There is a running debate over the relationship between work and disease, namely the disease known as repetitive strain injury (RSI), or else work-related osteomuscular disturbances (in Portuguese DORT), as it was renamed in 1997 by the Instituto Nacional de Seguridade Social (INSS) in accordance with the Disability Assessment Norms. Standing out among the signs brought about by this new infirmity process is its restricted focus on the subjective aspects in the analysis of the disease. Underlying the INSS stance is the view of a sickness-prone subject and the notion of health as capacity for work. This paper analyzes the historical context which characterizes these health/work and body/disease notions. It also discusses the construction of the 'sick-subject' and the way in which such a subjectivizing process entraps the subject itself. Finally, we show the way in which the INSS in Rio de Janeiro has managed to stop this disease of its work-related characteristics, fighting and reducing the occurrences of RSI as well as the costs with work compensation claims.
Resumo Apresenta as bases da neuropsiquiatrização das Lesões por Esforços Repetitivos. Utiliza-se como referência à polêmica desenvolvida na Austrália, nos anos 80, versando sobre: a iatrogenia da doença e a simulação dos pacientes; as implicações sociopolíticas dos diagnósticos das doenças crônicas e os discursos psiquiátricos, utilizados para definir e marcar a predisposição nas doenças osteomusculares que atingem os trabalhadores. Considera-se que esses encaminhamentos montam uma rede de alianças que se constituíram em armadilhas para a negação do nexo no Instituto Nacional de Seguridade Social/RJ em consonância com a prática instaurada em outros países. Essas estratégias fundadas na redução de custos dos benefícios tendem a descaracterizar o elemento trabalho como componente fundamental no processo gerador dessas doenças.Abstract The bases of neuropsychiatrization of the Repetitive Strain Injury. It's used as a reference to the polemic developed in Australia, in the 80's, dealing with: the iatrogenicity of the disease and the simulation by the patients; the sociopolitical implications of the diagnoses of chronical diseases and the psychiatric discourses used to define and mark the predisposition in the osteomuscular diseases that afflict the workers. It is considered that such orientations build a net of alliances that formed traps for nexus denial in the National Institute of Social Security/RJ in accordance with the practice instituted in other countries. Such strategies based on the cost reduction of the benefits tend to decharacterize the work element as a fundamental component in the generating process of such diseases.
ResumoO texto toma como referência a classificação das Lesões por Esforços Repetitivos na categoria de doença relacionada ao trabalho, datada de 1997, quando o Instituto Nacional de Seguridade Social investia na mudança do nome LER para DORT (Distúrbios Osteomusculares Relacionados ao Trabalho). São discutidas as estratégias da negação do nexo como uma produção de silêncio do corpo que adoece em função do trabalho. Apresentam-se o contexto histórico da conceituação de corpo e de doença e as várias leituras que, ao se atualizarem, vão compondo e decompondo os movimentos de criação e/ou cristalização desse território. Finalmente, aponta-se a doença como resistência do corpo aos seus constrangimentos. Palavras-chave: LER; DORT; subjetividade; saúde do trabalhador; perícia médica.
