РезюмеЦелью исследования явилось изучение взаимосвязи клинико функциональных показателей, артериальной ригидности (АР) и ремоде лирования левых отделов сердца у больных хронической обструктивной болезнью легких (ХОБЛ) и при сочетании ХОБЛ и ишемичес кой болезни сердца (ИБС). По результатам исследования показано, что пациенты с ХОБЛ тяжелого и крайне тяжелого течения, неза висимо от наличия ИБС, подвержены влиянию общих патогенетических факторов (курение, гипоксемия, системное воспаление), вызывающих ремоделирование сосудистой стенки. Избыточная АР существенно влияет на гемодинамику, ухудшая расслабление сер дечной мышцы, что приводит к нарушению диастолической функции и снижению сократительной способности левого желудочка (ЛЖ). У больных ХОБЛ тяжелого и крайне тяжелого течения, независимо от наличия ИБС, выявлены положительные корреляционные взаи мосвязи между уровнем маркеров системного воспаления, АР и нарушением систолической и диастолической функции ЛЖ. SummaryAim. The aim of this study was to evaluate a relationship between clinical and functional parameters, arterial stiffness and the left heart remodeling in patients with COPD and co morbid coronary artery disease (CAD). Methods. Fifty COPD patients with or without co morbid CAD were includ ed. This was a prospective observational 52 week study. Lung function was testing, non invasive arteriography, echocardiography were used. Oxygen saturation and cytokine (IL 6, IL 8, TNF α) were measured. Results. Severe and very severe COPD and CAD have common risk factors, such as smoking, hypoxemia, systemic inflammation, which can cause vascular wall remodeling.
