A higienização das mãos é o procedimento mais simples e eficaz na prevenção e controle das infecções relacionadas à assistência à saúde. Contudo, a adesão a esta prática é excessivamente baixa. Considera-se a infraestrutura inadequada dos serviços de saúde um dos fatores relacionados à baixa adesão. Diante disso, conduziu-se um estudo observacional e transversal para avaliar a infraestrutura hospitalar para a prática da higienização das mãos. Os sujeitos e a fonte de informação foram médicos, enfermeiros e a estrutura das unidades de um hospital da região noroeste do Paraná. Utilizou-se para a coleta de dados um instrumento estruturado e, para a análise, a estatística descritiva. Os insumos e equipamentos, tais como pias e dispensadores para a prática da higienização das mãos, eram limitados. Existem falhas na infraestrutura para a prática da higienização das mãos, o que pode comprometer a qualidade da assistência e a segurança do paciente.
RESUMODesenvolveu-se um estudo observacional prospectivo para avaliar o efeito de uma estratégia de promoção à higienização das mãos, na adesão dos profissionais da saúde, em uma Unidade de Terapia Intensiva (UTI). Utilizou-se um instrumento estruturado para os dados referentes à adesão à higienização das mãos dos sujeitos antes e após uma intervenção em uma UTI de um hospital da Região Noroeste do Paraná, em 2009. Para análise dos dados, mensurou-se a taxa de adesão pela razão entre o número de higienizações realizadas e o número de indicações que requeriam o procedimento. Utilizaram-se os testes qui-quadrado e o exato de Fisher para a comparação dos dados antes e após a intervenção. Houve aumento significativo na taxa de adesão global à higienização das mãos, de 21,7% para 28%; p=0.039, na categoria dos técnicos de enfermagem, na fricção antisséptica, antes do contato com o paciente e antes dos procedimentos limpos e assépticos. Em contraste, a adesão não melhorou entre os enfermeiros e médicos e nos momentos posteriores ao contato com o paciente. A intervenção com ênfase na promoção da higienização das mãos, associada ao incentivo à utilização de preparações alcoólicas, contribuiu para melhorar significativamente a adesão global à higienização das mãos pelos profissionais da saúde.Palavras-chave: Lavagem de Mãos. INTRODUÇÃOAs infecções relacionadas à assistência à saúde são consideradas um problema de saúde pública mundial, pois afetam a segurança do paciente, à medida que contribuem para prolongar as internações, aumentar a resistência dos microrganismos aos antimicrobianos, gerar custos adicionais aos pacientes, a seus familiares e aos serviços de saúde e elevar a mortalidade (1) . Em pacientes internados em Unidades de Terapia Intensiva (UTIs), acrescenta-se o risco de adquirir infecção em cinco a dez vezes em relação aos demais, dada a sua vulnerabilidade intrínseca e a exposição aos fatores de risco, que incluem os procedimentos invasivos, cirurgias complexas, medicamentos imunossupressores e antimicrobianos e as interações com a equipe de saúde e os fômites (2) . Para corroborar tais evidências, um estudo de revisão sobre infecções nosocomiais em UTIs destaca que o risco de infecção é diretamente proporcional à gravidade da doença, às condições nutricionais, à natureza dos procedimentos diagnósticos e /ou terapêuticos e ao tempo de internação (3) . Neste sentido, para prevenir e controlar as infecções relacionadas à assistência à saúde, a higienização das mãos é mundialmente reconhecida como o procedimento mais simples e eficaz no cumprimento desta função, pois muitas dessas infecções são transmitidas pelas mãos contaminadas dos profissionais da saúde durante a sua prática assistencial (1) . RESUMO Introdução: O treinamento em circuito é um modelo de treinamento resistido que permite uma variada combinação de sobrecarga e, por isso, requer mais informações para que se compreenda a demanda glicolítica anaeróbia durante sua execução. Objetivo: O objetivo foi comparar dois protocolos de treinamento com pesos, com (Tconv) e sem (Tcirc) pausa entre as execuções, quanto à resposta do lactato sanguíneo ([la]). Métodos: Onze homens (21,0 ± 2,3 anos; 76,7 ± 5,4 kg, 179,5 ± 7,0 cm) foram submetidos ao teste de repetição máxima. O Tcirc e o Tconv foram prescritos a 60%1RM, 12 repetições, sendo três passagens com pausa de 300 s para Tcirc e três séries de cada exercício com 180 s de pausa para o Tconv. Os exercícios que compuseram ambos os protocolos de treinamento foram: supino reto, cadeira extensora unilateral, peck-deck, mesa flexora, pulley alto, leg press 45º, remada horizontal e panturrilha no hack. O teste de ANOVA (Bonferroni post hoc) comparou o [la] no 1º, 3º e 5º minuto após as passagens no Tcirc e após cada série no Tconv. O teste t independente comparou as médias do pico de lactato entre Tcirc e Tconv. Em todas as análises adotou-se p ≤ 0,05. Resultados: Foram observadas diferenças para o [la] entre a 1ª (10,6 ± 1,0 mmol/l) e a 2ª passagem (13,5 ± 1,8 mmol/l, P = 0,01) e entre a 1ª e a 3ª passagem (15,0 ± 2,5 mmol/l, P < 0,01) no Tcirc. Durante Tconv, os maiores valores médios de [la] foram observadas nos exercícios pulley alto (11,2 ± 2,2 mmol/l) e leg press 45º (11,9 ± 2,6 mmol/l). Houve diferença (P < 0,01) ao comparar o pico de [la] após Tconv (12,8 ± 2,2 mmol/l) e Tcirc (15,9 ± 2,0 mmol/l). Conclusão: O Tcirc demanda maior resposta glicolítica, enfatizando sua efetividade no aumento da capacidade anaeróbia muscular. Além disso, a execução não intermitente do Tcirc pode explicar sua maior demanda glicolítica, uma vez que Tconv e Tcirc foram proporcionalmente delineados, quanto ao volume e carga do protocolo. Aims: The objective of this study was to assess the antimicrobial efficacy of alcohol‐based hand gels according to European Norm 1500 (EN 1500). Methods and Results: We assessed the antimicrobial efficacy of 12 alcohol‐based hand gels produced in Brazil, containing 70% w/w or v/v ethyl alcohol as the active ingredient, according to EN 1500, with a 30‐s application. In addition, 70% w/w ethyl alcohol and three alcohol‐based hand rubs commonly used in Europe and effective according to EN 1500 were also tested. Eight of 12 (67%) alcohol‐based hand gels produced in Brazil failed by EN 1500. In contrast, 70% w/w ethyl alcohol and European alcohol‐based hand rubs were approved by EN 1500. Conclusions: In this study, the majority of Brazilian alcohol‐based hand gels showed limited efficacy on hand hygiene within 30 s. Significance and Impact of the Study: The findings of this study may be used as an important argument to motivate Brazilian manufacturers to improve the antimicrobial efficacy of alcohol‐based hand gels, because it is prudent to suppose that alcohol‐based hand gels can be recommended for use in healthcare settings only if they show antimicrobial activity at least similar to that of alcohol‐based liquid preparations, including the traditional 70% w/w ethyl alcohol.
