O presente estudo objetivou investigar a forma como os usuários dos serviços públicos de saúde em Natal, Rio Grande do Norte, Brasil, "dão sentido" à sua situação de "paciente", interessando basicamente três aspectos: (1) a avaliação dos serviços recebidos; (2) a consciência de seus direitos como pacientes e seu papel na melhoria desses serviços; (3) as expectativas com relação ao encontro com o profissional de saúde. Os resultados apontam para as inúmeras deficiências encontradas no setor saúde e para a passividade e resignação da população em face do quadro atual. Reflete-se, ainda, sobre a necessidade de se valorizar a subjetividade do usuário, bem como sobre a relação profissional-paciente/cliente como um momento importante no processo de "humanização" das práticas de saúde. Esse reconhecimento deve possibilitar a construção, pelo usuário, de uma consciência crítica acerca de seus direitos e do seu papel como agente de mudanças no sistema de saúde.
RESUMO. Esse trabalho constitui uma reflexão sobre as práticas de saúde e as dificuldades da psicologia de se inserir como campo interdisciplinar de intervenção nessa área. Apesar de muito se falar do modelo biopsicossocial, observa-se que se continua privilegiando a etiologia biologicista, a concepção fragmentada de saúde e o caráter impositivo e normatizador da visão positivista de ciência, esquecendo-se a relevância dos aspectos sociais, psicológicos e ecológicos como mediadores dos processos saúde-doença. O modelo biomédico mostra-se totalmente funcional para as necessidades do sistema socioeconômico vigente, condicionando uma visão individualista, fragmentada e descontextualizada dos comportamentos humanos relacionados à saúde e à doença, pelo que se sugere um permanente processo de deconstrução sobre as formas vigentes de fazer ciência nessa área. Isso implica uma revisão crítica do desenvolvimento dos processos de significação relacionados com a saúde-doença e com a formação profissional, bem como da própria estruturação da saúde pública e dos modelos assistenciais implementados ao longo de sua história, implícita ou explicitamente dirigida para a população carente. Pressupõe, ainda, uma posição de permanente autocrítica voltada a nossa participação nesses processos, como profissionais. Palavras-chaves: psicologia social, saúde, modelo biomédico.ABSTRACT. The aim of this paper is, first, to research ideas, values and discourses mediating the health practices and the biomedical model of health, and, second, to investigate the difficulties Psychology has had for being considered an intervention approach in this area, from a critical psychosocial perspective. Although the bio-psychosocial model has been widely mentioned during the last decades, the practice in this field has privileged a biological aetiology, a fragmented conception of health and the enforcing and normalizing features from the positivist conception of science. The relevance of social, psychological and ecological aspects mediating the health-illness process is generally forgotten. The main issue is that the biomedical model continues to be accepted by the socio economical system, conditioning a fragmented, individualistic and de-contextualised conception of behaviours and actions related to health and illness. Thus it is necessary a permanent deconstructing process about the actual way of dealing with science in this area. This implies a critical revision of the development of the meaning process related with health and illness, the training of health professionals, as well as the public health structure and the assistance models implemented historically, implicitly or explicitly, addressed to the poor people. This also presupposes a permanent self-critical position in relation to our participation in these processes as professionals.
Apesar de ser tema recorrente no cotidiano, a promoção da saúde apresenta-se como um conceito complexo e multifacetado. O objetivo do presente trabalho é destacar alguns dos dilemas e aspectos problemáticos nas concepções e idéias sobre a temática. Enfatiza-se, também, a dificuldade de pensar a promoção da saúde sem considerar formas de erradicar as profundas iniqüidades sociais vigentes no contexto brasileiro. Aprofunda-se, ainda, essa reflexão no âmbito da Política Nacional de Promoção de Saúde, instituída em 2006, mostrando a relevância da desconstrução dos discursos politicamente corretos e o desenvolvimento dos processos de reflexão sobre as formas de atuar no campo da saúde.
BackgroundCervical cancer is highly preventable and treatable if detected early through regular screening. Women in the Canadian province of Newfoundland & Labrador have relatively low rates of cervical cancer screening, with rates of around 40 % between 2007 and 2009. Persistent infection with oncogenic human papillomavirus (HPV) is a necessary cause for the development of cervical cancer, and HPV testing, including self-sampling, has been suggested as an alternative method of cervical cancer screening that may alleviate some barriers to screening. Our objective was to determine whether offering self-collected HPV testing screening increased cervical cancer screening rates in rural communities.MethodsDuring the 2-year study, three community-based cohorts were assigned to receive either i) a cervical cancer education campaign with the option of HPV testing; ii) an educational campaign alone; iii) or no intervention. Self-collection kits were offered to eligible women at family medicine clinics and community centres, and participants were surveyed to determine their acceptance of the HPV self-collection kit. Paired proportions testing for before-after studies was used to determine differences in screening rates from baseline, and Chi Square analysis of three dimensional 2 × 2 × 2 tables compared the change between communities.ResultsCervical cancer screening increased by 15.2 % (p < 0.001) to 67.4 % in the community where self-collection was available, versus a 2.9 % increase (p = 0.07) in the community that received educational campaigns and 8.5 % in the community with no intervention (p = 0.193). The difference in change in rates was statistically significant between communities A and B (p < 0.001) but not between communities A and C (p = 0.193). The response rate was low, with only 9.5 % (168/1760) of eligible women opting to self-collect for HPV testing. Of the women who completed self-collection, 15.5 % (26) had not had a Pap smear in the last 3 years, and 88.7 % reported that they were somewhat or very satisfied with self-collection.ConclusionsOffering self-collected HPV testing increased the cervical cancer screening rate in a rural NL community. Women who completed self-collection had generally positive feelings about the experience. Offering HPV self-collection may increase screening compliance, particularly among women who do not present for routine Pap smears.
Universidade Federal do Rio Grande do Norte _____________________________________________________________________________________ RESUMO:A partir de uma visão unidimensional da saúde, a tendência das instituições sociais tem sido garantir a sobrevivência de crianças e adolescentes, cuidando dos denominados problemas orgânicos, desconsiderando os aspectos psicológicos e sócio-ambientais. O trabalho de pesquisa-intervenção num bairro da periferia de Natal, RN, possibilitou a identificação de um contexto de vida perpassado pela precariedade de serviços de infra-estrutura, altos índices de evasão escolar, relações de gênero opressivas e, ausência para ambos os sexos, de perspectivas e oportunidades acadêmicas e profissionais. Tal situação limita a consolidação de projetos de vida que transcendam a inserção no mercado informal de trabalho e o desempenho dos tradicionais papéis sexuais, gerando um ciclo de reprodução da pobreza que tende a se perpetuar. Almeja-se, portanto, a revisão crítica das idéias e valores que estão permeando a vivência da adolescência entre a população alvo e suas famílias, bem como entre os profissionais de educação e saúde que trabalham no bairro, inclusive a equipe da Universidade.
