<p><strong>Introducción</strong></p><p><strong></strong>Se realizó un estudio de cohorte prospectivo para evaluar los resultados funcionales de pacientes laborales con fracturas de falange tratadas mediante placas y establecer factores de mal pronóstico.</p><p><strong> Materiales y Métodos</strong></p><p><strong></strong>Desde mayo de 2012 hasta mayo de 2014, 55 pacientes laborales con fracturas de falange fueron operados consecutivamente, mediante reducción y osteosíntesis con placa y tornillos. Cuarenta y dos (39 hombres, edad promedio 30.76 años) fueron evaluados, con 68 falanges operadas (primera falange 15, segunda falange 53). El 28% de las fracturas fueron expuestas; el 38,24%, conminutas y el 11,76% tenía compromiso articular. El seguimiento promedio fue de 3.38 meses.</p><p><strong> Resultados</strong></p><p><strong></strong>Se logró la consolidación ósea de todas las fracturas a los 1.8 meses. La movilidad para el pulgar (Gingrass) fue buena en dos casos y regular en uno. En los restantes dedos (Belsky), fue excelente (35%), buena (55%) y mala (9%). El puntaje DASH promedio fue de 18,53. Se observaron peores resultados en las fracturas de primera falange respecto de la segunda falange y en pacientes con más edad que en los más jóvenes, ambos con significancia estadística. No hubo relación entre el resultado y las demás variables estudiadas. Cuatro pacientes tuvieron complicaciones (9,5%).</p><p><strong> Conclusiones</strong></p><p><strong></strong>Al comparar nuestra serie con otras publicadas, hubo menos complicaciones y los resultados fueron similares, pero a diferencia de otros autores, no acostumbramos a retirar el material ni a realizar tenólisis ni artrólisis. Se logró la consolidación ósea en todos los casos y los resultados fueron satisfactorios en el 90% de los pacientes.</p>
Se presenta el caso de un hombre de 27 años con una lesión grave articular interfalángica proximal en el dedo anular de la mano derecha, provocada por un proyectil de arma de fuego, que fue tratada con doble injerto osteocondral de costilla. Tenía una fractura expuesta y déficit de stock óseo y cartilaginoso tanto en la primera como en la segunda falange. Luego de la limpieza inicial, a las 3 semanas, se realizó una artroplastia interfalángica proximal con doble injerto osteocondral de costilla y fijador externo distractor de Suzuki. Se describe con detalle la técnica quirúrgica. A las 10 semanas posoperatorias, la flexión activa interfalángicaera de 75° y la extensión activa, de -15°, con articulación estable. Alta laboral a los 3 meses de la cirugía, sin dolor y un puntaje DASH de 14,2. Las radiografías mostraron la incorporación de los injertos, sin reabsorción y con una articulación congruente. Se valora la ventaja de esta técnica en cuanto a la escasa morbilidad para la zona dadora y la versatilidad para las reconstrucciones con defectos articulares. El resultado funcional temprano subjetivo y objetivo fue satisfactorio. No se pueden descartar complicaciones futuras.
Objective: Cleft palates are still considered as impossible or very difficult to study in 2D or in 3D, and impossible in routine examination. We described a new approach to identify normal palate and it's pathology. Since 1 year, a new treatment of 3D acquisition can be applied with this method. Method: This easy 2D approach, has been realised with success on every fetuses seen, aged from 18 to 35 Weeks since 2002 (that is to say more than 7000 fetuses), sometimes completed with 3D view. Using precise guide marks, this approach allows a direct very echogenic visualisation of the osseous palate's posterior wall, proving it's integrity. Keeping the same guide lines, 3D thick view often visualise the whole palate. Results: With this approach the visualisation of the posterior wall has always been successful and registered. First we could definite criteria of a normal palate. We could also have cleft lips and we could demonstrate the associated cleft palate and the duplication of the uvula. Further we could diagnose cleft palate without cleft lips, short palate (helped with 3D view), and one suspected ogival palate, confirmed with a sagittal view (3D thick view wasn't available). All these findings have been confirmed by postnatal examination. There hasn't be any false negative or false positive cases about the integrity of the palate. We described the guidelines we used in this study with 2D and 3D view, the criteria of normal palate, it's pitfalls, and the sonographic characteristics of cleft and in ogival forms.Conclusion: This study demonstrate than the palate is possible to visualise, even in routine, with a very good prediction of it's normality or pathology. This will be a great help for parents avoiding the surprise of this pathology or eliminate the cleft palate's fear. It will be also useful for the surgeon in case of cleft lip for example to explain the parents the more precisely possible what will occur after birth.
Objetivo: Evaluar los resultados de diferentes neurotizaciones utilizadas para la flexión del codo en pacientes con lesión traumática del plexo braquial. Materiales y Métodos: Entre abril de 2012 y enero de 2019, se operaron 13 pacientes (12 hombres) con lesión traumática del plexo braquial, 5 con parálisis totales sin recuperación, 4 con parálisis totales que recuperaron el tronco inferior parcialmente y 4 con parálisis altas. Las neurotizaciones para la flexión del codo fueron: 3 nervios intercostales con injerto sural a nervio musculocutáneo o su(s) rama(s) motora(s) (4 pacientes), 3 nervios intercostales a nervio musculocutáneo sin injerto (3 pacientes), nervio espinal accesorio a ramas motoras del nervio musculocutáneo con injerto sural (2 pacientes), fascículos del nervio cubital a rama motora del bíceps (3 pacientes) y fascículos del nervio cubital y fascículos del nervio mediano a ramasmotoras del bíceps y braquial anterior (3 pacientes). Se evaluaron la fuerza de flexión del codo (M0-M5), el dolor con la escala analógica visual y se utilizó el puntaje DASH. El seguimiento promedio fue de 50 meses. Resultados: La fuerza de flexión del codo fue M5 (1 paciente), M4 (7 pacientes), M3 (1 paciente), M2 (1 paciente) y M1 (2 pacientes). El puntaje DASH promedio fue de 54,1 antes de la cirugía y 29,5 en el posoperatorio. El puntaje de dolor preoperatorio fue de 7 y de 0,9 posoperatorio. No hubo complicaciones. Conclusiones: Las neurotizaciones lograron resultados satisfactorios en la reconstrucción de la flexión activadel codo en pacientes con lesión del plexo braquial.
