The femoral vein has been used as an alternative conduit to implant pacemakers in children of any weight. Such method associates endocardial pacing and good cosmetics. The aim of this study was to evaluate prospectively, since 1981, the long-term follow-up of 99 children, from newborn to 13 years old (average = 4.1 +/- 3.6 years, 56 girls), who underwent the implantation of pacemakers via the femoral vein. Atrioventricular block was present in 88% of patients, of congenital etiology in 39% and postoperative in 54%. Single chamber pacemakers were implanted in 92% of patients. During a mean follow-up of 5.3 +/- 5.0 years (maximum = 18.2 years), 5 patients died of cardiac causes, 4 of infection, 2 suddenly, and 3 of unknown causes. The 5-, 10- and 15-year actuarial survival rates were 83.7%, 75.7%, and 75.7%, respectively. Transfemoral leads were used for a mean of 48.9 +/- 44.0 months. Reasons for lead explantations were pacing failure in five patients, infection in eight, and elective in nine. The 2-, 5- and 10-year actuarial survivals of transfemoral leads were 87.6%, 73.8%, and 31.8%, respectively. The mean lead survival was 97 months. Overall, 105 reoperations were performed, 38 for battery depletion, 24 for body growth, 14 for infection or pocket revisions, and 27 for miscellaneous reasons. In conclusion, the durability and overall long-term performance of transfemoral leads were excellent.

FILHO, M.M., ET AL.: Probability of Occurrence of Life-Threatening Ventricular Arrhythmias in Chagas' Disease versus Non-Chagas' Disease. The implantable cardioverter defibrillator (ICD) is highly effective in the treatment of ventricular arrhythmias (VA) responsible for sudden cardiac death. However, the prob ability of occurrence of these arrhythmic events in presence of cardiomyopathy remains uncertain. The aim of this study was to compare the probability of nonoccurrence of life-threatening VA in ICD recipients with Chagas' versus non-Chagas' heart disease. Over a mean follow-up of 10.5 months, 53 ICD recipients (mean age = 50.1 years, 48 male) were evaluated. Eleven patients had Chagas' heart disease, 19 had idiopathic dilated cardiomyopathy and 23 had ischémie cardiomyopathy. Ventricular tachyarrhythmias with a cycle length < 315 ms were considered life-threatening. The cumulative probability of nonoccur rence of life-threatening VA was examined by Kaplan-Meyer method and the outcomes were submitted to the log rank test. At 2 years, the cumulative probability of life-threatening VA nonoccurrence was 0 in the Chagas' heart disease group versus 40% up to 55 months of follow-up in the non-Chagas' disease group (P = 0.0097). Among patients with cardiomyopathies of different etiologies, those with Chagas' heart dis ease had the lowest cumulative probability of nonoccurrence of life-threatening VA, confirming its unfa vorable prognosis and the importance of preventive measures against sudden death in this disease. The most probable causes of recurrent syncope were VT (43%) with poor prognosis, and paroxysmal atrioventricular block (21%) with a favorable prognosis. Absence of congestive heart failure, complexity of premature ventricular complexes by Holter, and absence of intraventricular heart block showed statistical correlation with normal electrophysiologic study, inducible VT, and normal HV interval, respectively.

ApresentaçãoAs diretrizes para o diagnóstico e tratamento das arritmias cardíacas surgiram a partir da filosofia da Sociedade Brasileira de Cardiologia (SBC) de manter atualizadas as informações de procedimentos relacionados ao diagnóstico e tratamento de pacientes com cardiopatias.O Departamento de Arritmias e Eletrofisiologia Clínica (DAEC) decidiu realizar um documento mais amplo que nos consensos realizados anteriormente, revisando criticamente, não apenas as informações das especialidades intervencionistas, como a eletrofisiologia invasiva e a estimulação cardíaca artificial, mas propondo diretrizes para temas anteriormente não abordados, de especial interesse dos cardiologistas clínicos, como as indicações e interpretações dos métodos não invasivos e o tratamento clínico das arritmias cardíacas.Nestas diretrizes, que contaram com a participação de 50 especialistas considerados os mais destacados nas respectivas áreas, procuramos estabelecer normas técnicas para a realização dos procedimentos intervencionistas, como qualificação e habilitação dos recursos humanos, condições de segurança dos serviços e equipamentos indispensáveis para realização destes procedimentos.
