Objetivo: determinar la prevalencia de lesiones escamosas intraepiteliales (LEI) y malignidad de casos referidos con hallazgos citológicos de "atipias escamosas de significado indeterminado" o "atipias escamosas que no permiten excluir lesión de alto grado" (ASC-US o ASC-H, por sus siglas en inglés, respectivamente). Metodología: se seleccionaron los informes de las pacientes referidas con interpretación citológica de ASC-US o ASC-H y se determinó el resultado del estudio histopatológico de la biopsia de cérvix, en el primer programa de promoción y prevención de cáncer de cuello uterino de la EPS Seguro Social, seccional Cundinamarca y Bogotá, entre junio del 2004 y septiembre del 2005. Se analizaron además las variables de edad y tipo de muestra.Resultados: se encontraron 944 casos, 931 remitidos como ASC-US y 13 como ASC-H, con edades entre 16 y 87 años; se incluyeron 939: 294 (31,3%) casos correspondieron a LEI o malignidad: 228 (24,28%) tuvieron LEI de bajo grado (BG), 62 (6,6%) LEI de alto grado (AG), 3 (0,32%) carcinomas escamocelulares y 1 (0,11%) adenocarcinoma. De los trece casos con ASC-H, 3 (23,1%) correspondieron a LEI-AG y 3 (23,1%) LEI-BG.Conclusiones: los hallazgos citológicos ASC-US o ASC-H en esta serie incluyen una proporción significativa de mujeres con riesgo de tener LEI o malignidad y permiten seleccionar un grupo particular de casos que ameritan un abordaje específico para definir su diagnóstico.
