Contribution à l'étude d'une souche de Trypanosoma gambiense (Feo) isolée d'un cas humain caractérisé par une durée de plus de 20 ans de parasitémie cliniquement inappréciable Par Jacques LAPIERRE et Marguerite COSTE Cette souche de Trypanosome, isolée, en novembre 1961, d'un cas humain, présente la particularité d'avoir déterminé, chez une jeune Togolaise, une parasitémie apparem ment inoffensive pendant plus de 23 ans, et de s'être montrée résistante à toute chimio thérapie. Voici résumée l'histoire étonnante du cas Féo : Féo, âgée de 4 ans, est reconnue trypanosomée le 27 juillet 1938 (trypanosomée n° 1499, registre n° 10 du secteur spécial 1/2 de Pagouda). La malade est alors en première période : deux éléments par mm3, à la cellule de Nageotte ; albuminorachie : 0,19. La présence d'adé nopathies n'est pas mentionnée. Ce même jour, on découvre que la mère de Féo est également parasitée, tandis que le père, dépisté en 1931, est considéré comme guéri. De 1938 à 1948, l'infection lymphatico-sanguine persiste malgré des tentatives réitérées de stérilisation périphérique à l'aide de tous les Trypanocides connus, y compris les penta midines aromatiques. Contrastant avec cette chimio-résistance totale, il faut signaler l'intégrité complète du névraxe, prouvée par 27 ponctions lombaires subies au cours de ces dix années et ayant montré un liquide céphalo-rachidien constamment normal. L'examen général révèle des signes auscultatoires cardiaques, se bornant à un deuxième bruit claqué, quelques discrètes adéno pathies. un foie légèrement augmenté de volume, une éosinophilie variant de 9 à 15 % sans helminthiase intestinale. Le développement de l'enfant est sensiblement normal et les signes fonctionnels sont minimes. En mai 1949, au cours d'une nouvelle enquête de dépistage, Féo présente, dans le sang, le suc ganglionnaire et la moelle osseuse, des Trypanosomes en assez grande quantité, dont la morphologie particulière attire l'attention de Lapeyssonnie qui les décrit ainsi : « Corps cellulaire le plus souvent trapu, massif et fortement coloré par le Giemsa ; la partie posté rieure de la membrane ondulante en particulier retient fortement les teintes basiques et appa raît très épaisse ; aucun des spécimens observés ne présente de flagelle libre. »
