O desenvolvimento de aditivos cirúrgicos bioativos para regular a inflamação e aumentar a velocidade do processo de cicatrização é um dos grandes desafios da pesquisa clínica. Pois em procedimentos cirúrgicos muitas vezes utilizavam técnicas que, na maioria das vezes, eram bem invasivas e causavam desconforto ao paciente. Nesse sentido, a fibrina rica em plaquetas (PRF) surge como uma alternativa natural, viável e bastante satisfatória com resultados favoráveis e apresentando baixos riscos. O PRF (Fibrina Rica em Plaquetas) foi desenvolvido por Choukroun para uso em cirurgia oral e maxilofacial, e tem diversas áreas de aplicação dentro da odontologia, podendo citar o aumento ósseo na implantodontia, levantamento de seio maxilar, transplante alveolar, cirurgia periodontal estética, dentre outros. A revisão a seguir trata-se de um estudo de revisão de literatura sobre a propriedades da fibrina rica em plaquetas (PRF) aplicada a cirurgia oral-protocolo choukroun, com base em 24 artigos selecionados de 40 artigos pesquisados enfocando na sua preparação, vantagens e desvantagens de usá-lo em aplicações clínicas, especialmente seu uso na cirurgia oral.
A perda dos dentes frequentemente leva à extrusão dos dentes antagonistas, o que resulta em interferência oclusal e espaço inadequado para restaurar o espaço protético. Um pré-requisito para tratar más oclusões dentárias e esqueléticas se faz lançando mão de ancoragens ortodônticas, definida simplesmente como uma resistência ao movimento dentário indesejado e é fundamentalmente a base para tratar vários distúrbios oclusais. A ancoragem é obtida através de aparelhos extra e intrabucais e dos próprios dentes, sendo estes métodos limitados, necessitando de outros dispositivos auxiliares, e um deles são os mini-implantes ou mini-parafusos. Por possuir diâmetro reduzido, os mini-implantes têm sua inserção facilitada o que amplia o leque de sítios de instalação, sendo eficientes no tratamento da extrusão. Portanto, os mini- implantes proporcionam menos efeitos colaterais, mais conforto, técnica simples de ser executada, mas que demanda planejamento prévio, custo baixo e aumenta a efetividade dos tratamentos ortodônticos e consequentemente sua previsibilidade terapêutica, diminua a necessidade de que haja colaboração do paciente, exceto na parte de higienização.
O objetivo desse estudo é demonstrar a eficiência da Cirurgia Ortognática Bimaxilar em pacientes com Síndrome da Apnéia Obstrutiva do Sono. Para tanto, procede-se o caso clínico de uma paciente com retrognatismo, com queixas quanto a dificuldades para dormir, ronco noturno, dificuldade para respirar e pouca disposição ao longo do dia. Também se queixava de sua aparência facial. Por intermédio dos exames clínicos intra-oral, extra-oral e comprovado pelos exames de imagem sendo assim diagnosticada como classe II de Angle, sendo o tratamento com cirurgia ortognática pertinente a pacientes que exibem imperfeições faciais compreendendo os dentes e o esqueleto da face, não sendo sanadas apenas com tratamento ortodôntico. Pois a deficiencia a ser solucionado está na mandíbula e/ou maxila e não apenas no posicionamento dental. Desse modo, observa-se que os resultados obtidos através da cirurgia com osteotomias e miorrafia de tração da musculatura supra hióidea se torna eficiente. Concluiu-se que quando bem planejado e indicado, a cirurgia ortognática não se torna um procedimento invasivo, mas sim, eficaz no tratamento.
