Introducción y objetivo: La neuritis vestibular es una entidad de etiología desconocida, siendo el origen viral el más aceptado. La incidencia anual es de 3-15 casos/100.000 habitantes. El diagnóstico se basa en la clínica y la exploración, siendo fundamental el diagnóstico diferencial con patología central. En el tratamiento se ha demostrado una más rápida recuperación de la paresia canalicular con la administración de corticoides sistémicos y una mejoría en la compensación central con la rehabilitación vestibular. Desde 2018, en la sección de Otoneurología del HUBU utilizamos el siguiente protocolo para el diagnóstico y tratamiento de pacientes con neuritis vestibular. Método: Para el diagnóstico usamos los criterios de vestibulopatía unilateral aguda de la Clasificación Internacional de Desórdenes Vestibulares. Como pruebas complementarias se realiza e-HIT y RMN de base de cráneo. A todos los pacientes, salvo contraindicación, se les administran corticoides sistémicos (mg/Kg 10 días) y se les entregan ejercicios de Cawthorne y Cooksey. En aquellos casos que no mejoran, se programa una rehabilitación vestibular hospitalaria individualizada. Se realiza seguimiento con nueva anamnesis, exploración física y e-HIT al mes, 4 y 10 meses del diagnóstico. Hemos realizado un estudio retrospectivo de los pacientes diagnosticados de neuritis vestibular en los últimos 4 años, recogiendo los siguientes datos: edad, sexo, lado afectado, nistagmo, sintomatología, ganancia y presencia de sacadas en e-HIT, resultados de RMN, FRCV, antecedente de infección, ingreso hospitalario. Resultados: Revisamos 58 pacientes con diagnóstico de neuritis vestibular entre 2018 y 2021. 7 de ellos han sido excluidos por no completar seguimiento. 51 recibieron seguimiento de 10 meses (23 mujeres y 28 varones). La desaparición del nistagmo se produjo en el 25.5% de los pacientes en el primer mes; en 31.4% en el cuarto; en 35.3% en el décimo, y persistió en el 7.8%. En el e-HIT la ganancia se normalizó en el 33.3% en el primer mes; en 15.7% en el cuarto, y en 31.4% en el décimo, persistiendo disminuida en el 19.6%. Los síntomas desaparecieron en el 19.6% en el primer mes; en 33.3% en el cuarto; en 39.2% en el décimo, y persistió más allá del décimo mes en el 7.9% de los pacientes. Se realizó una rehabilitación hospitalaria con posturografía dinámica y ejercicios de suelo en 15 pacientes, experimentado todos ellos una gran mejoría sintomática. RMN se hizo en 42 pacientes, siendo en todos ellos normal. El 37.3% de los casos presentaba FRCV. El 9,8% relataba antecedentes de infección respiratoria/gastrointestinal previo al debut vestibular. Por último, 33 pacientes requirieron ingreso hospitalario, mientras que 18 recibieron tratamiento domiciliario. Conclusiones: 41 pacientes presentaron una recuperación completa de la función vestibular durante el seguimiento realizado. En 47 desaparecieron los síntomas, recuperando una calidad de vida similar a la previa. En 10 pacientes persistió una hipofunción vestibular, siendo todos ellos rehabilitados hospitalariamente, con importante mejoría clínica. Recientemente se ha demostrado que la compensación central es significativamente mejor en aquellos pacientes en los que se realiza una rehabilitación hospitalaria precoz, lo que nos plantea si estamos seleccionando adecuadamente su momento de inicio.
Introducción y objetivo: La disfunción tubárica obstructiva consiste en el fallo de la función ventilatoria de la trompa de Eustaquio sobre el oído medio debido a una alteración en su apertura, condicionando presiones negativas en caja timpánica. Según su duración se clasifica en aguda (<3 meses) y crónica (>3 meses). Su etiología es variada, su diagnóstico principalmente clínico y su tratamiento inicial médico. No obstante, para aquellos casos refractarios que cumplan criterios disponemos de un tratamiento quirúrgico que ha demostrado buenos resultados a corto y largo plazo: la dilatación tubárica con balón. En el servicio de Otorrinolaringología del Hospital Universitario de Burgos (HUBU) hemos realizado un estudio retrospectivo de las dilataciones tubáricas con balón que hemos llevado a cabo entre 2018 y 2021. Método: Se realizó seguimiento de todas ellas con revisiones al mes, 3 meses y 6 meses, utilizando la otoscopia a los 6 meses como criterio objetivo de mejoría. Se realizaron un total de 25 procedimientos en 22 pacientes, 11 hombres y 11 mujeres. 3 eran menores de 18 años, 6 entre 18 y 40 años, y 13 entre 41 y 65 años. Todos los pacientes incluidos tenían diagnóstico de disfunción tubárica; además 11 de ellos presentaban OMS asociada, 8 tenían OMC con perforación timpánica y 3 presentaban atelectasias timpánicas. El 69% de los pacientes había requerido drenajes transtimpánicos (DTT) en más de 2 ocasiones. De las 25 intervenciones, 14 fueron unilaterales y 10 bilaterales, sólo 1 no se pudo canalizar. En muchas de ellas se realizaron cirugías asociadas en la misma intervención (12 DTT, 11 miringoplastias, 3 radiofrecuencias y 1 resección de adenoides). Como complicaciones, se notificaron 2 otorragias, 2 otorreas y 1 epistaxis. Resultados: Se empleó como criterio objetivo de mejoría la otoscopia realizada a los 6 meses (entendiendo como mejoría la disminución o ausencia de: retracciones, atelectasias, signos indirectos de ocupación mucosa en caja timpánica, perforaciones, etc). El 16% no presentó mejoría en la otoscopia. El 48% presentó clara mejoría. El 36% no fue valorable por presentar DTT in situ todavía. Se observó que donde se obtuvieron mejores resultados fue en las dilataciones tubáricas asociadas a miringoplastias, realizadas para cierre de perforación o como refuerzo para impedir retracciones. Discusión/Conclusiones: La dilatación tubárica con balón es una técnica sencilla y con escasas complicaciones. Ha demostrado buenos resultados, sobre todo cuando se asocia a miringoplastia. La principal indicación de la técnica es la otitis media secretora y el barotrauma, cuando son refractarios a tratamiento médico. Lo más importante y a su vez más complicado es realizar un adecuado diagnóstico clínico, ya que de ello dependerán los resultados obtenidos.
Introducción y objetivo: Las glándulas salivales pueden estar afectadas por diferentes patologías no tumorales como la litiasis o la presencia de moco en los ductos, que impiden la correcta eliminación de la saliva, provocando inflamación glandular. Hasta la aparición de la sialoendoscopia, se tomaba una actitud expectante o se realizaba la extirpación de la glándula afectada. Esta nueva técnica mínimamente invasiva, permite tanto el diagnóstico como el tratamiento de la patología de las glándulas salivales, sin la exéresis de las mismas. A través de un endoscopio de pequeño calibre, se consigue la visualización directa del conducto salival, pudiendo conseguir en un mismo procedimiento ambos objetivos. El propósito de esta comunicación es presentar nuestra experiencia con esta técnica desde su implantación en el Hospital Universitario de Burgos (2019), hasta el momento actual (2022). Método: Se realiza un estudio descriptivo, en el que se analizan retrospectivamente los pacientes intervenidos mediante sialoendoscopia simple o en el contexto de un abordaje combinado entre marzo de 2019 y mayo de 2022. Se recogen los datos epidemiológicos más destacables, así como otras variables de interés como la glándula afectada, la etiología o las complicaciones del procedimiento, entre otras. Resultados: Durante estos 3 años se han realizado 34 procedimientos, a un total de 32 pacientes, de los cuales 14 son mujeres (44%) y 18 hombres (56%). La edad media en el momento de la intervención fue de 56,25 años. En el 70,6% de los casos la afectación fue unilateral y la glándula más frecuentemente afectada fue la submaxilar en un 52,9%. La etiología más frecuente fue la litiasis (18 casos en total). En 22 casos el abordaje fue exclusivamente endoscópico mientras que en 12 casos el abordaje intraoral fue combinado. No se realizó ningún abordaje combinado parotídeo. Solo 2 pacientes precisaron reintervención quirúrgica por persistencia de la sintomatología, y otra paciente mujer está pendiente de parotidectomía. Un 20,6% de los casos presentó complicaciones perioperatorias, pero ninguna de las complicaciones ha sido grave ni permanente. Discusión: La introducción de la sialoendoscopia ha supuesto un cambio en el manejo de la patología salival. El abordaje mínimamente invasivo permite realizar esta técnica mediante cirugía ambulatoria, con una recuperación postoperatoria más temprana. Como otras técnicas intervencionistas, tiene una curva de aprendizaje prolongada. El conocimiento y manejo previo de la cirugía endoscópica nasosinusal, así como la formación quirúrgica específica en esta técnica, facilitará la adaptación a este procedimiento. El uso de anestesia general en las primeras intervenciones permitirá desarrollar esta técnica más cómodamente y minimizar la curva de aprendizaje. Según vayamos adquiriendo experiencia, conseguiremos minimizar el tiempo quirúrgico, e incluso se podrían realizar un mayor número de sialoendoscopias bajo anestesia local. Conclusiones: La sialoendoscopia se puede considerar una técnica segura y efectiva que nos permite en un mismo tiempo el diagnóstico y tratamiento de la patología inflamatoria de las glándulas salivares, bien sea por litiasis o por otras causas. Además, reduce el número de sialoadenectomías realizadas, lo que supone menor tiempo de ingreso postoperatorio, con menores efectos adversos, de escasa gravedad.
