Cet article met en lumière les formes et les effets du développement des activités privées au sein de l'hôpital public. Les consultations privées, la mise à disposition d'infrastructures publiques à des médecins du secteur libéral ou encore le recours fréquent aux intérimaires transforment les principes qui fondent l'ethos de service public de plusieurs façons. Le développement de ces activités fragilise l'égalité de traitement en différenciant les filières de soins selon les ressources des patient•es, remet en cause la défense de l'intérêt général en privilégiant les intérêts pécuniaires des médecins et ceux de l'hôpital sur ceux des patient•es, et affaiblit les collectifs de travail en favorisant la compétition entre les agents. De telles évolutions poussent certains agents à quitter l'hôpital public, tandis que d'autresfemmes, agents peu qualifiés, médecins des spécialités peu rentables -y restent en déplorant que certaines valeurs s'y érodent. When the race for profitability undermines the public service ethosThis article highlights the forms and effects of the increase in private activities within the public hospital. Private consultations, the loan of public infrastructures to private doctors and the frequent use of temporary doctors undermine the principles that underpin the public service ethos in several ways. The development of these activities weakens equality of treatment by differentiating access to care, it calls into question the defence of the general interest by giving priority to the financial interests of doctors and hospitals over those of patients, and weakens work collectives by fostering competition between healthcare staff. Such changes in the work place put workers in a difficult dilemma: to leave orfor women, low-skilled workers, and physicians in low-profit medical specialities -stay in the state hospital organization they work in, deploring the erosion of certain values.

Les agents de l'hôpital public face à l'épidémie de Covid-19 Maud Gelly et Alexis Spire Résumé L'épidémie de Covid-19 survenue en mars 2020 a mobilisé tous les personnels des hôpitaux les plus touchés et a contraint leur administration à suspendre les restrictions budgétaires pour accueillir l'afflux de malades. À partir d'une enquête qualitative menée dans deux hôpitaux d'Îlede-France et du Grand-Est, cette contribution montre ce qu'a représenté ce moment critique pour les personnels de ces institutions. Les auteurs mettent en évidence les ressorts d'un service public qui est confronté à des besoins débordant ses capacités. Dans ce contexte, les personnels ont consenti à fournir un surtravail et à s'investir dans leur mission sans compter, bien au-delà de ce que prévoient les règlements. Cet engagement trouve sa source dans l'intériorisation de la norme du travail bien fait et dans l'attachement à des collectifs de travail qui ont porté les agents durant cette période. Les rapports de domination entre catégories de personnels et entre services hospitaliers n'ont pas pour autant disparu et ont resurgi avec d'autant plus d'acuité une fois la crise passée.
