Prior to 1985 the Food and Agriculture Organization (FAO) estimated global food crop contamination with mycotoxins to be 25%. The origin of this statement is largely unknown. To assess the rationale for it, the relevant literature was reviewed and data of around 500,000 analyses from the European Food Safety Authority and large global survey for aflatoxins, fumonisins, deoxynivalenol, T-2 and HT-2 toxins, zearalenone and ochratoxin A in cereals and nuts were examined. Using different thresholds, i.e. limit of detection, the lower and upper regulatory limits of European Union (EU) legislation and Codex Alimentarius standards, the mycotoxin occurrence was estimated. Impact of different aspects on uncertainty of the occurrence estimates presented in literature and related to our results are critically discussed. Current mycotoxin occurrence above the EU and Codex limits appears to confirm the FAO 25% estimate, while this figure greatly underestimates the occurrence above the detectable levels (up to 60-80%). The high occurrence is likely explained by a combination of the improved sensitivity of analytical methods and impact of climate change. It is of immense importance that the detectable levels are not overlooked as through diets, humans are exposed to mycotoxin mixtures which can induce combined adverse health effects.

Deoxynivalenol (DON) is a mycotoxin primarily produced by Fusarium fungi, occurring predominantly in cereal grains. Following the request of the European Commission, the CONTAM Panel assessed the risk to animal and human health related to DON, 3-acetyl-DON (3-Ac-DON), 15-acetyl-DON (15-Ac-DON) and DON-3-glucoside in food and feed. A total of 27,537, 13,892, 7,270 and 2,266 analytical data for DON, 3-Ac-DON, 15-Ac-DON and DON-3-glucoside, respectively, in food, feed and unprocessed grains collected from 2007 to 2014 were used. For human exposure, grains and grain-based products were main sources, whereas in farm and companion animals, cereal grains, cereal by-products and forage maize contributed most. DON is rapidly absorbed, distributed, and excreted. Since 3-Ac-DON and 15-Ac-DON are largely deacetylated and DON-3-glucoside cleaved in the intestines the same toxic effects as DON can be expected. The TDI of 1 lg/kg bw per day, that was established for DON based on reduced body weight gain in mice, was therefore used as a group-TDI for the sum of DON, 3-Ac-DON, 15-Ac-DON and DON-3-glucoside. In order to assess acute human health risk, epidemiological data from mycotoxicoses were assessed and a group-ARfD of 8 lg/kg bw per eating occasion was calculated. Estimates of acute dietary exposures were below this dose and did not raise a health concern in humans. The estimated mean chronic dietary exposure was above the group-TDI in infants, toddlers and other children, and at high exposure also in adolescents and adults, indicating a potential health concern. Based on estimated mean dietary concentrations in ruminants, poultry, rabbits, dogs and cats, most farmed fish species and horses, adverse effects are not expected. Alternaria toxins are secondary metabolites produced by fungi that can contaminate cereals, oilseeds, fruits and vegetables. The chronic dietary exposure to four individual Alternaria toxins was estimated using 15,563 analytical results/4,249 samples (3,648 on alternariol (AOH), 3,654 on alternariol monomethyl ether (AME), 4,168 on tenuazonic acid (TeA) and 4,093 on tentoxin (TEN)). Most of the analytical results were left‐censored data (92%), with only 1,290 quantified results (mainly on ‘Oilseeds’ and ‘Grain milling products’). The highest exposure to AOH was estimated in ‘Toddlers’, with the mean exposure between 3.8 and 71.6 ng/kg body weight (bw) per day (minimum lower bound–maximum upper bound, (LB–UB)) and the 95th percentile exposure between 11.4 and 270.5 ng/kg bw per day (LB–UB). Overall, ‘Fruit and fruit products’ were the most important contributors to the dietary exposure to AOH. The highest exposure to AME was estimated in ‘Toddlers’, with mean exposure between 3.4 and 38.8 ng/kg bw per day (LB–UB) and 95th percentile exposure between 10.3 and 97.3 ng/kg bw per day (LB–UB). Overall, the main contributors to the dietary exposure to AME were ‘Vegetable oil’ and ‘Pome fruits’ (pears). The highest exposure to TeA was estimated in ‘Toddlers’ with mean exposure between 100 and 1,614 ng/kg bw per day (LB–UB), and in ‘Infants’ for the 95th percentile exposure estimations (98–3,603 ng/kg bw per day, LB–UB). ‘Cereal‐based food for infants and young children’ was the main contributor in ‘Infants’ and ‘Toddlers’, and tomatoes and tomato‐based products in other age classes. The highest exposure to TEN was estimated in ‘Toddlers’, with the mean exposure between 1.6 and 33.4 ng/kg bw per day (LB–UB), and the 95th percentile exposure around 55 ng/kg bw per day (UB) in different age classes. ‘Fruiting vegetables’ (tomatoes) were the major contributor to the dietary exposure to TEN. Although based on limited data, vegetarians seem to have higher dietary exposure to Alternaria toxins than the general population.

This study assessed, for the first time, the mycotoxicological risks from consumption of complementary foods by infants and young children in Nigeria. Molds belonging to Aspergillus aculeatinus, A. flavus, A. luchuensis, A. tubingensis, A. welwitschiae and Geotrichum candidum were recovered from the complementary foods. Twenty-eight major mycotoxins and derivatives, and another 109 microbial metabolites including chloramphenicol (a bacterial metabolite), were quantified in 137 food samples by LC-MS/MS. Aflatoxins and fumonisins co-contaminated 42% of the cereal- and nut-based food samples, at mean concentrations exceeding the EU limits of 0.1 and 200 μg/kg set for processed baby foods by 300 and six times, respectively. Milk contained mainly beauvericin, chloramphenicol and zearalenone. The trichothecenes, T-2 and HT-2 toxins, were quantified only in infant formula and at levels three times above the EU indicative level of 15 μg/kg for baby food. Chronic exposure estimate to carcinogenic aflatoxin was high causing low margin of exposure (MOE). Exposures to other mycotoxins either exceeded the established reference values by several fold or revealed low MOEs, pointing to important health risks in this highly vulnerable population. The observed mycotoxin mixtures may further increase risks of adverse health outcomes of exposure; this warrants urgent advocacy and regulatory interventions.
