We describe an elderly patient with left ventricular (LV) endocardial rupture with myocardial dissection and contained apical epicardial rupture following acute myocardial infarction in whom live/realtime three-dimensional transthoracic echocardiography (3DTTE) provided incremental value over traditional two-dimensional transthoracic echocardiography (2DTTE) in the assessment of this lesion. To the best of our knowledge, this has not been reported previously.
