Introdução: O angioedema vibratório (AV) é um subtipo de urticária induzível crônica que se manifesta com pápulas eritematosas ou angioedema após exposição da pele à vibração. Como a condição é rara, as informações disponíveis são limitadas e dificultam o seu diagnóstico e tratamento.Objetivos: Analisar os mecanismos fisiopatológicos do angioedema vibratório e as suas implicações clínicas no tratamento através de uma revisão sistemática de literatura Métodos: A pesquisa foi realizada em bases de dados como PubMed e Biblioteca Virtual de Saúde (BVS), utilizando os descritores “vibratory” and “angioedema” e incluindo pesquisas/revisões sistemáticas sem limitação de data de publicação. Enquanto aos critérios de qualidade, foram incluídos na pesquisa: amostragem com reduzido viés, randomização adequada, diagnóstico do AV por escalas validadas.Resultados: Em toda a literatura, 30 artigos foram encontrados, apenas 13 estudos preencheram critérios, sendo 5 relatos de casos e 5 revisões sistemáticas e 3 estudos observacionais. A falta de ensaios clínicos, coortes e de outros estudos com evidências consideradas mais expressivas, foi o principal fator limitante. Observamos que o AV pode ser dividido em hereditário, adquirido ou ocupacional, e os estímulos identificados e relatados na literatura que induzem a esta patologia incluem desde um corte de grama e passeios a cavalo, até ao ato de sacudir objetos, por exemplo. Enquanto a sua fisiopatogenia, ela não é completamente esclarecida, mas pôde-se concluir que que há níveis elevados de histamina sérica e degranulação de mastócitos, bem como a mutação de ganho de função no gene ADGRE2 em alguns casos esporádicos. Em relação ao tratamento farmacológico, os anti-histamínicos e os corticosteróides ainda são os medicamentos utilizados para o tratamento atual do AV. Todavia, alguns pacientes têm respostas insatisfatórias a esse protocolo, e podem se beneficiar com antileucotrienos, testes de produção de tolerância, anti IgE e antidepressivos tricíclicos. Conclusão: É notório que há diferentes mecanismos envolvidos no desencadeamento do AV que não estão completamente esclarecidos. Sendo assim, torna-se necessário a realização de mais estudos para um melhor conhecimento da patologia e dos seus mecanismos imunes, para que se tenha um tratamento farmacológico satisfatório para os pacientes.
