-Insular epilepsy has been rarely reported and its clinical and electrographic features are poorly understood. The electrographic study of the insula is difficult since it is hidden from the brain surface by the frontal and temporal lobe. A 48 years-old woman started having simple partial autonomic and complex partial seizures with automatisms and ictal left arm paresis 8 years prior to admission. Seizure's frequency was 1 per week. Pre-operative EEG showed a right temporal lobe focus. Neuropsychological testing disclosed right frontotemporal dysfunction. MRI showed a right anterior insular cavernous angioma. Intraoperative ECoG obtained after spliting of the sylvian fissure showed independent spiking from the insula and temporal lobe and insular spikes that spread to the temporal lobe. The cavernous angioma and the surrounding gliotic tissue were removed and the temporal lobe was left in place. Post-resection ECoG still disclosed independent temporal and insular spiking with a lower frequency. The patient has been seizure-free since surgery. Insular epilepsy may share many clinical and electroencephalographic features with temporal lobe epilepsy.KEY WORDS: epilepsy, insula, electrocorticography, cavernous angioma.Epilepsia insular: similaridades à epilepsia do lobo temporal -relato de caso RESUMO -A epilepsia insular tem sido raramente relatada e suas características clínicas e eletrencefalográficas são pobremente conhecidas. O estudo eletrográfico da ínsula é difícil já que ela se encontra recoberta pelos lobos frontal e temporal. Uma paciente, de 48 anos, começou a ter crises parciais simples autonômicas e crises parciais complexas com automatismos e paresia crítica de membro superior esquerdo 8 anos antes desta internação. A frequência de crises era de 1/semana . O EEG pré-operatório mostrou foco temporal direito. Testagem neuropsicológica demonstrou disfunção fronto-temporal direita. RMN demonstrou cavernoma insular anterior direito. A eletrocorticografia intraoperatória obtida após a abertura da fissura sylviana demonstrou a presença de espículas independentes na ínsula e no lobo temporal, além de descargas que se originavam na ínsula e espraiavam ao lobo temporal. O angioma cavernoso e a área gliótica ao seu redor foram removidos e o lobo temporal foi deixado em seu lugar. A eletrocorticografia após a ressecção ainda demonstrou a presença de descargas nestas regiões, em menor frequência. A paciente está sem crises desde a cirurgia. A epilepsia insular pode compartilhar diversos aspectos clínicos e eletrográficos com a epilepsia do lobo temporal. O pseudoaneurisma de artéria mesentérica superior é uma doença rara, porém com alta taxa de ruptura e mortalidade. Sua etiologia geralmente é infecciosa e comumente o diagnóstico é feito por meio de achado nos exames de imagem. Historicamente, seu tratamento de eleição tem sido o reparo cirúrgico aberto; entretanto, é associado a inúmeras complicações e dificuldades técnicas. Relatamos um caso de pseudoaneurisma de artéria mesentérica superior em um paciente portador de abscesso hepático no qual, após resolução do processo infeccioso, foi empregado, com sucesso, um tratamento minimamente invasivo endovascular, com implante de micromolas e stent não-recoberto.
-We evaluate the results of stenting and angioplasty on carotid bifurcation stenotic lesions using protection systems, emphasizing the indications and technical aspects. Seventy-nine patients, mean age 64.5 years were treated from February,1998 to March, 2003. All patients were included in NASCET study criteria. Forty three patients were treated without the protection systems and thirty six were treated with carotid protection filtering system (Angioguard, EPI). Technical success and 6-months carotid Doppler ultrasound follow-up showing stent patency were achieved in all patients. One major stroke and one death due to intracranial reperfusion bleeding occurred in patients treated without cerebral protection devices. Only one patient presenting hyper perfusion syndrome improving after 7 days, was found in the group treated with the cerebral protection filter mechanism, no other neurologic symptom or death occured in this group. Stenting and angioplasty with protection systems for thromboembolic debris is a safe endovascular method to treat stenotic lesions in the carotid bifurcation with low morbidity and mortality.KEY WORDS: carotid artery stent, carotid artery atherosclerosis, endovascular therapy.Tratamento endovascular das lesões estenóticas em bifurcação carotídea: estudo retrospectivo de 79 pacientes tratados por "stent" e angioplastia com e sem mecanismos de proteção cerebral RESUMO -Este estudo mostra a experiência e resultados da terapêutica endovascular nas lesões estenóti-cas em bifurcação carotídea. Entre fevereiro de 1998 e março de 2003 foram tratados 79 pacientes com idades entre 57 e 72 anos (média 64,5 anos) sendo 45 do masculino e 34 do feminino. Todos os pacientes enquadravam-se nos critérios do estudo "NASCET" com comprometimento das artérias carótidas internas. Dos 79 pacientes, 43 foram tratados sem os sistemas de proteção e 36 pacientes foram tratados com sistema de proteção. Dos 36 pacientes tratados com sistema de proteção foram utilizados filtros de proteção (Angioguard e EPI). Observou-se melhora angiográfica em todos os 79 pacientes tratados. Ultrasom Doppler realizado após 6 meses mostrou artérias pérvias em todos os pacientes. Nos 43 pacientes tratados por "stent"/angioplastia sem a utilização de sistemas de proteção tivemos um paciente com déficit neurológi-co que perdurou por mais de 30 dias ("stroke major") e um óbito por transformação hemorrágica devido à reperfusão. Ocorreu síndrome de hiperperfusão em 1 paciente dos 36 pacientes tratados com o sistema de proteção, regredindo em 7 dias, sendo que nos demais não houve morbi-mortalidade. A angioplastia e "stent" no território carotídeo com a utilização de sistema de proteção é um método seguro com reduzida morbidade e mortalidade estando indicada nas lesões estenóticas das bifurcações carotídeas. -The classical imaging gold-standard for this diagnosis is the presence of tumor lateral to the carotid artery. Seventeen patients with pituitary macroadenomas with intraoperative confirmation of cavernous sinus invasion were studied with MRI. Only 8 patients had tumor lateral to the carotid artery; 13 had tumor within the carotid syphon and all lacked the ring enhancement of the medial wall of the cavernous sinus. In 10 patients, widening of the posterior double leaflets of the cavernous sinus could be. All patients were operated by the transesphenoidal route. Only one patient was cured by surgery alone. Only 3 patients disclosing the above mentioned MRI features were identified in a series of 250 patients and did not have cavernous sinus invasion. The present criteria proved to be useful in the pre-operative diagnosis of cavenous sinus invasion and patients' counselling. Pre-operative diagnosis of cavernous sinus invasion of pituitary tumors has a great impact in the management of such patients.KEY WORDS: pituitary tumors, magnetic resonance image, cavernous sinus invasion. Invasão de seio cavernoso por tumores pituitários: correlação neurorradiológica e clínico-cirúrgicaRESUMO -O critério imagenológico clássico para diagnóstico de invasão de seio cavernoso é a presença de tumor lateralmente à carótida em cortes coronais. Estudamos 17 pacientes com macroadenomas hipofisários com ressonância compátivel a invasão de seio cavernoso e que tiveram esta invasão confirmada cirurgicamente. Somente 8 destes pacientes possuíam tumor lateralmente à carótida (critério clássico). Treze pacientes possuíam tumor localizado entre as porções intracavernosas da carótida e em todos havia ruptura da captação anelar de contraste no seio cavernoso. Dez pacientes possuíam abertura dos folhetos posteriores do seio cavernoso. Todos os pacientes foram tratados por via transesfenoidal. Somente um caso obteve cura endocrinológica. Somente 3 pacientes portadores de algum destes sinais imagenológicos foram identificados em uma série de cerca de 250 casos e não possuíam invasão de seio cavernoso. Os critérios acima parecem adequadamente permitir prever a invasão do seio. Este diagnóstico é relevante já que a maioria dos pacientes com invasão de seio cavernoso acaba realizando terapia complementar. RESUMO -Analisamos 15 casos de meduloblastoma em adultos (8 homens e 7 mulheres), com idade média de 23,7 anos (variação de 13 a 46) submetidos a ressecção cirúrgica no período de fevereiro de 1988 a outubro de 1995. Os tumores se localizavam no hemisfério cerebelar em 7 casos (1 com extensão supratentorial e outro atingindo a cisterna do ângulo pontocerebelar), no vermis e hemisférico em 4 e apenas no vermis em outros 4. A ressecção foi considerada radical em 7 casos, subtotal em 7 e parcial em um. Não houve mortalidade operatória. Medulloblastoma in adulthood: analysis of a casuistics and surgical resultsABSTRACT -We report on 15 cases of medulloblastoma of adult onset (8 male and 7 female) operated upon posterior fossa approach from February 1988 to October 1995. Tumors were localized in cerebellar hemisphere in 7 cases (one with extension to supratentorial notch and another case reaching the cerebello-pontine angle cistern), in vermis and hemisphere in four, only in vermis in another four. Resection was total in seven patients, subtotal in other seven, and partial in one.There was no operative mortality. Aspects regarding biological behavior, diagnosis, pathological findings, surgery and survival are discussed as well as prognostic factors.KEY WORDS: posterior cranial fossa, medulloblastoma, primitive neuroectodermic tumor, adult hood.Desde 1925, quando Bailey e Cushing 1 reconheceram e denominaram o meduloblastoma como um processo distinto dos gliomas e sarcomas, ele tem sido reconhecido como um tumor cerebelar maligno relacionado à faixa etária pediátrica 2,3 , com um pico de incidência aos 5 anos de idade 4 . Entretanto o seu aparecimento em adolescentes e adultos não é infreqüente. Em seu artigo original, Bailey e Cushing descreveram 9 pacientes com idades superiores a 18 anos, portadores de meduloblastoma, que possuíam semelhanças em relação ao comportamento clínico e aspecto histopatológico. O comportamento biológico do meduloblastoma do adulto bem como seus aspectos histopatológicos têm sido estudados, procurando-se estabelecer parâmetros para avaliação prognóstica 2,[4][5][6][7][8][9][10][11][12][13][14][15] . O padrão de resposta ao tratamento cirúrgico e radioterápico difere entre o grupo adulto e o pediátrico 2, 7-9,16 , com recorrência local mais frequente no primeiro 12 .O objetivo deste artigo é mostrar nossa experiência cirúrgica com meduloblastomas no adulto e enfatizar os fatores relativos ao comportamento clínico, diagnóstico, cirurgia, bem como a análise de complicações e sobrevida, ressaltando possíveis fatores prognósticos.
Mesial temporal sclerosis is the main pathological substrate present in refractory temporal lobe epilepsy and its presence is often related to the occurrence of febrile seizures in infancy. There is an on-going discussion on the nature of mesial temporal sclerosis as it related to epilepsy: cause or consequence. A previously normal child developed hyperosmolar coma after abdominal surgery at the age of 6. Three months afterwards he developed simple and complex partial seizures with an increasing frequency and refractory to multiple mono- and polytherapic drug regimens. He was evaluated for surgery at the age of 13. Ictal and interictal recordings showed left temporal lobe abnormalities. Early CT scanning suggested left temporal atrophy. MRI showed mesial temporal sclerosis. Neuropsychological testing showed verbal memory deficits and he passed a left carotid artery amytal injection. He was submitted to a cortico-amygdalo-hippocampectomy and has been seizure-free since then. The clinical data obtained from this patient suggest that at least in this case mesial temporal sclerosis would be related to the cause of epilepsy and not resultant from repeated seizure activity.
Background: Spinal subdural hematoma (SSDH) is a rare complication of lumbar discectomy. Here, the authors reviewed 10 articles concerning the etiology, clinical, diagnostic, and surgical management of SSDH. Case Description: A postoperative SSDH occurred following a lumbar microdiscectomy in an 80-year-old patient in the absence of a dural injury. Conclusion: SSDH is a rare complication of lumbar discectomy and may even occur without a dural fistula. Notably, magnetic resonance is the diagnostic study of choice to identify this pathology that may then be appropriately managed.
