OBJECTIVE To assess whether sex, education level, and health insurance affect the use of health services among the adult Brazilian population with chronic noncommunicable diseases (NCD).METHODS Data from a cross-sectional survey were analyzed, the National Health Survey (PNS). Frequency of use of services in the population that referred at least one NCD were compared with the frequency from a population that did not report NCD, according to sex, education level, health insurance, and NCD number (1, 2, 3, 4, or more). The prevalence and prevalence ratios were calculated crude and adjusted for sex, age, region, and 95% confidence intervals.RESULTS The presence of a noncommunicable disease was associated with increase in hospitalizations in the last 12 months, in 1.7 times (95%CI 1.53–1.9). Failing to perform usual activities in the last two weeks for health reasons was 3.1 times higher in NCD carriers (95%CI 2.78–3.46); while the prevalence of medical consultation in the last 12 months was 1.26 times higher (95%CI 1.24–1.28). NCD carriers make more use of health services, as well as women, people with higher number of comorbidities, with health insurance, and higher education level.CONCLUSIONS NCD carriers make more use of health services, as well as women, people with higher number of comorbidities, with health insurance, and higher education level.
Este artigo apresenta os principais resultados da Pesquisa Nacional de Saúde do Escolar (PeNSE). Aplicou-se questionário em uma amostra de conglomerados de 60.973 estudantes do 9º ano do ensino fundamental de escolas públicas e privadas das capitais dos estados brasileiros e do Distrito Federal, entre março e junho de 2009. Analisam-se prevalências e intervalos de confiança de 95% (IC95%) das situações de violência envolvendo adolescentes. Foram identificadas as seguintes situações: insegurança no trajeto casa-escola (6,4%; IC95%: 6,1%-6,8%) e na escola (5,5%; IC95%: 5,2%-5,8%); envolvimento em brigas com agressão física (12,9%; IC95%: 12,4%-13,4%), com arma branca (6,1%; IC95%: 5,7%-6,4%) ou arma de fogo (4,0%; IC95%: 3,7%-4,3%); agressão física por familiar (9,5%; IC95%: 9,1%-9,9%). As situações de violência foram mais prevalentes entre estudantes do sexo masculino. Houve grande variação segundo as cidades estudadas. Os adolescentes estão expostos a diferentes manifestações de violência nas instituições que supostamente deveriam garantir sua proteção e desenvolvimento saudável e seguro - a escola e o lar. Esses resultados visam apoiar medidas de promoção à saúde e prevenção desses fatores de risco.
This article examines progress made towards the implementation of the core priorities laid out in the National Health Promotion Policy (PNPS, acronym in Portuguese) and current challenges, highlighting aspects that are essential to ensuring the sustainability of this policy in times of crisis. It consists of a narrative review drawing on published research and official government documents. The PNPS was approved in 2006 and revised in 2014 and emphasizes the importance of social determinants of health and the adoption of an intersectoral approach to health promotion based on shared responsibility networks aimed at improving quality of life. Progress has been made across all core priorities: tackling the use of tobacco and its derivatives; tackling alcohol and other drug abuse; promoting safe and sustainable mobility; adequate and healthy food; physical activity; promoting a culture of peace and human rights; and promoting sustainable development. However, this progress is seriously threatened by the grave political, economic and institutional crisis that plagues the country, notably budget cuts and a spending cap that limits public spending for the next 20 years imposed by Constitutional Amendment Nº 95, painting a future full of uncertainties.
O objetivo do estudo foi descrever notificações de violência contra os idosos (> 60 anos) captadas pelo Sistema de Informação de Agravos de Notificação - versão net (Sinan Net) em 2010. Realizou-se estudo descritivo, retrospectivo, com dados analisados no Stata versão 11. Estimaram-se razões de proporção (RP) de violência segundo variáveis selecionadas. Das 3.593 notificações de violência contra idosos, 52,3% eram referentes ao sexo feminino. A violência física foi significativamente mais frequente no sexo masculino (RP = 0,82), no grupo com 60 a 69 anos, fora do domicílio, praticada por agressores que não eram filhos, com suspeita de ingestão de bebida alcoólica. A violência psicológica foi mais frequente entre idosas (RP = 2,17), no domicílio, infligida pelos filhos, com suspeita de uso de bebida alcoólica e de maneira crônica. A negligência predominou no sexo feminino (RP = 1,24), no grupo a partir de 70 anos, no domicílio, perpetrada pelos filhos e recorrente. A violência sexual foi mais comum no sexo feminino (RP = 5,21), por agressores que não eram filhos, mas que consumiram bebida alcoólica. O conhecimento das diferentes manifestações da violência contra idosos subsidia ações para o seu enfrentamento, identificando características de vulnerabilidade onde as redes de apoio podem intervir.
The study shows the importance of external causes among young people and men as a cause of premature death and disabilities, which is a priority problem in the country. The Global Burden of Disease study may support public policies for violence prevention.
