocele in a young female. The ureteral orifice is engaged with the blade, and a series of incisions are made horizontally until adequate unroofing of the ureterocele is achieved.RESULTS: This modified instrument allows use of the pediatric optical urethrotome in the proper upright and ergonomic position obviating the need to rotate the scope and lens system prior to incision. In addition, use of the instrument in this manner allows the surgeon more delicate manipulation of the scope required in pediatric patients, especially in neonates, during incision of PUVs. We have found this technique to be safe, effective, and minimize instrumentation of the delicate newborn male urethra. Although we have not required electrocautery, this instrument does allow passage of a bugbee electrode.CONCLUSIONS: The modified pediatric urethrotome is a well balanced and ergonomic instrument allowing safe and effective incision of PUV s and ureteroceles in the pediatric population.
