El hemangioendotelioma epitelioide (HEE) es un tumor maligno posiblemente de origen vascular. Su localización usual es los tejidos blandos, mediastino, sistema nervioso central, pleura, pulmones, hígado y hueso. Sin embargo, es extremadamente rara su aparición en el corazón. Se presenta el caso de un paciente masculino de 67 años de edad, quien consulta por disnea progresiva, edema de miembros inferiores y hepatomegalia congestiva dolorosa. Se diagnóstica mediante biopsia HEE cardiaco primario. Posteriormente, se documenta recidiva tumoral, razón por la que fue evaluado por la junta médica de tumores que decidió iniciar quimioterapia (paclitaxel más doxorrubicina liposomal durante 6 ciclos). El paciente continúa en seguimiento por oncología con evidencia de enfermedad estable. El HEE cardiaco es un tumor muy raro. El tratamiento con paclitaxel más doxorrubicina es un esquema de quimioterapia que puede ser considerado en el manejo del HEE cardíaco primario avanzado o recurrente después de la cirugía cardíaca.
Objetivo: describir un caso de trasplante hepático en un paciente con resultado positivo en la prueba del coronavirus del síndrome respiratorio agudo grave de tipo 2 (SARS-CoV-2) con éxito en el postrasplante temprano, pero que desarrolló complicaciones asociadas a la inmunosupresión y trombosis portal sin una trombofilia identificada en un centro de alta complejidad de un país latinoamericano. Descripción del caso: paciente de 48 años con diagnóstico de cirrosis hepática secundaria a esteatohepatitis no alcohólica (NASH) complicada por varios episodios de ascitis portal hipertensiva y encefalopatía hepática, ingresada para trasplante hepático ortóptico. En los exámenes iniciales tuvo una prueba positiva para SARS-CoV-2 y era asintomático respiratorio. El trasplante se realizó con éxito luego de la autorización del comité de infección. Después del primer mes posoperatorio presentó diarrea, ascitis y daño renal agudo. Los niveles de tacrolimus en el reingreso fueron superiores a 10 ng/mL y hubo una mejoría clínica significativa con la suspensión del fármaco. Finalmente, el paciente requirió retrasplante por trombosis de la vena porta y de las venas suprahepáticas, aunque no se identificó la etiología. Conclusión: se describe uno de los primeros informes de trasplante de hígado en un paciente con recuperación reciente de COVID-19 y pruebas persistentemente positivas. En el postrasplante temprano hubo una buena respuesta; sin embargo, luego del primer mes presentó complicaciones relacionadas con la inmunosupresión. Este caso también plantea la posible asociación entre el SARS-CoV-2 y el desarrollo de trombosis en la circulación portal hepática.
Introducción: El hipotiroidismo refractario es una entidad en la cual, los pacientes requieren dosis elevadas de hormona tiroidea, persistiendo con los síntomas de hipotiroidismo y con niveles de hormona estimulante de tiroides (TSH) alta, representando un desafío para el clínico. La compresión de sus causas permite un mejor abordaje diagnóstico y así mismo evita la sobredosificación y efectos adversos de la misma. Objetivo: Presentar los conceptos clave que ayuden a identificar las causas de la refractariedad al tratamiento, incluyendo adherencia, malabsorción o incremento en la demanda de la hormona tiroidea. Métodos: Se realizo una búsqueda de los principales conceptos a acerca de las causas, el diagnóstico y tratamiento de hipotiroidismo refractario, de acuerdo con la revisión presentada en el congreso de la asociación colombiana de Medicina interna (ACMI) los días 13 y 14 de mayo de 2022. Resultados: La elevación persistente de TSH, en pacientes que reciben suplementación de hormona tiroidea, puede estar presente hasta un 30% de los pacientes que padecen hipotiroidismo. La causa más frecuente es la mala adherencia al tratamiento, seguido de la malabsorción e incremento en la demanda del medicamento. El abordaje diagnóstico debe ser secuencial para encontrar la causa, en algunos realizando estudios especializados para descartar malabsorción. Conclusiones: Identificar las causas de hipotiroidismo refractario al tratamiento son clave para tener un adecuado control sintomático y clínico de la patología, y del mismo modo evitar el empleo de dosis supraterapéuticas de levotiroxina, ya que está relacionado con efectos adversos mayores a nivel cardiovascular y óseo.
