Introdução: O aneurisma cerebral é a formação de uma dilatação em vasos cerebrais. Diversas formas de conduta estão descritas nas diretrizes de tratamento, sendo as mais comuns a clipagem e a embolização. Objetivo: Comparar epidemiologicamente clipagem e a embolização endoscópica como tratamento de aneurisma no Brasil, entre 2010 e 2020. Metodologia: Estudo analítico, observacional, longitudinal e retrospectivo. Foi realizada a estimativa de tendência da taxa de mortalidade de cada procedimento utilizando o método de regressão linear Prais-Winsten e o cálculo de razão de odds para comparação da variável óbito. Resultados: Foram gastos R$393 milhões na realização de embolização de aneurismas e R$99 milhões em clipagens. Para embolização, a taxa de mortalidade apresentou ritmo constante, enquanto para clipagens a tendência foi crescente. A análise de odds demonstrou maior risco de mortalidade para realização de clipagens. Conclusão: Os achados do estudo evidenciam um predomínio de tratamento endovascular no SUS, porém ele está associado a maiores gastos. Por ser um procedimento mais invasivo, a clipagem pode estar associada a um maior risco de mortalidade. Foi observada uma redução dos gastos despendidos com ambos os procedimentos ao longo do período analisado, sugerindo diminuição da realização dessas terapias no SUS.
Introdução A Síndrome de Guillan-Barré (SGB) é uma polirradiculoneuropatia inflamatória reportada, geralmente, após uma infecção viral. A SGB foi descrita em relatos de casos como decorrente da Doença do Coronavírus 2019 (COVID-19), embora não haja estudos que comprovem a relação. Com o início da campanha mundial de imunização contra a COVID-19, a SGB também tem sido apontada como um possível efeito adverso pós-vacinal. Descrição do caso Homem, 53 anos, iniciou quadro de paresia de membros inferiores (MMII) associada à parestesia em pés e mão direita. Evoluiu com 3 episódios de queda de própria altura, paresia de membros superiores (MMSS) e dor em queimação na região escapular direita e irradiação para região lombar. Queixou-se de retenção urinária e constipação intestinal. Relatou imunização contra a COVID-19 (BNT162b2/Pfizer) há 20 dias e início dos sintomas há 6 dias. Glasgow 15 na admissão, apresentou: paralisia facial de padrão nuclear em hemiface direita, desvio de rima bucal para a esquerda, disartria grave, força grau 3 em MMSS e grau 2 em MMII e arreflexia tetrassegmentar, orientação e sensibilidade preservadas. A hipótese diagnóstica é de SGB variante Miller-Fisher, com déficit do VII nervo craniano à direita, síndrome disautonômica e síndrome padrão de acometimento de segundo neurônio motor. Comentários Várias descrições já realizadas associam vacinas à patogênese da SGB, como a da Influenza H1N1 e a do pólio. Isto pode ser explicado pela resposta imune anormal às proteínas-alvo específicas contidas nos imunizantes, à semelhança de patógenos virais. Em relação à BNT162b2, poucos relatos são encontrados na literatura e, apesar de não ser possível descartar a hipótese, fatores apontam para baixa probabilidade de associação. Os fatores são: nenhum dos materiais imunogênicos adicionais é conhecido por desencadear SGB; diante da ausência de estudos prospectivos de alta qualidade não foi encontrada associação estatística entre a infecção por Sars-Cov-2 e a SGB; e a não similaridade entre os casos descritos (neste, o paciente de idade adulta média tomou apenas a 1ª dose e não manifestou sintomas infecciosos gastrointestinais ou respiratórios prodrômicos, enquanto outros pacientes pertenciam a diversas faixas etárias, variaram entre 1ª ou 2ª dose e, naqueles que tiveram pródromos, houve detecção de agentes conhecidamente desencadeadores de SGB). Contudo, este é o primeiro caso descrito de SGB variante Miller-Fisher na pós-vacinação por BNT162b2.
Objective: Breast cancer is one of the leading causes of death in women worldwide. Several studies highlight the correlation between delayed treatment and high mortality. However, few researchers have addressed the main reasons for long delays in the healthcare system. This study aims to analyze different aspects influencing the postponement of treatment in Brazil. Methodology: An ecological approach using the secondary data from the Oncology Panel of the Brazilian Unified Health System (SUS). The data were collected on February 13, 2020, and included the diagnoses of breast cancer (ICD-10: C50 and D05) from 2015 to 2020. The aspects analyzed included the waiting time for the first treatment, procedure, staging, and region. Anatomopathological diagnoses made after a surgical procedure were not included. Results: According to the Brazilian laws, oncological patients need to start treatment within 60 days (5). However, the data reveal that only 45.22% of breast cancer patients (stages 2–4) started the treatment within 60 days. This is more alarming when looking at the procedures. Over half of the patients submitted to chemotherapy and 76.8% submitted to radiotherapy received their treatment after 60 days from the diagnosis. Interestingly, our geographic analysis did not reveal significant disparities among regions. The lower-income regions had 49.01–57.07% of patients treated after the recommended time frame. The higher-income regions had values from 43.44% to 54.44%. Conclusion: Despite the knowledge that later treatments result in worse outcomes and the legal right of earlier treatment, our results show a current controversial frame in Brazil. Most patients take longer than recommended, and the worst scenario is for radiotherapy patients. No substantial differences were found among lower- and higher-income regions, although it is likely that further analysis with different approaches could be more sensitive to deny or confirm this assumption.
Background: Meningitis is an inflammatory process in the membranes that line the central nervous system (CNS). Objectives: To analyze the behavior of meningitis mortality rate by region of Brazil, between 2010 and 2019. Desing and setting: Analytical, observational and retrospective study. Deaths number from meningitis was recorded through the Mortality Information System (SIM) stratified by Brazilian region, and population data from IBGE. Methods: Mortality rate (MR) per 1 million inhabitants was calculated. MR trend in each region was provided by segmented linear regression. MR annual percentage changes (APCs) were adjusted with a 95% confidence interval (CI). Results: In this period, there were 10007 deaths from meningitis in Brazil. MR by region varied from 3.77 (North Region) to 6.03 (Southeast Region), while in Brazil it was 4.93. A decreasing trend is noticeable in Brazil MR (APC= -3.2; 95%CI= -3.9; -2.4). Central-West region had the largest drop (APC= - 4.1; 95%CI= - 7.9; -0.2), followed by Southeast. In contrast, South and North regions had stationary trend and Northeast had a double behavior: decreasing from 2010 to 2015 (APC= -7.7; 95%CI= -11.7; -3.5) and stationary from 2015 to 2019. Conclusions: Despite the country decreasing trend, these results are heterogeneous. Variables such as vaccination coverage, population adherence and socioeconomic status can influence meningitis MR trends. Thus, it is understood the need for programs aimed at disease treatment and prevention to reduce regional inequalities and meningitis mortality.
