Bioelectrical impedance vector-analysis (BIVA) describes cell-mass, cell function and hydration status of an individual or a group. The goal of the present investigation was to provide bioelectrical impedance data for 525 male road cyclists (155 professionals, 79 elite, 59 elite-youth, and 232 amateurs) at the time of their optimal performance level. Data were plotted on the resistance-reactance (R-Xc) graph to characterize cyclists group vectors using BIVA. Compared to the general male population, the mean vector position of the road cyclists indicates a higher body cell mass (BCM) and phase angle (p<0.001). The vector position of the high-performance, compared to the amateur cyclists showed similar patterns with higher BCM and phase angles and higher reactance values for the high-performance athletes (p<0.001). The bio-impedance data were used to calculate the 50%, 75%, and 95% tolerance ellipses of each group of cyclists. The characteristic vector positions of the road cyclists indicate normal hydration and greater muscle mass and function of the high-performance cyclists compared to amateur cyclists and the normal population. The cyclists specific tolerance ellipses, particularly the high-performance cyclists might be used for classifying a cyclist according to the individual vector position and to define target vector regions for lower level cyclists.
Cervical spondylotic myelopathy is a well-known cause of disability among older people. A significant amount of these patients is asymptomatic. Once the symptoms start, the worsening may follow a progressive manner. We should suspect of spondylotic myelopathy in any individual over 55 years presenting progressive changes in gait or losing fine motor control of the upper limbs. Despite its frequent prevalence, this condition is still neglected and many times confused with other supratentorial lesions regarding diagnostic. Here we address some of most important aspects of this disease, calling attention to pathophysiology, the natural history, presentation, differential diagnosis, clinical assessment, and treatment.
O conhecimento sobre a COVID-19, a infecção pelo novo coronavírus (SARS-CoV-2), vem aumentando rapidamente à medida que a pandemia se alastra e o número de acometidos cresce de forma exponencial em nível global. Embora as vias de transmissão não estejam ainda absolutamente determinadas, está bem estabelecida à transmissão pessoa a pessoa. Contudo, ainda há muitas lacunas no conhecimento sobre a epidemiologia e o quadro clínico da Covid-19, incluindo período mais exato de incubação, possibilidade de transmissão a partir de portadores assintomáticos e índice de transmissibilidade. A preocupação com a infecção na gestação e suas repercussões para a gestante e para o concepto é grande e as evidências científicas sobre o comportamento da infecção nesse grupo são esparsas e o ritmo constante de informações novas e conflitantes tem sido um desafio geral para todas as especialidades médicas, incluindo a terapia intensiva neonatal. Ainda não está comprovada a transmissão vertical durante a gestação e nem no período neonatal, pela amamentação. Por outro lado, o consenso chinês é contrário à evidência disponível, ao afirmar que existiria a possibilidade de transmissão vertical do COVID-19 no período intrauterino, com o potencial de malformação, também durante o parto e na amamentação, portanto, deveria ser contraindicada, mesmo em casos apenas suspeitos. O objetivo do presente trabalho foi, por meio da literatura científica, baseada no período de 15 de dezembro de 2019 a 18 de abril de 2020, analisar e informar a situação atual dos conhecimentos dos potenciais efeitos adversos da COVID-19 no decurso da gravidez e nos recém-nascidos.
Introdução: Recém-nascidos acometidos pela taquipneia transitória do recém-nascido (TTRN) cursam com desequilíbrio da biomecânica toracoabdominal e desconforto respiratório e necessitam de cuidados especiais. A fisioterapia respiratória convencional em recém-nascidos tem efeitos adversos, podendo ser o método reequilíbrio toracoabdominal (RTA) uma alternativa terapêutica. Objetivo: Comparar a fisioterapia respiratória convencional com o método RTA em recém-nascidos com TTRN. Métodos: Ensaio clínico intervencional, comparativo e randomizado. 49 recém-nascidos foram divididos em dois grupos: fisioterapia respiratória convencional (n=20) e RTA (n=29). Parâmetros fisiológicos (frequência respiratória, frequência cardíaca, saturação de pulso de oxigênio, temperatura axila), dor, estado comportamental, desconforto respiratório e desequilíbrio da biomecânica respiratória foram avaliados antes e após os manuseios. Resultados: A mediana da idade gestacional foi 38 semanas e a do peso de nascimento 2.940 g. Após os manuseios, não houve diferença entre os grupos quanto à dor (p=0,63), o estado comportamental (p=0,11) e os parâmetros fisiológicos (frequência respiratória, p=0,18; frequência cardíaca, p=0,82; SpO2, p=0,74; temperatura axila, p=0,29). O método RTA mostrou-se superior a fisioterapia respiratória convencional na melhora da biomecânica respiratória (elevação do esterno, p=0,01; elevação dos ombros, p=0,02) e do desconforto respiratório (p=0,009). Conclusão: O método RTA mostrou-se seguro e superior à fisioterapia respiratória convencional em recém-nascidos com TTRN.Palavras-chave: recém-nascido, taquipneia transitória do recém-nascido, modalidades de fisioterapia.
Objective: make a review of the main neurological diseases associated with neuromuscular scoliosis and perioperative care during scoliosis correction surgeries. Method: Non-systematic review using a Google Scholar platform for articles with the descriptor "neuromuscular scoliosis" in the journal title. Results and Discussion: 46 articles in English and 3 articles in Portuguese were used for this review. Conclusion: Neuromuscular scoliosis has the fastest course of evolution, higher rates of disability and higher rates of complications in corrective surgery when compared to idiopathic scoliosis. The knowledge of neurological diseases more associated with neuromuscular scoliosis helps in its early detection and its long-term monitoring.I.
Introdução: A coagulação intravascular disseminada (CIVD) é, atualmente, definida como uma síndrome adquirida, caracterizada pela ativação difusa da coagulação intravascular, levando à formação e deposição de fibrina na microvasculatura. Há relatos de coagulopatia relacionada à Covid-19, doença pelo novo coronavírus SARS-CoV-2. Método: Atualização da Literatura acerca da associação entre Covid-19 e a presença de CIVD nas bases dados LIlacs, Bireme, Pubmed (inglês e português) no presente ano.Discussão: Alguns autores vêm estudando o papel da anticoagulação no manejo dos pacientes com infecção grave e (CIVD). Dados de inúmeras pesquisas sugerem efeitos benéficos do uso de anticoagulantes, o que vem sendo recomendado em algumas instituições no Brasil e no mundo. Vale ressaltar que a eficácia da medida ainda não foi validada. Conclusão: Até o presente momento não existem recomendações precisas em relação ao manejo dos pacientes com coagulopatia por Covid-19. Além disso, são excassos os dados na literatura que associam o uso de anticoagulantes ou antiplaquetários à ocorrência ou gravidade da Covid-19.
Introduction: The outbreak of COVID-19 among humans in Brazil and its spread around the world is strongly impacting mental health. Health problems such as anxiety, depression, insomnia and fear were the most related to. High levels of stress during the day can impair quality and time of sleep. Method: The purpose of this article was to update, through a bibliographic search, the effects of the pandemic on sleep disorders, with insomnia being the most related. A systematic search had been carried out in the main databases: Lilacs, Bireme and PubMed. The languages used in the search were Portuguese and English, from articles published this year. Using the following keywords: Sleep quality, COVID-19, Insomnia, Cognitive behavioral therapy. Discussion: The concern about financial and political uncertainties, feelings of impotence and fear of self-contamination and of family members are factors that corroborate with the worsening of the sleep quality of the population and health professionals’ working on the front lines of the fight against the pandemic, leading to increased cases of insomnia. Behavioral Cognitive Therapy, hypnotherapy and social interaction mediated by Web conferencing tools are the best ways to treat these symptoms at the moment. Even so, the brazilian literature on the subject is extremely scarce, making it an extremely relevant subject for future research.
