El objetivo de este artículo es presentar la utilidad y aspectos prácticos de la posturografía dinámica computarizada en el diseño de un programa de rehabilitación vestibular. SUMMARY: Vestibular rehabilitation is essential in the treatment of unsteadiness as it works on compensation improving patient balance and quality of life. The objective of this article is to introduce the usefulness and practical aspects of computerized dynamic posturography in the vestibular rehabilitation program design.
Introducción y objetivo: La neuritis vestibular es una entidad de etiología desconocida, siendo el origen viral el más aceptado. La incidencia anual es de 3-15 casos/100.000 habitantes. El diagnóstico se basa en la clínica y la exploración, siendo fundamental el diagnóstico diferencial con patología central. En el tratamiento se ha demostrado una más rápida recuperación de la paresia canalicular con la administración de corticoides sistémicos y una mejoría en la compensación central con la rehabilitación vestibular. Desde 2018, en la sección de Otoneurología del HUBU utilizamos el siguiente protocolo para el diagnóstico y tratamiento de pacientes con neuritis vestibular. Método: Para el diagnóstico usamos los criterios de vestibulopatía unilateral aguda de la Clasificación Internacional de Desórdenes Vestibulares. Como pruebas complementarias se realiza e-HIT y RMN de base de cráneo. A todos los pacientes, salvo contraindicación, se les administran corticoides sistémicos (mg/Kg 10 días) y se les entregan ejercicios de Cawthorne y Cooksey. En aquellos casos que no mejoran, se programa una rehabilitación vestibular hospitalaria individualizada. Se realiza seguimiento con nueva anamnesis, exploración física y e-HIT al mes, 4 y 10 meses del diagnóstico. Hemos realizado un estudio retrospectivo de los pacientes diagnosticados de neuritis vestibular en los últimos 4 años, recogiendo los siguientes datos: edad, sexo, lado afectado, nistagmo, sintomatología, ganancia y presencia de sacadas en e-HIT, resultados de RMN, FRCV, antecedente de infección, ingreso hospitalario. Resultados: Revisamos 58 pacientes con diagnóstico de neuritis vestibular entre 2018 y 2021. 7 de ellos han sido excluidos por no completar seguimiento. 51 recibieron seguimiento de 10 meses (23 mujeres y 28 varones). La desaparición del nistagmo se produjo en el 25.5% de los pacientes en el primer mes; en 31.4% en el cuarto; en 35.3% en el décimo, y persistió en el 7.8%. En el e-HIT la ganancia se normalizó en el 33.3% en el primer mes; en 15.7% en el cuarto, y en 31.4% en el décimo, persistiendo disminuida en el 19.6%. Los síntomas desaparecieron en el 19.6% en el primer mes; en 33.3% en el cuarto; en 39.2% en el décimo, y persistió más allá del décimo mes en el 7.9% de los pacientes. Se realizó una rehabilitación hospitalaria con posturografía dinámica y ejercicios de suelo en 15 pacientes, experimentado todos ellos una gran mejoría sintomática. RMN se hizo en 42 pacientes, siendo en todos ellos normal. El 37.3% de los casos presentaba FRCV. El 9,8% relataba antecedentes de infección respiratoria/gastrointestinal previo al debut vestibular. Por último, 33 pacientes requirieron ingreso hospitalario, mientras que 18 recibieron tratamiento domiciliario. Conclusiones: 41 pacientes presentaron una recuperación completa de la función vestibular durante el seguimiento realizado. En 47 desaparecieron los síntomas, recuperando una calidad de vida similar a la previa. En 10 pacientes persistió una hipofunción vestibular, siendo todos ellos rehabilitados hospitalariamente, con importante mejoría clínica. Recientemente se ha demostrado que la compensación central es significativamente mejor en aquellos pacientes en los que se realiza una rehabilitación hospitalaria precoz, lo que nos plantea si estamos seleccionando adecuadamente su momento de inicio.
