Tumor glómico (sinonimia: glomangioma, glomangiomioma, angiomioneuroma, aneurisma neuromioarterial, vasculo-neuroma). Discusión El tumor glómico es un tumor vascular, benigno y raro. Se origina en el cuerpo glómico, que consiste en un aparato neuromioarterial, es decir, está compuesto por células nerviosas, células de músculo liso y estructuras vasculares. Se encuentra en la capa reticular que es la más profunda de la dermis, con distribución en todo el cuerpo, aunque alcanza las mayores concentraciones en las manos y los pies. Estructuralmente, consta de una arteriola aferente, una anastomosis arteriovenosa central tortuosa-también llamada canal de Sucquet-Hoyer-, una vena colectora, el retículo intraglomerular (que contiene células glómicas, fibras nerviosas no mielinizadas y células intersticiales) y una cápsula compuesta por retículo neurovascular. Las células glómicas son células musculares lisas especializadas que derivan de los pericitos de Zimmermann y son particularmente densas alrededor de estos espacios vasculares dilatados. Además, también pueden encontrarse fibras nerviosas y mastocitos aumentados en número. Se trata, por tanto, de una anastomosis arteriovenosa muy especializada. El cuerpo glómico es responsable de la termorregulación. Wood describió, por primera vez, los hallazgos clínicos asociados con el tumor glómico, en 1812. Presentó casos de nódulos subcutáneos dolorosos que se caracterizaban por dolor y sensibilidad a la temperatura, y que eran curables con escisión. En 1878, Kolaczek describe la lesión en los dedos de la mano con localización subungueal. Es Masson quien le dio su nombre, en 1924, haciendo la descripción completa del tumor y proponiendo su clasificación anatomopatológica. El origen del tumor, según Masson, estaría en el cuerpo glómico neuromioarterial descrito por Popoff, años más tarde. Desde el punto de vista histológico, el tumor glómico es un hamartoma, que se produce como resultado de la hiperplasia de los componentes del cuerpo glómico neuromioarterial y está constituido típicamente por tres componentes (Figura 10). Según el componente histológico predominante, pueden categorizarse tres diferentes tipos histológicos: 1. Mixoide o tipo I: también llamado glomangiomioma, con predominio del componente vascular y de células musculares lisas. Es el tipo menos frecuente (5%).
