Background: Dravet syndrome (DS) is a rare treatment-resistant epileptic encephalopathy, with a high mortality rate, resulting from mutations in the SCN1A gene (encoding the α1 subunit of the NaV1.1 channel). Studies show that cannabidiol is effective in reducing the frequency of seizures. Objective: Analyze the efficacy of cannabidiol for the treatment of refractory epilepsy in DS. Methods: In April 2021, the PubMed database was queried to search for the terms “Cannabidiol” AND “Dravet Syndrome”, with the filters: Clinical trial, Randomized controlled trial and the articles were limited to the last 5 years. With this, 8 articles were found, of which 3 were selected. Exclusion criteria: (1) articles that didn’t evaluate the effectiveness of treatment (2) review articles. Results: A randomized, double-blind study of 120 patients, which a dose of 20 mg/kg/day was used, found that cannabidiol use led to lower frequency of seizures, reducing from 12, 4 to 5.9 seizures per month; and 5% of participants were seizure-free. A similar reduction was documented by an open clinical trial, which there was a reduction of 39.5% in total seizure frequency and 2.9% were seizure-free. Additionally, another randomized, double-blind study reported a percentage reduction in seizure frequency of 48.7% for the group of patients given 10 mg/kg/day, and 45.7% for the group given 20 mg/kg/day. Conclusion: In DS patients, cannabidiol shows efficacy to reduce the frequency of seizures. However, more studies are needed to validate its application.
Introdução: A taxa de mortalidade infantil (TMI) é a frequência de óbitos de crianças menores de um ano de idade a cada mil crianças nascidas vivas. Objetivo: Caracterizar a incidência dos óbitos e o desempenho da TMI em relação às principais variáveis sociodemográficas e clínicas da criança e da mãe, na Macrorregião Leste de Minas Gerais, de 2008 a 2019. Materiais e Métodos: Foram utilizados dados secundários de domínios público dos Sistemas de Informação de Mortalidade e de Nascidos Vivos da Secretaria Estadual de Saúde de Minas Gerais na construção de um estudo ecológico de series temporais e abordagem descritiva observacional de caráter transversal. Resultados: Foram registrados 1.517 óbitos e TMI acumulada de 14,3/1000 nascidos vivos (NV). Na análise ano a ano não foi possível observar um padrão temporal conforme observado no âmbito nacional e estadual, sendo que a curva da evolução temporal da TMI mostrou-se anárquica. O componente neonatal precoce registrou as maiores taxas. Na análise dos dados maternos, destacou-se por estarem atreladas às taxas mais elevadas, a baixa escolaridade (123,5 e 21,5/1000 NV), a idade reprodutiva em extremos (27,5 e 52,6/1000 NV) e o parto vaginal (15,7/1000 NV). Já em relação à criança, o sexo masculino (15,6/1000 NV), a raça/cor branca (20,3/1000 NV) e nascimentos ocorridos em domicílio (328,24/1000 NV) apresentaram maior TMI. Observou-se um alto percentual de mortes evitáveis (68%) destacando aquelas que são evitáveis por adequada atenção a mulher na gestação como a mais pronunciada. Conclusões: Com base nesses achados, nota-se a necessidade da gestão integrada nas redes de atenção à saúde materno-infantil ampliando assim a capacidade dos municípios de assistir com qualidade a gestação, o parto e o nascimento, a fim de atuar na redução da mortalidade infantil.
