We present the case of an 18-year-old high-level gymnast who sustained a stress fracture of the scaphoid associated with a distal radial epiphysiolysis. Clinical evaluation demonstrated decreased range of motion of the affected wrist and insidious pain on the snuffbox and tenderness on the distal radial physis. He was submitted to surgical treatment with scaphoid percutaneous fixation and radial styloid process in situ fixation. Clinical features improved, and he got back to competition 6 months after surgery without symptoms and with complete range of motion.
Apresenta-se o relato de caso de um ginasta de 18 anos de idade, que recebeu o diagnóstico de fratura por estresse do escafoide combinada com epifisiólise distal do rádio, e descreve-se seu tratamento. Após breve revisão da literatura sobre essa rara associação de lesões, os autores pedem atenção para a importância do exame físico e de imagem no correto diagnóstico e na observação de possíveis lesões combinadas.
Objective: To evaluate the results from transposition of the triceps for elbow flexion in patients with chronic and complete injury to the upper trunk of the brachial plexus. Methods: This was a retrospective study, including only patients who had biceps grade 0 and triceps grade 5, who underwent anterior transfer of the triceps muscle, performed between 1998 and 2005. The affected side, sex, type of accident, strength of elbow flexion, complications and patient satisfaction were investigated in 11 cases. Results: 10 patients were male; the age range was from 24 to 49 years, with a mean of 33.7 years. The minimum time between injury and surgery was 21 months (range 21-74 months). The left side was affected in eight cases, and the right only in three. Good results were obtained in 10 patients, who acquired elbow flexion strength of grade 3 (two cases) and grade 4 (eight cases), while one evolved unfavorably with grade 2 strength. Two cases had complications (initial compartment syndrome and insufficient tensioning). All the patients said that they were satisfied with the procedure. Conclusion: Anterior transposition of the triceps muscle provided patient satisfaction in all cases except one, attaining strength grade 4 in eight cases, grade 3 in two cases and grade 2 in one case.
O papel desempenhado pelas anormalidades da coluna cervical na gênese da cefaléia cervicogênica é destacado. O trabalho é o de análise do papel das disfunções e das anormalidades da coluna cervical na gênese das algias crânio-faciais. É ressaltada a importância da síndromes dolorosas miofasciais na gênese das cefaléias e do tratamento farmacológico e dos procedimentosde medicina física no controle desta condição álgica. A compreensão do papel destas anormalidades é importante, nãoapenas com finalidade de classificação como também para o estabelecimento de condutas terapêuticas em casos de dor crânio-facial.
We present a case report of an 18 year-old gymnast who was diagnosed with a scaphoid stress fracture associated with a distal radial epiphysiolysis, reporting the treatment of choice. After a brief literature review about this rare association, the authors ask for attention concerning the importance of physical and image examination in making right diagnosis and finding associated injuries.
